Long-term outcome after laparoscopic splenectomy related to hematologic diagnosis.

BACKGROUND: Laparoscopic splenectomy (LS) has been demonstrated as an effective and safe treatment for hematological disorders requiring spleen removal, especially in cases of normal-sized spleens. However, although results are promising, long-term outcome data are lacking. We reviewed our clinical experience with LS in a series of 255 cases, with particular attention to the long-term outcome related to the disease process requiring LS. METHODS: From February 1993 to October 2003, LS was attempted in 255 patients (100 males and 155 females with a mean age of 45 +/- 19 years) and clinical information was recorded in a prospective database. Indications for splenectomy included idiopathic thrombocytopenic purpura (ITP) (n = 115), HIV-ITP (n = 9), Evans syndrome (n = 6), autoimmune hemolytic anemia (AIHA) (n = 13), hereditary spherocytosis (HS) (n = 19), hematologic malignancy (n = 66), thrombotic thrombocytopenic purpura (n = 1), and others (n = 26). Long-term postoperative follow-up evaluation was obtained through clinical notes, follow-up visits by the referring hematologist, and by phone interviews both with patients and with the referring hematologist. RESULTS: A total of 186 patients (73%) were available for a mean follow-up of 35 months (range, 1-104). Of the ITP patients, 87 (76%) were followed up, with a remission rate of 89% (complete remission in 75%). A similar remission rate was observed in ITP-HIV; in patients available for follow-up (78%), complete remission was achieved in 83%. In Evans, complete remission was achieved in all patients available for follow-up (67%). Clinical response for hemolytic disease ranged between 70% for AIHA and 100% for HS. In the malignant group, the late mortality rate was 22%. The mortality rate in the miscellaneous group was 5%. No cases of splenectomy-related sepsis occurred during follow-up. CONCLUSIONS: LS offers advantages for all types of splenic diseases requiring surgery. It provides not only good clinical short-term outcome but also satisfactory long-term hematological results.

A comparative evaluation of Etest and broth microdilution methods for fluconazole and itraconazole susceptibility testing of Candida spp.

The Etest strip is a promising tool of broad application in clinical microbiology. The method provides MIC readings and is easier to perform than broth microdilution. We carried out a study to compare the MICs of fluconazole and itraconazole obtained by the Etest with those obtained by broth microdilution, performed according to the guidelines of the NCCLS document M27-A, with 402 clinical isolates (360 Candida albicans, 17 Candida tropicalis, nine Candida krusei, nine Candida glabrata and seven Candida parapsilosis) and seven control isolates. The agreement between MICs by the two methods (at +/- 2 dilutions) was 74.5% for fluconazole and 61.4% for itraconazole. These results suggest that further development is necessary to standardize the medium and incubation conditions before introduction of the Etest as a routine method in the clinical microbiology laboratory for fluconazole and itraconazole susceptibility testing.

Response to fluconazole and itraconazole of Candida spp. in denture stomatitis.

The significance of Candida albicans in the development of denture stomatitis (DS), as well as the clinical and microbiological efficacy of treatment with fluconazole and itraconazole was studied in 115 patients affected with DS and 200 controls (100 healthy patients with dental prosthesis and 100 healthy patients without prosthesis). Specimens were taken from all patients; subsequently all patients with positive culture of the DS group were treated with fluconazole. A second specimen was taken after 15 days of treatment with fluconazole, and if the results were positive again, treatment with itraconazole was instituted and the patients were given appointments for taking a third specimen. The incidence of C. albicans was 92% in the group of patients with DS. After treatment with fluconazole, a clinical cure of 97% and a microbiological cure of 78% was obtained in the patients with DS. In 3.2% of the cases strains resistant to fluconazole were found. The cases of microbiological resistance to fluconazole were treated with itraconazole resulting in a clinical cure of 100% and a microbiological cure of 77%. The results show the poor correlation of the clinico-microbiological response after treatment with these antifungal agents in denture stomatitis.

Antifungal activity of LY295337 in vitro against clinical isolates of Candida spp.

LY295337 is a new antifungal agent derived from the echinocandins; it shows in vitro and in vivo activity against a great variety of pathogenic fungi. In this study, we evaluated the in vitro activity of LY295337 against 201 strains of Candida spp. using two culture media: Sabouraud dextrose broth (SDB) and RPMI 1640; fluconazole was used as reference antifungal agent. The minimum inhibitory concentration (MIC) was determined by the broth microdilution method for LY295337 in SDB and RPMI 1640, and for fluconazole by an agar dilution method. The fungicidal activity of LY295337 was also determined. The MIC-50 as well as MIC-90 values obtained for LY295337 in SDB (range 0.025-0.6 mg/l) were lower than those obtained for RPMI 1640 (0.035-1.25 mg/l). All the strains resistant to fluconazole were sensitive to LY295337 (MIC < 0.6 mg/l). These results show that LY295337 is an excellent antifungal agent with a fungicidal activity in vitro against various species of Candida.

Antifungal activity of sertaconazole in vitro against clinical isolates of Candida spp..

Sertaconazole is a new azolic derivative containing a benzo(b)-thiophene group, for topical use. It showed in vitro fungistatic and fungicidal activity against yeasts, dermatophytes, opportunistic filamentous fungi and Gram-positive bacteria. In this study, we have evaluated the activity of sertaconazole in vitro against 215 strains of Candida spp. Fluconazole and ketoconazole were used throughout as reference compounds. The minimum inhibitory concentration (MIC) was determined in Casitone medium and the breakpoint was obtained spectrophotometrically and visually. Visual reading of MICs correlated with the spectrophotometric determination, the values of MIC50 and MIC90 showed no difference (+/- 1 dilution) in any of the species tested (range 0.1-16 mg/l). These results show that sertaconazole is an excellent antifungal agent and fungicide in vitro against various species of Candida.

[Study of dietary habits of students in Galicia].

The food habits of 168 university students in Galicia (a region in Northwest Spain) were investigated by means of a questionnaire. Alcohol consumption, smoking and frequency of physical exercise were also evaluated, and height, body weight and blood pressure were determined for all subjects. Our results show that in the diet of university students all groups of foods are present, and their frequency consumption is within the recommended intake by Health Authorities. Smoking and alcohol consumption is low among the study population.

[Nutritional status of the university student population in Galicia].

In this work we have investigated the dietary habits of 107 university students in Northwest Spain. From the different types of questionnaires that exist, the "24 hours recall" has been elected. Biochemical analysis were carried out in blood and urine to complete the nutritional study. Our results indicate that the distribution of energy intake over the day is generally acceptable though in many cases a bigger breakfast would be advisable. The results of blood and urine analysis were in all cases within the corresponding normal ranges which means that there aren't any evidence of pathological alteration.

[Consumption of lipids in the university population of Spain, evaluation of the hepatic profile and of serum cholesterol].

In the present study we tried to characterise the diet of university students in Northwest Spain, paying particular attention to lipids and fibre intake. We administered a 24 hour recall dietary-habits questionnaire. Saturated and monounsaturated fats predominated, with the intake of polyunsaturated fatty acids being considerably lower. Mean daily cholesterol intake was in most cases in excess of the recommended maximum and mean fibre intake showed a considerable variation among individuals. The obtained hepatic profile is found within normal range. The calories provided by the diet result adapted in half of the studied population approximately, while the fourth part does not cover the recommended quantity. These results could be considered a risk in the future.

Post-chemonucleolysis computed tomographic findings.

The CT findings observed 6 months and 2 years after chemonucleolysis in 50 and 34 patients are reported. Emphasis is placed on the persistence of the hernia image in nearly 70% of the cases, together with an increase in degenerative intradiscal signs and degeneration of the facet joints; however, this did not lead to poor results 2 years after chemonucleolysis.
