Terfenadine in chronic urticaria: a comparison with clemastine and placebo.

Terfenadine, a new specific and peripherally acting antihistamine, was compared with clemastine and placebo in a prospective, randomized, double-blind, double-dummy, crossover study of the treatment of chronic urticaria. Sixty patients with chronic urticaria participated in this multicenter trial. The treatment time was two weeks per drug treatment, totalling six weeks. Terfenadine proved more effective than clemastine and was associated with significantly less sedation. Few adverse reactions were noted.

Treatment of vitiligo with oral methoxsalen and UVA.

139 patients with vitiligo of long duration were treated with oral 8-MOP and UVA. 22 patients had focal and 117 generalized vitiligo. 27 had vitiligo on the skin of their face and/or neck and 58 on their hands and/or feet. Vitiligo of the face and/or neck responded very well to the treatment, 14 of 27 (52%) repigmenting completely, while the treatment results of hands and/or feet were poor (complete repigmentation in 2 of 58 cases (3%). In cases with focal vitiligo total repigmentation was observed much more frequently (in 11 of 22 cases) than in cases with generalized vitiligo (in 23 of 117 cases). Acute toxic effects were frequent, erythema in 55 cases (40%), nausea in 46 cases (33%), pruritus in 16 cases (12%) and headache in 8 cases (6%). Long-term adverse effects such as hypertrichosis and actinic keratoses occurred in 3 cases each. 6 patients (4%) had elevated transminases at some stage of the treatment. This did not cause withdrawal from treatment in any of the cases.

A double-blind comparison of sulconazole nitrate 1% cream with clotrimazole 1% cream in the treatment of dermatophytoses.

Forty patients with dermatophytosis were treated with either sulconazole 1% cream or clotrimazole 1% cream (twenty patients in each group) twice daily for 4 weeks. Overall clinical improvement with respect to baseline at weeks 2, 3 and 4 favoured sulconazole-treated patients (the differences were statistically significant). The weekly mean severity scores for the sulconazole-treated patients for erythema, scaling, fissuring and itching were consistently lower than those for the clotrimazole-treated patients and the differences were statistically significant for erythema and sealing. It was concluded that sulconazole nitrate 1% cream is very effective against various dermatophytoses.

Anamnestic data in allergic rhinitis.

In a study of 770 patients with allergic rhinitis, anamnestic data were collected by means of a questionnaire. 484 (63%) of the patients were women and 286 (37%) men. 45.5% of the patients had perennial symptoms, 33.5% had seasonal symptoms only, and 21% had predominantly seasonal but also some perennial symptoms. More than 50% of the patients belonged to the age group 21-40 years. The seasonal group included significantly more young patients than the perennial group. More than 50% of the parents and about 45% of the siblings had some atopic disorder. About 20% of the rhinitis patients had a concomitant allergic disorder. Sneezing was the commonest complaint in both rhinitis groups, but it was significantly more frequent in patients with seasonal rhinitis. Nasal blockage, again, was significantly commoner in perennial rhinitis. More than half of the patients reported severest symptoms in the morning. 282 patients had some domestic animal or pet, and 34% of these developed symptoms when close to an animal. Hypersensitivity to animals was significantly more frequent in patients who had domestic animals or pets than in those who did not. There was a clear correlation in perennial allergic rhinitis between the duration of rhinitis and deterioration of the sense of smell.

The effect of etretinate on fibronectin in psoriatic skin.

The distribution of fibronectin in psoriatic skin was studied in 6 patients during treatment with oral etretinate using indirect immunofluorescence technique. In untreated lesions fibronectin was clearly visualized in the dermo-epidermal junction (DEJ) and in the walls of papillary capillaries, and showed a reticular or fibrillar pattern in the dermis. In the horny layer there was some fluorescence which we regarded largely as unspecific. In all patients a transient accentuation of fibronectin accumulation in DEJ was seen after 3-7 days of treatment. Except for some decrease in the amount and intensity of capillary fluorescence, no other notable changes occurred in the fibronectin distribution during 4 weeks of treatment.

PUVA treatment and skin cancer: A follow-up study.

Altogether 525 PUVA-treated psoriatics were re-examined. The mean follow-up period since the beginning of PUVA therapy was 2.1 years (range 1-3.6 years). In 107 cases (20%) the total UVA dose was below 100 J/cm2 and in 20 (4%) over 1000 J/cm2. During PUVA treatment or at re-examination of 14 of the 525 patients (3%) were found to have skin lesions suspected of being malignant. A total of 23 biopsies were taken and in one case a basal-cell carcinoma was histologically verified. This had almost certainly been present before the beginning of the PUVA therapy. Two other patients, both previously treated with methotrexate, had reversible lesions with Bowenoid histology. Both these patients had received a total UVA dose of more than 1000 J/cm2. The incidence of skin carcinomas in the present PUVA treated series was lower than that in hospitalized psoriatics treated with regimens other than PUVA and was not higher than the incidence in the age-matched general population in Finland.

Nasal reactions elicited by unilateral allergen challenge.

Nasal reactions to unilateral allergen provocation were studied separately in both nasal cavities of 9 subjects with established seasonal allergic rhinitis. Three tests with the same allergen at the same concentration were performed in the same cavity at 48-h intervals. The parameters observed were clinical symptoms, changes in nasal airway resistance on rhinomanometry, and amount, weight and histamine content of the collected secretion. Nasal obstruction increased significantly on the provoked side but not contralaterally. Secretion increased symmetrically but the histamine content rose only on the provoked side. No priming effect was observed. The results are compatible with the view that the release of histamine has a 2-fold effect. Histamine directly caused vasodilatation of capacitance vessels and capillaries, which resulted in obstruction on the provoked side, and indirectly the histamine release led to stimulation of sensory nerve endings, which by triggering parasympathetic reflexes caused rhinorrhea in both nasal halves.
