[Clinical efficacy of using ximedon in surgical practice].

Results of clinical trial of the new pyrimidine derivative--ximedone are presented. The drug was administered per os 0.5 g x 4 times a day (0.5 h before meals). Total course dose achieved 60 g. Ximedone implementation for complex treatment of burns, trophic ulcer of lower limbs, different inflammations demonstrated positive results. Prophylaxy administration of the drug in abdominal surgery at acute appendix resulted with lower incidence of postoperative complications. High efficacy and good tolerability of the drug allows to recommend it for wide medical use.

[Laparoscopic cholecystectomy: possibilities, shortcomings, prospects].

The results of surgical treatment of acute cholecystitis in 350 patients have been analysed. The role of concomitant diseases and age of a patients in choosing the method of surgical treatment is stressed. The advantages and shortcomings of the low-traumatic surgical methods are discussed. The way of choosing the proper surgical method in treatment of acute cholecystitis is proposed.

[Comparative assessment of methods of extracorporeal detoxication in obstructive jaundice].

Comparative analysis of the methods of extracorporeal detoxification in 168 patients was conducted. Endoscopic and surgical methods of treatment were applied in 40 patients without extracorporeal detoxification. Various types of extracorporeal detoxification were used along with endoscopic and operative methods in 168 patients: plasmapheresis (PP) in 72, thoracic duct (TD) drainage in 45, and hemosorption in 41 patients. In another 10 patients various types of detoxification were combined. TD drainage is indicated in prolonged obstructive jaundice, particularly when attended with cholangitis. TD drainage or PP is advisable in concomitant renal failure, PP and hemosorption have a good effect in obstructive jaundice as methods of emergency detoxification. Their efficacy increases if they are applied after endoscopic detoxification of the biliary tract.

[Device for wound debridement].

A device for prevention of wound infection in emergency surgery is suggested. It is distinguished by its funnel having an elastic prolongation in the form of a porolone "skirt". The openings on the funnel have the shape of a truncated cone positioned at an angle to the axis of the device and directed at the center. On the basis of the treatment of 60 patients, a comparative analysis was conducted of the use of the traditional apparatus producing a pulsed stream of fluid under pressure and the device suggested by the authors. The following advantages of the suggested device were revealed: good washing of the wound; no spattering of the washing solution; easy preparation for the procedure; economy of the washing solution. No purulent complications occurred in operations under contaminated and infected conditions when the authors' device was used for the prevention of wound complications. Purulent wound complications were encountered in 18.2% of patients in the compared group of similar cases in which an apparatus creating a pulsed stream of solution was used.

[Determination of antimicrobial activity of wound discharge for assessing wound healing].

The authors studied the dynamics of changes of antimicrobial activity of the wound discharge in 70 patients, aged from 18 to 83 years, who underwent operations for emergency diseases of the abdominal organs with postoperative suppuration of the wound. The study was conducted at the peak of suppuration, in the regeneration phase, and before closure of the wound with sutures. The antimicrobial activity of the wound discharge was found to be low at the peak of wound suppuration with marked necrotic changes of the tissues, as well as in poor granulations, and good in uncomplicated course of the wound process. Changes of antimicrobial activity of the wound discharge, both in uncomplicated and complicated course of the wound process, correlated with the clinical signs of healing. The obtained data make in possible to determine antimicrobial activity of the wound discharge for evaluation of healing, prognostication of bacterial complications in the wound, and control over the efficacy of the treatment.

[Physico-chemical parameters of bile in evaluating the course of an inflammatory process in the biliary tract].

Bile electric conductivity and pH were studied in dynamics in 165 patients with acute cholecystitis. The bile was obtained from drains of the gallbladder, choledochus and its terminal part and from percutaneous-transhepatic cholangiostoma. The results of the study proved that the combined changes of the mentioned physicochemical parameters of the bile are determined by the course of the inflammatory process in the biliary tract and pancreas and by the degree of hepatic functional insufficiency. The study demonstrated that in intensification of the inflammation in the biliary tract the changes of bile pH and electric conductivity occur 24-36 hours before the changes of clinical and laboratory parameters. On basis of the changes of the mentioned bile physicochemical parameters the authors determined the terms for removing the drain from the gallbladder and the choledochus and the possibility for undertaking an operation for cleansing the biliary tract, and introduced corrections into the postoperative conservative treatment.

[Auto-dermal monophilic plastic surgery of ventral hernia].

To improve the results of surgical management of postoperative ventral hernias, the authors applied their suggested variant of autodermic plasty of the hernial openings. The method consists in using for the plasty a band or graft of autologous skin armored with a monophilic thread. To reduce lymphorrhea the autodermic graft was covered with a hernial sac on a pedicle. The method was applied in 93 patients with good late-term results. The number of postoperative complications reduced to 3.5% and mortality to 1%, recurrences were not encountered.

[The effect of xymedon on healing of linear wounds].

In the experiment on 173 rats the stimulating effect of xymedon on the processes of reparative regeneration in a wound was established. This effect manifests itself in increasing the elasticity and strength of the cicatricial tissue. The use of xymedon in the clinic permitted to improve the course of postoperative period in patients with uncomplicated forms of acute appendicitis and reduce the incidence of complications of the wound healing.

[The use of xymedon for wound healing stimulation and prevention of suppuration of surgical wounds].

Xymedon, a new preparation of the pyrimidine series, was studied in experiment and in the clinic. Experiments showed that on the 5th postoperative day the tensile strength of the scar was greater by 57% and the elasticity strength was greater by 21.5% in animals given Xymedon that in the control animals; the tensile strength was 42.4% greater than in animals given methyluracil. Oral administration of Xymedon in the postoperative period in 92 patients with various forms of acute appendicitis allowed earlier removal of the sutures and reduction of the number of wound complications from 13.8 to 3.3%, as a result of which the patients were discharged from the clinic within a shorter period. The authors recommend Xymedon as a stimulator of reparative processes in the wound and for the prevention of postoperative wound suppuration in emergency abdominal surgery.

[Mechanical jaundice: laparotomy or endoscopy?].

The results of diagnosis and surgical and endoscopic treatment in 285 patients with jaundice are analysed. The diagnostic-therapeutic tactics were divided into 3 periods. In the first period the diagnosis was based on the clinical findings and the results of laboratory biochemical tests. The postoperative death rate in this period was 15.3%. The second and third periods were characterized by early complex use of endoscopic diagnosis and treatment, which made it possible to reduce significantly the number of preoperative diagnostic errors and postoperative death rate. In the second period it was 5.9%, while in the third period there were no fatal outcomes. It is concluded that early endoscopic diagnosis and treatment in cases with obstructive jaundice is advisable.

[Causes of recurrence of pain in patients after cholecystectomy and its treatment].

The authors analysed the results of diagnosis and treatment of the postcholecystectomy syndrome in 131 patients. Recurrence of biliary colic after cholecystectomy is linked in most patients with functional disorders of the sphincter of Oddi (27.4%) which disappear in many cases after cholecystectomy. In part of the patients, however, stable functional disorders of the major duodenal papilla become sources of organic changes later not only in the papilla but also in the organs of the pancreaticobiliary system. This disorder in the condition of the papilla must be detected in time and treated by a complex of nonoperative measures; if these measures prove ineffective, endoscopic or operative papillosphincterotomy must be carried out.
