[Inverted papilloma of the renal pelvis].

A 60-year-old man with inverted papilloma of right renal pelvis is presented. On visit to hospital for benign prostatic hyperplasia with prostatic stone, he was found to have microscopic hematuria and intravenous pyelography and retrograde pyelography demonstrated a filling defect with papillary contour. We diagnosed a right renal pelvic tumor, which showed to be low grade because of negative cytology. Therefore, we performed tumor resection with renal preservation. Pathological diagnosis was inverted papilloma. From the Japanese and foreign literature, 16 cases of renal pelvic inverted papilloma were collected. They were reviewed clinically and pathologically, and the therapy was discussed.

[A case of nephrogenic adenoma in the female urethral diverticulum].

A case of nephrogenic adenoma in the female urethral diverticulum is reported. A 39-year-old woman visited our hospital with the chief complaint of perineal pain. Observation of her perineum and transvaginal digital examination revealed urethral diverticulum, and then diverticulectomy was performed. Histological diagnosis was urethral diverticulum associated with nephrogenic adenoma. In Japan, this is the 1st report of nephrogenic adenoma arising in the female urethral diverticulum. We reviewed 305 cases of nephrogenic adenoma of the urinary tract in the English and Japanese literature.

[Clinical application of Sonolith 3000 type on extracorporeal shock wave lithotripsy for renal and ureteral stones].

With Sonolith 3000, a new extracorporeal shock wave lithotriptor, we performed extracorporeal shock wave lithotripsy (ESWL) on 34 patients with upper urinary stones, including 5 patients with target stones of this clinical application in both sides. The 39 target stones in the 34 patients were located in the renal calyx (13 cases), renal pelvis (14 cases), renal calyx and pelvis (1 case), renal pelvis and ureter (1 case), and upper ureter (10 cases). The ultrasound-aiming system gave a satisfactory imaging of the target stones in 94.9% of cases (37/39 cases). During the ESWL treatment, 32 patients (94.2%) did not need anesthesia, but 2 patients (5.9%) needed epidural anesthesia because of intolerable pain and/or terror of pain. The average number of treatments per case was 2.08, and the average dose of shock waves per treatment was 3,691.1. The stone-free rate on the 14th, 42nd and 90th days after the last ESWL treatment were 28.2% (11/39), 51.3% (20/39) and 64.1% (25/39), respectively. Out of 39 cases, 31 cases (79.5%) were either stone-free or had only sand-like residual stones on the 90th day. Most cases had minimal side effects, such as transient macroscopic hematuria (100.0%), skin eruption (50.0%) and back pain (41.2%), but one case needed a 1,200 ml transfusion because of perirenal hematoma immediately after 1st session of ESWL treatment with 2,300 shock waves (13 kv). The serum total bilirubin level on 1st day after the last session of ESWL treatment was significantly increased when compared with the preoperative level, but the level was not significantly increased after the 1st session.(ABSTRACT TRUNCATED AT 250 WORDS)

[Primary carcinoma in situ of the ureter: a case report].

A case of primary carcinoma in situ of the ureter in a 77-year-old man is reported. The patient had been to another hospital with right flank pain and macroscopic hematuria. Ultrasound sonogram showed right hydronephrosis. An excretory urogram showed right hydronephrosis and stenosis of right ureter. He was referred to our hospital for further evaluation and treatment. Retrograde pyelogram demonstrated a right ureteral stricture at the level of S1-2, but no space occupying lesion was detected in the ureter. Cytology of voided urine was negative for malignant cells and no other abnormal findings were present. Probe laparotomy was performed under the preoperative diagnosis of ureteral stricture. During the operation, frozen section examination of the stenotic ureter showed carcinoma in situ and so we performed right total nephroureterectomy with a bladder cuff. Pathologic diagnosis was primary carcinoma in situ of the right ureter. The patient has been doing well for six months postoperatively with no evidence of recurrent or metastatic disease.

Spontaneous rupture of pyelocaliceal diverticulum.

One rare case of spontaneous rupture of pyelocaliceal diverticulum is reported in a 31-year-old Japanese man presented with a severe left flank pain. Intravenous pyelography and computerized tomography demonstrated extravasation of contrast medium from his diverticulum which contained numerous calculi. As a possible cause of spontaneous rupture of pyelocaliceal diverticulum in this case, we suggest that the paper-thin parenchyma covering the diverticulum was not strong enough to tolerate the minimum elevation of intrapelvic pressure due to the obstruction of delivered stones.

[Extramammary Paget's disease with a large mass in male genitalia: a case report].

A 57-year-old Japanese man presented with a mass 3 cm in diameter at the root of the penis. The patient had noticed the mass growing for 5 years, but had no pain or itching. Histological examination revealed it to be invasive Paget's disease, and the mass was resected along with skin of normal appearance within 3 cm around it. Regional lymph node metastasis was also revealed in the operation, and irradiation at the pelvic and inguinal region was done. Extramammary Paget's disease with a large mass is relatively rare, and invasive extramammary Paget's disease has a poor prognosis.

[A case of papillary cystadenoma of epididymis].

We present a case of papillary cystadenoma of the right epididymis in a 52-year-old man. He visited our hospital complaining of painless mass in his right scrotum. This painless hard mass was palpable at the head of his right epididymis and was 1 cm in diameter. Right epididymectomy was performed. A histological examination revealed epithelial proliferation of ectatic efferent ducts and microcysts filled with papillary processes, and a diagnosis of papillary cystadenoma of the right epididymis was made. In Japan, our present case is the 12th of papillary cystadenoma of epididymis. We chiefly reviewed the clinical aspects of this disease, especially the relationship between this disease and von Hippel-Lindau's disease.

[Congenital unilateral multicystic kidney in an adult].

Congenital unilateral multicystic kidney is relatively rare in adult (16% in adult, and 84% in children). Most of the adult cases are asymptomatic, and should be followed up conservatively, but nephrectomy was performed in many cases reported in Japan, because it was difficult to distinguish those cases from renal tuberculosis. Here we present one adult case, and discuss its clinical findings, diagnosis, treatment, etiology and embryology.

[Localized amyloidosis of urinary bladder: a case report].

A case of localized amyloidosis of the urinary bladder is reported. A 82-year-old woman visited our hospital with the chief complaint of miction pain and residual urine sensation. Cystoscopic examination revealed a broad-based and nonpapillary tumor without bleeding on the right lateral wall. A transurethral biopsy of this tumor was performed. A histopathological examination with H.E. and Congo red stains demonstrated amyloid deposition in the submucosal layers of the vesical wall. Rectal biopsy and other findings suggested no deposition of amyloid in other organs. On the basis of these findings, we made a diagnosis of localized amyloidosis of the urinary bladder. To the best of our knowledge, the present case is the 23rd of localized amyloidosis of urinary bladder in Japan. The patient was asymptomatic after biopsy. We discuss the clinical features and management of this disease.

[Hydronephrosis caused by Crohn's disease: a case report--review of 41 cases with urinary tract complication reported in Japan].

A 34 year-old man, who had been under care with diagnosis of Crohn's disease in the department of gastroenterology of our hospital since 1983, was referred to our urological clinic on May 21, 1987, because of right hydronephrosis found on ultrasonography. He did not complain of any urological symptoms. He underwent further roentgenographic examinations and a diagnosis of hydronephrosis complicated with Crohn's disease was made. On surgery of July 30, resection of ileocecal lesion, end-to-end ileocolostomy, right ureterolysis were performed. He is now visiting our clinic without recurrence of hydronephrosis up to present (7 months). In addition, we reviewed the 41 cases of urological disorders complicated with Crohn's disease in the literature reported in Japan. The cases were of vesicoenteric fistula including pericystitis (33 cases), hydronephrosis (10 cases), urolithiasis (2 cases) and ureterocolonic fistula (1 case). The patients were from 10 to 57 years (27.2 years in average) old. Sex distribution was uneven, 39 of patients were men and 2 were women. Urologic surgery was performed in almost all of the cases except for the patient with urolithiasis. All the patients having vesicoenteric fistula had urological complaints but the patients with hydronephrosis were relatively free from urological complaints. Thus, Crohn's disease has been recognized as an important gastrointestinal disease for urologists, and we will emphasize that periodical abdominal ultrasonography and urography for the patients with this disease should be necessary for checking up other complications such as hydronephrosis.

[A case of adrenal myelolipoma].

A case of adrenal myelolipoma is presented. The patient was a 61-year-old woman who complained of lumbago this time. A tumor of the left adrenal gland, however, had been found by computed tomography 4 years earlier. Judging from the CT, the size of this tumor had not changed at all, although the density of the mass on admission had reduced, compared with that of 4 years earlier. Laboratory examinations of adrenal function was normal. Left adrenalectomy was performed. Histologically, this tumor consisted of adipose and some hematopoietic tissue. We reviewed 43 cases of adrenal myelolipoma resected surgically in Japan.

[Mixed gonadal dysgenesis with plasty in males].

A 13-year-old Korean boy is reported here as a case of mixed gonadal dysgenesis. The patient presented with perineal hypospadia. He had scrotal testis, epididymis, vas deferens on right side, and abdominal streak gonad, Fallopian tube on left side. He had also uterus and vagina. His chromosome was 46XY type. Among 59 reported cases in Japan, 12 had undergone plasty into male. All of them had hypospadia. In 51 cases, 17 had testes in scrotum. None of them had a testicular tumor. So we decided to do plasty in male, and to follow up his testis quite carefully.

[A case of multilocular renal cyst].

A case of multilocular renal cyst is presented. The patient, a 65-year-old female, who had had macroscopic hematuria since 1979, came to our hospital in 1986. Computed tomography demonstrated a cystic mass with septa enhanced by contrast medium. The lesion was located at the anterior aspect of the midportion of the left kidney. These findings suggested a multilocular renal cyst. Therefore, enucleation of the mass was performed after confirmation of the absence of malignant cells in the septum on the frozen slide. Histological diagnosis was the same as the clinical diagnosis. We reviewed 65 cases of multilocular renal cyst in Japan and discuss preoperative diagnosis and operative procedure.

[A study on bacteria within stones in urolithiasis].

The bacteria in 37 stones obtained from 37 patients with urinary stone diseases, that is, 11 renal stones (containing 2 staghorn calculi), 21 ureteral stones, 4 bladder stones and 1 urethral stone, were studied, according to the Nemoy & Stamey's method. The stones were collected by partial nephrectomy (1 case), nephrolithotomy (1 case), pyelolithotomy (1 case), percutaneous nephrolithotripsy (PNL) (6 cases), 12 ureterolithotomies, transurethral ureterolithotripsy (2 cases), cystolithotripsies (4 cases) and spontaneous deliveries (10 cases). According to a stone analysis by infrared spectrophotometer revealed 30 were noninfection stones (81.1%) and 7 infection stones (18.9%). Of these 37 stones, 5 stones (13.5% of 11 stones) had bacteria within them. These stones consisted of 4 infection stones (57.1% of all infection stones) and 1 noninfection stone (3.3% of all noninfection stones). Out of 4 patients having bacteria within their stones, urine culture of whom were carried out before stones were collected, only one patient (25%) had the same species of bacterium (E. coli) both within the stone and in urine. The E. coli within the stone and that in urine, however, showed quite different reactions to some antibiotics. The fact that all organisms within stones cannot be detected by urine culture before collecting stones of the patients in our present study, suggests that patients undergoing endourological surgery, such as extracorporeal shock wave lithotripsy and PNL, may have a risk of complications, such as severe urinary tract infection and urosepsis, if the possibility of the presence of organisms within stones is not taken into consideration.

Reduction of testicular human chorionic gonadotropin receptors by human chorionic gonadotropin in infertile men.

We measured testicular human chorionic gonadotropin (hCG) receptors in 30 infertile men either before or 1, 3, 5, 7, or 14 days after a single administration of 5000 IU of hCG. For 5 days the administration of hCG significantly reduced the testicular binding of 125I-hCG compared with that of the testes before administration. From the 7th day the binding capacity began to increase and returned to the preadministration level 14 days after the treatment. Occupied hCG binding sites accounted for about half of the reduction in binding sites on the day after administration of hCG. After this time, however, the occupancy did not contribute so much to the reduction in binding sites. These findings suggest that the reduction in testicular hCG binding sites after a single administration of 5000 IU of hCG is due to not only occupancy but also down-regulation of the binding sites.

[A case of secondary malignant lymphoma of the urinary bladder].

A case of secondary malignant lymphoma of the urinary bladder is presented. Clinical diagnosis was metastatic small cell carcinoma in urinary bladder. Pathological diagnosis after autopsy, however, revealed vesical involvement of malignant lymphoma. The primary focus was considered to be the lung, the biopsy specimen of which was initially diagnosed as small cell carcinoma, because clinical manifestation first occurred in the lung and the metastasis to subcutaneous tissue, bladder and other abdominal organs was found subsequently. In a study of secondary involvement of genitourinary organs as seen in the present case, we reviewed 303 patients who had died of malignant lymphoma at our institute between 1960 and 1985.

[A case of retroperitoneal fibrosis demonstrating a remarkable response to steroids].

We present a case of idiopathic retroperitoneal fibrosis that demonstrated a remarkable response to steroids. The patient, a 73-year-old man, complained of left flank pain and weight loss. Erythrocyte sedimentation rate was elevated and both CRP and antinuclear factor were positive. DIP showed left hydronephrosis, which proved to be due to the stenosis of the left ureter at the level of L5 by retrograde pyelography. Then abdominal computed tomography revealed a large retroperitoneal mass with the density of soft tissue in which the left ureter was involved. Based upon the above findings, we made the diagnosis of idiopathic retroperitoneal fibrosis. After confirming histological diagnosis by biopsy from the mass, we performed left ureterolysis. Since then, he has been treated with steroid administration. Now we can recognize a remarkable reduction of the mass in abdominal CT and improvement of the left renal function in DIP. We reviewed the association of autoimmune disease with idiopathic retroperitoneal fibrosis and recent therapies for this disease.

Secondary involvement of genitourinary organs in malignant lymphoma.

We examined 322 patients who had died of malignant lymphoma at our institute between 1958 and 1985 in order to study secondary involvement of genitourinary organs (GUO). Secondary involvement of GUO was more common in non-Hodgkin lymphoma (NHL) than in Hodgkin's disease. The most commonly affected GUO was the kidney, which was secondarily involved in 121 out of 322 patients (37.6%). The adrenal gland was the second most commonly affected organ: 58 cases (18%) were affected by the disease and 40 of them were associated with renal involvement. Third and fourth were the urinary bladder (8.4%) and the testis (5.9%). Secondary involvement of the bladder in NHL was often accompanied with that of other GUO. Next, the relationship between metastases and histological characteristics in NHL was investigated. However, no significant correlation proved to exist between histological grade and metastases to the GUO.
