[Personal experience with the Milwaukee orthosis in the treatment of idiopathic scoliosis].

The authors evaluate the results of treatment of idiopathic scoliosis by a Milwaukee brace in 192 patients with treatment initiated and terminated in 1975 to 1985. In the group of 116 juvenile scolioses at the end of the investigation 26 had improved, 53 did not improve and 37 deteriorated. In the group of adolescent scoliosis 18 improved, 44 did not improve and 11 deteriorated. In the group of improved patients the number of persons declines with the time interval after completed treatment. With the increasing severity of the initial curve the effect of treatment by a brace diminishes. The percentage of non-cooperating patients is substantially higher in the group of not improved and deteriorated cases. In 26 selected patients the authors investigated the lordosis-promoting effect of the brace on the thoracic spine. Because of progression of the deformity the authors operated 12 patients with juvenile and one patients with adolescent scoliosis.

[Possibilities in modern spinal surgery].

Rapid development spine surgery brings about all over the world new surgical techniques and new instrumentation which in comparison to the previous ones achieve far better results. The authors present a survey of 1033 operations on the spine divided into 5 groups: 1. deformities (scoliosis, kyphosis and their combination, olisthesis, 2 injuries, 3. tumours, 4, inflammations and 5. other diseases (myelopathy, stenosis, failed back etc.). In comparison to previous years the year 1988 shows the increase in the number of operated injuries (from 15% to 23%), of tumours (from 3% to 5%) and other diseases (from 2% to 8%). A survey is presented of the surgical techniques and instrumentation applied in individual groups.

[Changes in pulmonary shunting and blood lactate during general anesthesia with controlled hypotension in patients with idiopathic scoliosis].

In their work the authors observed the influence of controlled hypotension by means of sodium nitroprusside on the acidobasic balance, the oxygenation parameters, pulmonary shunt and lactatemia in patients with idiopathic scoliosis in the course of dorsal fusion by Harrington instrumentation. In comparison with the control group of patients there had not occurred significant - from the viewpoint of statistics - changes in ph, paCO2 and pvO2. However, the pulmonary shunt (Qva/Qt) doubled (p less than 0.002) which resulted in a significant decrease of paO2 (p less than 0.02) but in no case there occurred hypoxemia. The values of the lactate both in the venous and arterial blood were increased (p less than 0.001, or p less than 0.02) in contrast to the control group of patients, but they did not mean the increased production of lactate during intracellular hypoxia. As a result the regulated hypotension by means of sodium nitroprusside can be applied also in patients with altered pulmonary functions.

Changes in vertebral end plates in congenital spine deformities.

The authors investigated morphologically material from 30 patients with congenital spine deformities operated on at the 2nd Orthopaedic University Department in Brno, and the spines of two patients who underwent a spinal operation and later died from other causes, and also the spines of young, healthy road accident victims. In the course of operations for congenital deformities of the spine representative tissue samples were taken (hemivertebrae, quarter vertebrae, etc.), along with the adjacent parts of intervertebral disks. The authors concentrated mainly on the growth defects of deformed vertebrae and microscopic changes in the adjacent cartilage plates. In this way light was thrown on some questions of both defects of formation and defects of segmentation of the mesenchymal base of the spine. The defects of ossification and growth disorders observed are, in the authors' opinion, an important component in the occurrence of congenital deformities and idiopathic scolioses and have a major effect on their progression.
