[Paradigm as a methodological approach to analyzing Russia's public health system].

A concept of paradigm is applied to the analysis of the national public health system. Whether it can be used as a methodological regulator in public health management is considered. Evidence is provided for the theoretical premises of constructing a paradigm of the public health system, its philosophical bases, the notional system, methodology, and the technological framework. The theoretical and practical aspects of public health paradigm, such as identification, interpretation, and rationalization are used to analyze at the conceptual and practical level, as well as search for new ways of reforming the public health system by the paradigm approach as a deeper development of the existing paradigm or its modification. This can become a foundation for the realignments of the theoretical and practical bases of the national public health system. The formation of the new paradigm of the public health system is to take place not in a revolutionary, but in a developmental mode. Priority should be given not to radical transformations and drastic changing of the key concepts, but to the fluent increasing of new knowledge and their inclusion of the theory and practice of public health. The promises of using the paradigm methodology, allowing the internal and external principles of the operation and function of the public health system to be integrated. In this context, the paradigm approach may be used as a methodological tool for analyzing person-society-state-economy-management relations within the framework of the public health system.

[The health of the population of the Russian Federation: the problems and the outlook].

The paper provides evidence that the general assessment of Russia's population health suggests its poor state which can lead to deterioration of the quality of the population and to its limited creative participation in improving the socioeconomic situation in the country. Exigency of the nation's health promotion requires that an elaborate and sane, responsible state policy should be worked out in health protection with the purpose of legal regulation of the organizational, structural, financial, and economic principles of the functioning of the public health care system at the federal, regional, and local levels. Russia needs the legal reformation of the public health system, by preserving its beneficial characteristics and by revealing the mechanisms of its adaptation to the changing legal and socioeconomic conditions.

[Demographic processes in the countries of Eastern Europe 1945-1990].

An analysis is made of changes in the demographic processes in the countries of Eastern Europe over the period from 1945 to 1990 within both the general regularities and national peculiarities according to the parameters of statics and dynamics of population movement. The positive tendencies in the demographic processes are pointed out, first of all in infant mortality rates and mean expectation of life at birth in Eastern European countries by decades reflecting the peculiarities of changes as compared with developed countries.

[Evaluation of the technology of health care delivery under new economics].

The development and improvement of technology in health care delivery and its utilization in applied public health is a prerequisite for improving the quality of curative and diagnostic process, rational use of material and technical resources and medical personnel of curative and preventive establishments under new economic methods of public health management. The volume standards of health care delivery comprising the standard, optimum sets of measures for patients including the list of diagnostic and treatment procedures can be considered as the main instrument and the major criteria in evaluating the completeness and quality of health care delivery at hospital stage and at different regional levels.
