[3-years results of endoprosthetic management of radiation-induced hip joint necrosis].

Radioosteonecrosis of the acetabulum is a rare side effect of radiotherapy. The latency period of 2 to 17 years is much longer than that reported for other bones (1-3 years). The reason for this is unknown. Plane radiograms show sclerosis, cysts and fractures of the acetabulum and, in most cases, a migration of the femoral head with flattening. The diagnosis of a radioosteonecrosis has to be made by exclusion of malignomas by scintigraphy and MRT or CT. This paper presents the follow-up of four patients treated by total hip replacement (THR) for radioosteonecrosis of the acetabulum. Within 9 months postoperatively the fractures of the acetabulum had healed with full weight bearing. 36 to 78 months after operation no signs of progression of the bone changes were detected. In this period the loosening of one spherical thread cup occurred, but a radiolucent gap above a cemented cup stayed unchanged. We conclude that irradiation damage of the acetabulum can be treated successfully by THR even in cases with fractures of the acetabulum as far as a stable implantation can be achieved and the remaining osteoblasts are preserved by the procedure.

[Aseptic necrosis of the lunate bone. Etiology, clinical aspects, stage-adjusted therapy].

The development of the aseptic necrosis of the lunate bone is so far not fully understood. Therapeutical problems result from the very late discovery of the disease and from the central position in the carpal bone system. Bad long-term-results come out of therapies not able to revitalize the lunate bone. Therefore the therapeutical goal attempted must be to keep the structures of the bone alive and to avoid major pathological changes of the whole carpal bone system. More or less good results could be shown for these patients with a total break-down of the lunate bone treated with the intercarpal-arthrodesis described by Graner.

[Graner's intercarpal arthrodesis as therapy of aseptic lunate bone necrosis].

From 1978 till 1989 at the Department of Orthopaedics (University of Graz, Faculty of Medicine, Division of Surgery) 25 patients were operated for the reason of an avascular necrosis of the lunate bone when the bone structures were already destroyed. The form of therapy was the intercarpal-arthrodesis, developed by Orlando Graner 1966, in a slightly modified way. On the one hand through creating an intercarpal block it is possible to create more or less plain sides of the radiocarpal joint and on the other hand one can stop the carpal collapse what explains long-term-results rather acceptable. The most important disadvantage of this form of therapy is the often found loss of range of movement in the operated radiocarpal joint.

[Surgical therapy of malacia of the semilunar bone with a modified Graner method].

All operations for lunatomalacia are linked to considerable alteration and functional loss of the afflicted wrist joint. The risk of secondary arthrosis due to incongruity is very high, and not only because of the complicated biomechanics of the carpal bones. The authors present their experiences and results with seventeen patients treated for Kienböck's disease, of whom fourteen have been reviewed, by a modification of the operation described by Orlando Graner. On the basis of this evaluation and comparison to the results described by other authors, the method of Graner seems to be definitely suitable, even for younger patients. The disadvantage of the relatively obvious restriction in mobility is offset by a nearly complete freedom from pain, full strength, and only minor secondary arthrosis, even with long postoperative periods.

[Intercarpal pseudarthrosis as a complication of Graner's operation].

Since 1978 eighteen patients were treated for Kienböck's disease by the Graner procedure. The satisfactory long-term results can be explained by the preservation of the radiocarpal joint. By restoring fairly congruent joint surfaces, secondary radiocarpal joint arthrosis, which affects long-term results, may be avoided. Employing Graner's procedure, the authors observed two cases with pseudarthrosis within the intercarpal arthrodesis. They interpret this complication to be a result of micro-movements with the new bone block in the former midcarpal joint level. These movements may be avoided by careful removal of all the cartilage from all joint parts which are to be fused.

Adult↗

[Results of the surgical treatment of supra- and diacondylar humeral fractures].

45 patients suffering from supra-diacondylar fractures of the distal end of the humerus were treated surgically and had a follow-up covering a time of 6 to 170 months. In 76% the results obtained were excellent. This demonstrates the superiority of operative treatment in this type of fracture. The most important factor for good exposure of the joint is osteotomy of the olecranon. Fixation of the fragments was carried out according to the principles of AO. The aim--stable fixation--could not be attained in all cases.

Adult↗

[Results of proximal tibia osteotomy in varus deformity].

54 patients (mean age 63.5 years), mostly women, suffering from degenerative arthritis of the knee joint underwent high osteotomy of the condylus of the tibia. Fixation together with compression was achieved by external fixation. The patients were not followed up until one year after completion of treatment. In a percentage of 90 the patients were freed from pain, in 10% the results obtained did not satisfy the patients.

Arthritis↗

[Osteoradionecrosis of the hip joint].

The radiation induced osteonecrosis of the hip is a rare complication after radiation therapy. The time of radiological manifestation is dramatically longer (2 to 17 years) than reported from other bones. There is an inverse correlation between age at the time of radiotherapy and the latency time. Sclerosis, focal lyses, fractures and migration of the femoral head are seen in the radiographs. Differential diagnosis to neoplastic changes should be achieved by bone scan and MRI. We have seen five patients with this condition and had operated on four of them. Treated with THR the fractured acetabulae healed within 9 months and the sclerosis diminished. We did not see any progression of the damage induced by irradiation.

[Post-traumatic femur head necrosis following trochanter fracture].

Avascular necrosis of the femoral head after fractures of the major or minor trochanter have not been published yet. We are able to present such a casereport of a 12 years old girl. We discuss the different reasons for femoral head necrosis by this case.
