Participation of complement in nasal polyposis.

No changes in immunoglobulin (IgG, IgM and IgA) and complement components C3 and C4 levels could be observed in patients suffering from both recurrent and acute nasal polyposis. However, in both groups of patients a significant decrease of the alternative pathway of complement activation (up to 80%) was found. In addition, patients suffering from the acute disease showed even decreased classical complement pathway (up to 20%). Examination of complement activation pathways was shown to be advantageous for the follow-up of the course of this disease.

[The presence of human papillomavirus deoxyribonucleic acid in tumors in the otorhinolaryngologic area].

14 tumours in the ENT region were tested for the presence of human papillomaviruses (HPV) DNA. The dot-blot hybridization technique was used for HPV sequence detection. HPV DNA mixture of types 6b, 11, 16 and 18 served as a probe. The following five samples showed positive reaction: 3 carcinomas of the larynx, 1 carcinoma of the nasal cavity and 1 papilloma of the nasal cavity.

[Rational antibiotic therapy in ORL oncologic surgery].

The decisive point is the suitable time of antibiotic administration, their combination and possibility to extend the surgical repertoire by prevention in particular of general complications. Wound healing is more rapid, and intended adjuvant treatment can, due to the reduce period of wound healing, be applied much sooner. It is more effective and the attainment of target structures by the administered drugs is not blocked by prolonged processes which are sometimes associated with fibrosis, scars and regressive tissue changes.

[Manifestations of epidermolysis bullosa in the deglutition tract].

An account of a rare mucosal form of epidermolysis bullosa dystrophica in a 26-year-old female patient. The mucosal manifestations involved severe stenosis of the oral portion of the oesophagus. After exhausting all conservative therapeutic procedures (corticoids, transfer factor, dilatation), the stenosis of the oesophagus was successfully operated by resection of the stenotic portion and a plastic operation.

[The tensor tympani muscle reflex in nasopharyngeal carcinoma].

The initial stages of carcinoma of the nasopharynx may be manifested by different types of impaired hearing. The Eustachian tube may be obstructed by a tumour or its orifice compressed, sometimes the tumour penetrates into the middle ear. The authors discuss the position which may develop in infiltration of the semicanalis m. tensoris tympani and how functional disorders of this muscle may be manifested. They describe the provoked reflex of this muscle by trimolar acetic asid and investigate tympanometric reactions of this artificially provoked contraction of the m. tensor tympani in patients with carcinoma of the nasopharynx.

[Verrucous carcinoma of the larynx].

The authors give an account on the treatment and their own observations of the biological behaviour of verrucous carcinoma in five patients treated in the Radiotherapeutic Institute of the Municipal Institute of National Health and ENT Clinic of the Institute for Postgraduate Medical Training. They recommend in verrucous formations of the larynx bioptic examination and investigation of the presence of koilocytes. Koilocytes are generally considered a biological correlate of the presence of the HPV virus. At present the HUV virus, types 16, 18 and 30, are considered the possible aetiological agent of verrucous carcinoma. The method of choice in the treatment of verrucous carcinoma is surgery.

[Non-differentiated carcinoma of the nasopharynx in children].

The authors deal in their paper with the problem of nondifferentiated carcinoma in childhood. They draw attention to data in the literature which report an incidence of 1-15% and compare with these data their own experience according to which in 158 patients with carcinoma of the nasopharynx there were seven children. They emphasize the inexpressive character of symptoms and thus the relatively long time which elapses before causal anti-tumourous treatment is started in child patients. They emphasize the importance of a uniform diagnostic protocol where they accentuate the directoscopy of the Rosenmüller fossa, tympanometry, computed tomography and draw attention to the advantages of EBV serology and mapping according to the HLA system. As far as treatment is concerned, they recommend radiotherapy, supplemented by adjuvant administration of the transfer factor and Mitolactol. In the conclusion the authors emphasize the necessity of team work to ensure correct interpretation of results from the diagnostic sphere and to ensure treatment in the initial stages when therapy gives the most permanent results.

[Endocrine orbital diseases].

The authors summarize their experience with so-called orbital compression which they performed in eight patients solely on account of endocrine orbitopathy. From the total number of twelve operations (four times bilateral) they evaluate the results as unsatisfactory. It is a palliative surgical procedure which gives subjective relief and after some time in some instances also functional results. Although this type of disease is not frequent, this method is practically the only solution.

[HLA in cancers of Waldeyer's pharyngeal ring].

HLA antigens were investigated in 129 patients with various types of carcinomas of Waldeyer's ring; most of them were undifferentiated carcinomas. Significant relations were demonstrated to the antigens of the HLA system particularly in locus B (B17) and DR (DR4). Differences in the frequency of other antigens compared with a control group were not statistically significant. The presence of HLA antigens serves as a useful auxiliary clinical marker which does not change in the individual patient. It characterises the patient immunogenetically and it may also serve as guidance for differential diagnosis in the initial tumour stages. At this stage, diagnosis may be difficult with all known methods, particularly of occult head and neck tumours.

[Diagnostic contributions to the tympanometric study of malignant nasopharyngeal processes].

Extensive studies in areas of high prevalence of nasopharyngeal carcinoma have demonstrated that the most frequent site of origin is Rosenmuller's fossa. Even a small tumour may cause changes in the lumen of the orifice of the Eustachian tube either because of compression or because of a growth of the tumour directly into the lumen of the tube. This causes various wellknown disorders which are frequent initial signs and symptoms of the disease. Initially, subjective symptoms and signs are not accompanied by objective pathological findings except for a pathological tympanogram. The present method of tympanometric examination offers the possibility of detection of several initial stages of nasopharyngeal carcinoma, and combined with nasopharyngoscopy proves to be a very useful complementary examination to serology, biopsy and molecular-biological methods.
