[The relation between the level of antibodies to the glycosaminoglycans of the cartilage in patients with osteoarthritis and the efficacy of treatment with chondroprotectors].

AIM: To improve the efficacy of osteoarthrosis (AO) treatment by chondroprotectors by defining individual indications for their use, based on the initial level of antibodies to cartilaginous glycosaminoglycans (GAG) in the blood serum. MATERIALS AND METHODS: Anti-GAG were detected by indirect solid-phase enzyme immunoassay. The antigen was a sulfated GAG preparation GAG-polysulfate (arteparon) manufactured by Luitpold-Werk (Germany). Sera of 110 OA patients were tested. Fifty of them were treated by rumalon, 20 with arteparon, and protocols of 40 patients included no chondroprotectors. RESULTS: The highest levels of anti-GAG were found in OA patients with multiple involvement of the joints, rapidly progressing disease, and secondary synovitis. The titer of antibodies increases with disease duration and in patients with the roentgenological stage of OA, reaching the maximum by 10-15 years of disease or by stage III. Efficacy of chondroprotectors was lower at lower levels of antibodies to cartilaginous GAG. CONCLUSION: Chondroprotector therapy of patients with initially high levels of antibodies to GAG is unadvisable, for it can lead to exacerbations, specifically, to secondary synovitis.

[The clinico-rheoencephalographic characteristics of a hyperemia syndrome].

The author present comparative analysis of rheoencephalographic (REG) data obtained from 500 differently aged Kiev residents with atherosclerotic dyscirculatory encephalopathy after Chernobyl accident. Changes of principal REG parameters are determined as the plethoric syndrome (PS). Main variants of nerves characterizing PS are divided into three types. Data obtained by the authors confirmed changes of the vascular system under the new ecological conditions.

[The clinical course of the immediate and late convalescent periods in viral meningoencephalitis].

The frequency and severity of residual phenomena after viral meningoencephalitis is to large degree determined by the premorbid background, etiology, severity of the acute period of the disease hospitalization terms. Among the residual phenomena functional disorders are more frequent while organic are rare and regress within 1.5-2 years. Complete cure of the patients should therefore be 1.5-2 years after discharge from the hospital.

[Highly active interferon in the combined treatment of patients with meningoencephalitis].

High-active interferon was used in the complex treatment of 49 patients with nervous system diseases of viral-bacterial etiology. A control group of 28 similar patients were also analyzed. A positive clinical effect was observed in early use of interferon. The clinical course of the disease was milder, the duration of clinical symptoms reduced, healing of the CSF was enhanced, the development of protracted forms of the disease was preventable, lethality reduced 2.9 times with interferon in the complex treatment of these patients.

[Lesions of the nervous system in Haemophilus influenzae infection in adults].

The authors depict 13 cases of CNS injuries in the form of meningities, meningoencephalitis and encephalitis in adults, provoked by Haemophilus influenzae. Provide the clinical, laboratory and instrumental data obtained during examination of the patients in the acute disease stage and during follow-up studies lasting up to 5 years. Rare strains of Haemophilus influenzae, serotypes "c" and "d", were detected in the CSF. Concomitant virus infection was confirmed in seven patients. It is desirable that chloramphenicol, erythromycin or ampicillin be used in such cases. Patients who suffered influenzal meningitis should undergo follow-up studies.

[Sanitary education of workers of bakeries and pastry enterprises].

The paper presents experience with sanitary education of workers from food industrial enterprises. The study revealed a number of disadvantages of the methodological aspect of sanitary education of these workers. A sanitary education programme has been worked out for workers engaged in food and food processing industries (production of bread, baked goods, and confectionery) and approved by the State Sanitary and Epidemiological Surveillance Department), Ministry of Health of Russia on March 1999.

[Optimization of artificial pulmonary ventilation in patients with organophosphorus compound poisoning complicated by development of exotoxic shock].

Central hemodynamics, phase structure of left-ventricular systole, and paO2 were studied in 104 patients with organophosphorus poisoning complicated by exotoxic shock during forced ventilation of the lungs (FVL) with positive zero expiration pressure, positive end expiration pressure +5, +7, +10 cm H2O, and high-frequency FVL. FVL with positive end expiration pressure +5 or +10 cm H2O was found to be the optimal for patients with compensated shock. In patients with decompensated phase of exotoxic shock FVL with positive zero expiration pressure and FVL with positive end expiration pressure +5 cm H2O, although having no negative impact on the hemo- and cardiodynamics, did not provide sufficient oxygenation of the blood. Higher values of positive end expiration pressure (+7, +10 cm H2O) brought about disorders of heart work. High-frequency FVL appears to be the most rational in patients with decompensated phase of shock.

[Nonsteroid anti-inflammatory agents (acelysin) in postoperative analgesia and intensive care].

Efficacy of nonsteroid antiinflammatory drugs in the system of postoperative intensive care of patients to relieve postoperative pain, reduce the aftereffects of surgical injury to tissues, and thromboembolic complications is pathogenetically validated. A high efficacy of water-soluble acetylsalicylic acid, acelysin, in patients after extensive oncologic surgery is demonstrated. Acelysin had a good analgesic effect without side effects and permitted reducing the dose of opiates for postoperative analgesia by 3 times in comparison with the control group. Acelysin infusion in the immediate postoperative period reliably prevented thromboembolic complications and ruled out the necessity of resorting to heparin. Acelysin is regarded as a special nonopiate component of postoperative intensive care noticeably improving its efficacy.

[Butorfanol tartrate (beforol) in cancer surgery].

The results of the clinical study of beforal (butorfanol) ("Spofa", Czechoslovakia), a synthetic analgetic from a class of opiate agonists-antagonists, in 17 cancer patients at all stages of surgical treatment have been summarized. It has been established that beforal is a potent analgetic analogous in antistressor properties to moradol ("Galenika", Yugoslavia), however differing by a more prompt onset and discontinuation of action, thus improving the control over general anesthesia. A comparative assessment of circulation parameters and stress factors (glucose, cortisol, blood plasma STH) has shown that beforal exceeds fentanyl in the efficacy of prevention of surgical trauma during and after extended cancer surgery.

[The therapeutic efficacy of reaferon in meningoencephalitis].

There have been under observation 63 patients with meningoencephalitis at the age of 15-52 years whose complex treatment included reaferon and 42 patients with the same disease who were not given the drug. Reaferon was noted to be effective in meningoencephalitis. It shortened the period of clinical manifestation of the disease, hastened sanation of the liquor, influenced beneficially interferon production and modulated activity of the natural killers.

[Analysis of respiratory complications during artificial ventilation of the lungs in patients with organophosphorus compound poisoning].

The authors analyze the respiratory complications in patients with acute respiratory failure (ARF) developing as a result of poisoning with organophosphorous (OPC). Twenty-two percent of 470 patients had to be maintained on forced ventilation because of ARF. The indications for forced ventilation of the lungs are as follows: paralysis of respiratory muscles, 90% drop of vital capacity of the lungs, and disorders of the gaseous composition of the blood (PaCO2 over 56 +/- 3 mm Hg and PaO2 below 67 +/- 5 mm Hg). The incidence of respiratory complications among patients on forced ventilation was as high as 92%, which is due to the pathogenesis of the chemical disease developing after OPC poisoning. The complications are the most incident among men aged 30 to 60. The respiratory distress syndrome of adults ranks first among the respiratory complications. It was diagnosed in 70.6% of dead subjects in 34.4% of convalescents. This syndrome is one main cause of high mortality, particularly so in subjects dying in three or more days of intensive care. The duration of forced ventilation of the lungs does not depend on the time when the patient was transferred on ventilation or on the poison. Analysis of mortality indicates that it is lower among patients administered forced ventilation at the prehospital stage.

[Use of hemodialysis in intensive care of organophosphorus insecticide poisoning].

A total of 163 hemoperfusions carried out in order to remove poison in patients with acute poisoning with organophosphorous insecticides (OPI) showed that this treatment modality effectively removes OPI from te organism. In patients with the most severe forms of acute poisoning hemoperfusion may be effectively combined with hemadsorption and peritoneal dialysis. This variant of detoxication ensures a good clinical effect and a stable decrease of the toxic agent in the blood. The proposed method of blood stabilization with sodium citrate in the extracorporeal contour provides a reliable extracorporeal stabilization of the blood and does not deteriorate the efficacy of hemoperfusion.
