[Extracorporeal circulation with danaparoid sodium for valve replacement in thrombocytopenia induced by type II heparin].

A type II heparin-induced thrombocytopenia (HIT) was diagnosed in a 64-year-old woman at day 20 of intravenous unfractionated heparin (UFH) therapy, given after myocardial infarction treated by angioplasty and intracoronary stent. The infarction was complicated by a mitral insufficiency that led to a mitral valve replacement. Cardiopulmonary bypass was successfully performed with sodium danaparoid (Orgaran), as an alternative to UFH, without thrombotic or haemorrhagic complications and the follow-up was uneventful.

[Computer-assisted coronary surgery].

Routine totally endoscopic, beating heart, coronary surgery should be made possible by the use of computer enhanced surgical techniques. It includes a totally endoscopic mammary artery harvesting, a correct exposure and an adequate stabilization of the coronary artery at the anastomotic site, a perfect anastomosis of the mammary artery on the left anterior descending coronary artery using a microsurgical suture technique. This complex surgical protocol will be reached by a step by step approach. The first 20 patients who accepted to be operated with tele-manipulated instruments make the substance of this first report. In 19 cases, the dissection of the internal mammary artery could be performed with an optimal result: the lack of bleeding during the dissection emphasizes the excellent visualization of the operative field and the precision of the dissection. The satisfactory blood flow in the mammary artery at the time of the coronary anastomosis suggests the lack of spasm and confirms the atraumatic dissection. The distal anastomosis of the coronary bypass has been performed through a mid line sternotomy to avoid an excessive prolongation of the operative time. The anatomic conditions and the quality of the vessel wall allowed to perform the coronary anastomosis with the tele-manipulated instruments in nine cases only: in six patients, the mammary artery has been implanted on the descending artery, in three, a venous autograft on the diagonal branch. Our initial clinical experience with this new technique suggests that a very precise and fine surgery can be performed with an acceptable prolongation of the operative time. More experience and further developments in the instrumentation are nevertheless required to allow completion of the entire procedure totally closed chest, on a beating heart.

[Interruption of the aortic arch and an aorto-pulmonary fistula. Simultaneous correction of lesions in a 5 1/2 month old infant].

Aortic arch hypoplasia is rare but carries a poor prognosis. It is usually associated with a left to right shunt at ventricular level. The association of an aorto-pulmonary fistula is exceptional. A case of a 5 1/2 month old infant treated successfully by simultaneous correction is reported with a review of the physiopathological changes. Pulmonary hypertension and aortic perfusion depend on the calibre of the ductus arteriosus and its evolution. The therapeutic problems which result are the suppression or palliation of pulmonary hypertension and resaturisation of distal aortic vascularisation. Surgery is the only hope of cure, and current techniques are reviewed: two stage procedures with revascularisation of the distal aorta and pulmonary artery banding, or simultaneous correction of aortic continuity with suppression of the left to right shunt under cardiopulmonary bypass. The technical conditions are analysed: perfusion of the different aortic segments, methods of reestablishing aortic continuity. Good results are as yet rare, and are obtained after complete correction under cardiopulmonary bypass.

[Amiodarone hydrochloride and anesthesia in heart surgery. Use of an injectable form in the correction of peroperative rhythm disorders].

The amiodarone is studied in the context of cardiac surgery to evaluate its anti-arrythmic properties in its injectable form and possible interference with the anaesthetic drugs. Two groups of patients were established: - the first group of 10 patients received the amiodarone per-os in the immediate pre-operative period, - the second group of 19 patients received injectable amiodarone either by direct intraveinous injection at a dose of 5 mg/kg or in I.V. drip for rhytmic irregulatities during surgery. The anti-arrythmic property of the product is confirmed. No incompatibility with the anaesthetics employed (neuroleptanalgesia) was observed. Howerer, a potentialization of the sympatholytic alpha-effects by the anaesthesia is observed. The fall in arterial pressure, when it is measureable, is of the order of a third of the initial value, with a return to the previous level within about ten minutes.
