[Medical health care services for the population under the condition of social differentiation].

Monitoring of availability of medical care and drugs for the population has been carried out by interviewing more than 4000 residents of Russian cities. Stratification of respondents by the level of life showed that more than 70% respondents evaluate their level of life as below the medium. A social gradient was detected: direct relationship between population morbidity and conditions of life. [Population health and socioeconomic problems of society].

The results of a social-and-hygienic study of the health condition of different social-and-economic population groups are under consideration. It was established that the income status of people determines directly their life quality, i.e. the volume and assortment of foodstuffs, the regularity and value of recreation, the mode of labor and life, a possibility to improve their professional skills, satisfaction of spiritual needs etc. The differences of the life quality between various social-and-economic groups are a social gradient predetermining the differences in their health condition. As the material welfare of a family decreases, the frequency rate of diseases among both the adults and children goes up. The property status determines a degree of availability of medical care and of medical preparations. The definition of a realistic picture of population health is necessary to provide an information foundation for determining the population's objective needs in various types of medical assistance, in medical drugs and in medical equipment means.

Child↗

[Children's health under the conditions of social differentiation].

Results of social-and-hygienic research of the health condition of children belonging to different social-and-economic population categories are under discussion. The material family status is shown to directly affect the child's life quality, i.e. family life mode, psychological climate, scope and variety of food-stuffs, the possibility to have a regular and valuable recreation, educational regime, and the possibility to satisfy the spiritual demands of a child etc. The morbidity goes up among children as the family income decreases. The social-and-economic conditions are the uttermost and often decisive risk factor that provokes the development of deviations in children's health. The material stratification of society conditions, for children, differing degrees of access to public benefits that are involved in shaping the children's health, thus, entailing the disequilibrium in their health. A realistic pattern of children's health is needed as a data base to define an objective necessity of children in medical care of different types as well as their necessity in medical drugs and equipment.

[An analysis of the activities of hospital institutions on the territorial level by using the system of diagnosis-related groups].

The authors present the results of analysis of the activity of hospitals at one territory of Russia making use of a system of Diagnosis Related Groups (DRG). Factographic analysis of database for inpatients, based on the DRG typology of patients, helped determine the profile of each hospital, group of hospitals, and the entire territorial hospital network. The results of analysis permit us to define the main profiles of each hospital (or hospital type) and assess its actual and potential resources. Such method of analysis, based on the DRG typology of patients, may be used for rapid assessment of the activities of hospitals in order to determine the trends in possible changes of the hospital profile.

Diagnosis-Related Groups↗

[Diagnosis-related groups: a comparative analysis of hospital activity in different countries].

Comparative study of hospital activities in the Novgorod district and nine countries of the European Community, carried out within the framework of the International Project CAMISE (Case Mix and Severity) of the European Community on the basis of the classification system for diagnosis-related groups, showed a lower Case Mix Index, a longer hospital stay for all major disease, and a lower surgical activity in the hospitals of the Novgorod district in comparison with other European countries. The detected differences are largely due to specific features in the organization of hospital service in Russia (no special hospitals for acute and chronic patients, no nursing homes) and insufficiently intensive therapeutic and diagnostic process.
