The deafferentation facial pain syndrome, caused by physical, herpetic or tumourous lesions of the trigeminal sensory root, was successfully managed with ultrasonic trigeminal nucleotomy. The article presents this new surgical technique and its results in 14 patients suffering from severe facial pain and dysaesthesias.
Four cases of medically intractable trigeminal neuralgia caused by parapontine epidermoid tumours are reported. All patients presented with typical trigeminal neuralgia and low-density area in the parapontine region on CT-scans. Neurologically in all cases slight hypaesthesia in various divisions of the trigeminal nerve without other neurological signs was revealed. Trigeminal pain was completely relieved after surgery in all patients. Facial sensory disturbances and pain occur in patients with parapontine epidermoid tumour as a result of direct tumourous compression (and hypothetically toxic effects on the nerve root from the tumourous contents).
The commonly used method of BAER evaluation is measuring the latency to the extreme points of the components. Because of the influence of noise it is sometimes impossible to determine precisely the true top of the peak. It is possible to avoid this defect by estimating the latency to the middle of the peak front. In this research S.D.s for both methods of evaluation were calculated for all interpeak intervals. According to the obtained data the use of the middle of the front of the peak for the latency evaluation gave 24% reduction in S.D.s.
