Cytotoxic effects of volatile anesthetics with free radicals undergoing laparoscopic surgery.

BACKGROUND: Free radicals induced by several diseases can trigger oxidative stress, leading to the production of malondialdehyde (MDA) and protein carbonyl content (CB). Volatile agents are able to increase the extent of oxidative status. However, the effects of these agents together with pneumoperitonium (Pp) have not been reported. We aimed to investigate the role of volatile anesthetics and ischemic injury during Pp on free radicals and scavenging enzymes in laparoscopic abdominal surgery. METHODS AND MATERIALS: Forty patients were examined. Patients were randomly divided into four groups in order to receive sevoflurane-fentanyl (SF = 10), sevoflurane-N(2)O (SN = 10), desflurane-fentanyl (DF = 10), and desflurane-N(2)O (DN = 10), respectively. Tidal volume and ventilation frequency were kept unchanged during the operation. Intraabdominal pressure was remained constant at 12 mm Hg. Baseline values in venous blood samples were preoperatively taken and blood was also taken postoperatively at the 6th and the 24th hours. After collection of blood samples into citrate (3.5 mg/mL blood) containing glass tubes, erythrocyte sediments were prepared for the analyses. Then malondialdehyde levels, protein carbonyl content, and sulfhydryl (SH) groups were measured. RESULTS: The levels of MDA and protein carbonyl content were significantly higher at the 6th hour rather than the 24th hour postoperatively with desflurane anesthesia. In addition, SH groups were significantly different between the 6th hour and the 24th hour measurements (P < 0.05). In our study, desflurane caused a statistically significant increase in MDA levels and protein carbonyl content and a decrease in SH groups. When the two groups were compared, in the case of MDA and CB values, a significant increase was observed in the 6th and the 24th hour, where there was a decrease in SH groups in the desflurane group (P < 0.05). These parameters did not change in the sevoflurane group (P > 0.05). CONCLUSION: We concluded that desflurane was affected by desflurane with low flow anesthesia in patients undergoing laparoscopic abdominal surgery. Significant influence on oxidative stress and antioxidant mechanics was not seen with sevoflurane anesthesia. Our studies support that oxidant and antioxidant defense mechanisms were altered in the desflurane group and this alteration improved after a combination of desflurane-N(2)O.

Preemptive analgesia in laparoscopic cholecystectomy: a randomized controlled study.

In pain control after laparoscopic cholecystectomy, subhepatic administration of bupivacaine immediately after the creation of pneumoperitoneum has been shown to be more effective than administration before the withdrawal of the trocars. We aimed to investigate the effect of intraperitoneal bupivacaine administration to the subhepatic area before the creation of the pneumoperitoneum. Eighty patients undergoing elective laparoscopic cholecystectomy under general anesthesia were included in a prospective, randomized study. Patients received 20 mL of 0.5% bupivacaine in the subhepatic area just after intubation, before pneumoperitoneum (group 1), immediately after the creation of the pneumoperitoneum (group 2), just before the removal of the trocars (group 3), or received no local anesthetic (group 4). The degree of the postoperative pain was assessed at 0, 4, 8, 12, and 24 hours after the surgery. The consumption of analgesics (diclofenac sodium) was also recorded. The pain scores and analgesic consumption did not differ among groups 1, 3, and 4. The pain scores of group 2 were lower at each time point compared to the other groups (P < 0.001). Postoperative analgesic consumption in group 2 was reduced compared to the other groups (23.4 +/- 35.9 mg vs. 80.0 +/- 66.3 mg, P = 0.005 [group 1], 69.6 +/- 62.2 mg, P = 0.026 [group 3], and 70.0 +/- 59.9 mg, P = 0.022 [group 4]). The subhepatic infiltration of 20 mL of 0.5% bupivacaine offers good postoperative analgesia when applied just after the creation of the pneumoperitoneum, not before the pneumoperitoneum or after the termination of the pneumoperitoneum.

Laparoscopic enucleation of mesenteric cyst: a case report.

Intra-abdominal lesions, such as mesenteric cysts, are uncommon disorders. Most are discovered incidentally during routine abdominal examinations. They generally do not show typical clinical findings. Laparoscopic surgery has been used to remove a wide variety of mesenteric cysts. We report a patient with a mesenteric cyst, who was treated by laparoscopic enucleation.

Are there adverse effects of herniorrhaphy techniques on testicular perfusion? Evaluation by color Doppler ultrasonography.

PURPOSE: To evaluate the efficacy of prosthetic meshes on testicular perfusion using color Doppler ultrasound (CDUS). MATERIALS AND METHODS: This study is a randomized prospective clinical trial with blind assessment of outcome. A total of 26 patients who underwent elective herniorrhaphy for groin hernia participated in the study. Each patient was randomly assigned to 1 of 2 groups: totally extraperitoneal preperitoneal hernia repair (TEP; n=13, mean age 46.7+/-1.6 years) or Lichtenstein hernia repair (LHR; n=13, mean age 54.2+/-2.7 years). Six of the patients had bilateral hernias (n=3 for each group). Blood flow indexes of the spermatic artery including end diastolic velocity (EDV), peak systolic velocity (PSV) and the resistive index (RI) were examined by CDUS. Ultrasound was performed just before the operations and repeated after 3 months. Kruskal-Wallis and Mann-Whitney U tests were used to evaluate the differences. RESULTS: No statistically significant differences were found between the preoperative and postoperative measurements which included blood flow parameters (PSV, EDV, and RI) of the testes (p>0.05). CONCLUSION: Neither the TEP or LHR technique had any decreasing effect on blood flow.

The correlation between renal tissue oxidative stress parameters and TNF-alpha levels in an experimental model of ischemia-reperfusion injury in mice.

BACKGROUND: To investigate the relationship between oxidative stress and pro-inflammatory response of the renal tissue at different ischemia times in I/R mice. METHODS: Twenty-four male Swiss dormice were subjected to 30, 45 or 60 minutes of ischemia and then 60 minutes of reperfusion (Group 1 I 30/R60, Group 2, .I45/R60, and Group 3, I 60/R60 respectively). As markers of oxidative stress and antioxidant activity, levels of thiobarbituric acid reactive substances (TBARS), protein carbonyls and protein sulfhydryls (SH), tissue superoxide dismutase (SOD) and catalase (CAT) activities of the renal tissue and also renal tissue pro-inflammatory marker TNF-alpha levels were assessed. RESULTS: The levels of TBARS and protein carbonyl rised in I30/R60 group (p=0.01). In I45/R60 group, levels of TBARS, protein carbonyls and TNF-alpha levels were significantly higher while SOD and CAT activities (p=0.01), and the levels of SH were significantly decreased (p=0.05). These findings were more relevant for I60/R60 group. Higher TNF-alpha levels correlated positively with higher levels of TBARS, and protein carbonyls and negatively with SOD, CAT and renal tissue SH (p=0.001). CONCLUSIONS: In mice, oxidative stress after 45 minutes ischemia and 60 min reperfusion could induce pro-inflammatory cascade mediated through TNF-alpha.

Which is more effective in the prevention of renal ischemia-reperfusion-induced oxidative injury in the early period in mice: interleukin (IL)-10 or anti-IL-12?

OBJECTIVES: Ischemia-reperfusion (I-R) injury induces production of reactive oxygen species (ROS) and some inflammatory mediators like tumor necrosis factor (TNF). We compared the effects of IL-10 and anti IL-12 antibody (Ab) on TNF-alpha production and oxidative stress markers. We also searched for which one, anti IL-12 or IL-10, is superior in the prevention of I-R induced oxidative stress. DESIGN AND METHODS: The animals were divided into four groups: control (n = 5), I-R (n = 5), I-R + IL-10 (n = 5), I-R + anti IL-12 Ab (n = 5). Mice were subjected to renal ischemia by clamping the left pedicle for 45 min, and were then reperfused for 1 h. RESULTS: Under conditions of IL-12 blockade and IL-10 treatment, I-R-induced tissue TNF-alpha levels were significantly reduced (P < 0.01). IL-10 treatment decreased I-R-induced thiobarbituric acid reactive substances (TBARS) and protein carbonyl content levels (P < 0.05). After IL-10 treatment, decrease in tissue reduced glutathione (GSH) levels were prevented. Renal superoxide dismutase (SOD) and catalase (CAT) activities were observed to be decreased after I-R. IL-10 treatment increased both SOD and CAT activities over control values (P < 0.05). CONCLUSION: These data indicated that IL-10 treatment may be more effective than anti-IL-12 treatment in the prevention of renal I-R-induced oxidative injury in the early period.

The effects of increased intra-abdominal pressure on bacterial translocation.

In this study, we investigated the effect of different values of intra-abdominal pressure on bacterial translocation. Twenty-four Wistar-Albino rats were divided into four groups. The animals belonging to the Control group were not subjected to any increased intra-abdominal pressure. In groups I, II and III, an intra-abdominal pressure of 14, 20, and 25 mmHg, respectively, was established by carbon dioxide pneumoperitoneum for a period of 60 minutes. Four hours after the pneumoperitoneum, all animals were sacrificed to evaluate the degree of bacterial translocation at this time. Liver, spleen and mesenteric lymph nodes were excised under sterile conditions. Bacterial growth was assessed using standard bacteriological techniques and compared statistically. The Kruskal-Wallis and Mann-Whitney U tests were used for the statistical analysis. Different amounts of bacterial growth were found in all of the animals subjected to increased intra-abdominal pressure, except for the controls. Bacterial translocation was detected at an intra-abdominal pressure of 14 mmHg but this finding was not statistically significant (p > 0.05). There was a significant increase in bacterial growth in animals subjected to an intra- abdominal pressure of 20 mmHg or above (p < 0.001). As a result, we found that bacterial translocation started when the intra-abdominal pressure reached a level of 14 mmHg. Patients should be closely monitored for septic complication risks following laparoscopic procedures in which the intra-abdominal pressure exceeds 20 mmHg.

Can laparoscopic hernia repair alter function and volume of testis? Randomized clinical trial.

Testicular atrophy is a sequela of inguinal hernioplasty. The purpose of this study was to evaluate the effects of Lichtenstein (LHR) and laparoscopic totally extraperitoneal (TEP) hernia repair techniques on testicular function and volume. This study is a randomized prospective clinical trial with the blind assessment of outcome. A total of 26 patients who underwent elective herniorrhaphy for groin hernia were included in the study. Each patient was randomly assigned into one of two groups: either TEP or LHR (n = 13 for each). Six of the patients had bilateral hernia (n = 3 for each group). Luteinizing hormone (LH), follicle stimulating hormone (FSH), testosterone levels, and testicular volume by Doppler ultrasonography were detected just before and 3 months after the operation. LH, FSH levels did not change, when compared preoperative and postoperatively in both groups. Testicular volume and testosterone levels were observed to be significantly decreased after TEP when compared with LHR while no significant preoperative changes were observed between those groups. This affected the testicular volume in normal limits. TEP or LHR could not affect LH, FSH, testosterone values, but TEP could lead a decreasing effect on volume of testis in normal limits.

[Afyon sultandagi earthquake].

BACKGROUND: Ninety-six percent of Turkey's land is in the earthquake zone. Consequently the results of every earthquake should be presented and discussed. The aim of this study was to evaluate the results of Afyon Sultandagi Earthquake. METHODS: The records from Governorship of Afyon, local hospitals and Public Work Directorate have been evaluated retrospectively and compared with current literature. RESULTS: An earthquake was detected with the magnitude of 6.0 at 9:11 A.M. at February 3rd, 2002, in the Sultandagi town, which is localized in Afyon city of Turkey. Bolvadin and Cay towns were also affected from the disaster. This earthquake, caused 39 deaths (23 of them were older than 65 year-old; 15 male and 24 female), 325 injured persons; 315 cows and water buffaloes and 3,176 sheep, 14,328 poultry were dead, and 15,032 buildings were damaged. The causes of deaths in human beings were: myocardial infarction in three patients and traumatic cerebral hematoma in one; the others expired from respiratory insufficiency due to dust inhalation because of the traditional architecture of the rural area. Health professionals reached the victims in about four hours, 35 of the injured patients were treated with hospital care and the remaining did not require health support.

Changes in hepatic TNF-alpha levels, antioxidant status, and oxidation products after renal ischemia/reperfusion injury in mice.

BACKGROUND: Ischemia/reperfusion (I/R) injury induces an inflammatory response and production of reactive oxygen species (ROS), which affects the organs remote to the sites of I/R. The aim was to assess the hepatic changes after renal I/R injury. MATERIALS AND METHODS: Twenty mice were subjected to either sham operation or varying degrees of renal I/R injury. Hepatic TNF-alpha levels, myeloperoxidase (MPO), superoxide dismutase (SOD), and catalase (CAT) activities and reduced glutathione (GSH) levels, thiobarbituric acid-reactive substances (TBARS), and protein carbonyl levels were evaluated to show hepatic response to renal I/R injury. RESULTS: Hepatic tumor necrosis factor-alpha levels were found to be increased significantly after 30 min ischemia-1 h reperfusion and remained elevated through 60 min ischemia-1 h reperfusion. Supporting the neutrophil recruitment, about 10-fold increase in MPO activity was detected after 30 min ischemia-1 h reperfusion. Antioxidant enzymes were detected to be decreased after 30 min ischemia-1 h reperfusion and reached to the minimum levels after 60 min ischemia-1 h reperfusion. Decreased levels of GSH and increased levels of TBARS and protein carbonyls after 60 min ischemia-1 h reperfusion supported the ROS-mediated biomolecular alterations. CONCLUSIONS: A minumum of 30 min ischemia-1 h reperfusion is enough to elicit remote effects of renal I/R injury. Care should be taken to protect other organs remote from I/R sites especially during renal surgery.

Usefulness of the Helicobacter pylori stool antigen test for detection Helicobacter pylori infection.

Several diagnostic tests are available for evaluating Helicobacter pylori (H. pylori) infection: histological examination, culture of gastric biopsy specimens, rapid urease test, urea breath test and serology. In this study, we assessed the reliability of a newly developed enzyme immunoassay HpSA (H. pylori Stool Antigen) kit for detecting H. pylori antigen in stool. Eighty-five patients (50 males, 35 females; mean age 41.6 +/- 9.8 years) with dyspeptic symptoms who were examined by upper gastrointestinal endoscopy. The patients with a history of previous treatment with proton pump inhibitors, bismuth compounds or antibiotics were excluded. During the endoscopic examination biopsies were taken from antrum and corpus for rapid urease test and histological examination. Stool specimens were submitted to the laboratory and HpSA test was performed. H. pylori was considered in condition with rapid urease test and histopathological examination for H. pylori positive. Forty-six of 85 patients were positive and remaining 39 patients were negative for H. pylori with the rapid urease test and pathologic evaluation. When 0.160 was adopted as the cut-off value, in accordance with the manufacturer's recommendations; stool antigen has been detected in 45 of the 46 H. pylori positive patients. The sensitivity and specificity of HpSA test were 97.8%, 94.9% respectively. These results indicate that HpSA is a highly reliable diagnostic method for H. pylori infection.

Octenidine hydrochloride in hydatid disease.

Hydatid disease is still endemic in many devoloping countries and continues to be an important cause of morbidity. The objective of this study was to determine the in vitro scolicidal effects of octenidine hydrochloride in different concentrations using different exposure times. After hydatid cyst liquid was left to precipitate for 1 h to obtain cystic sand, various concentrations of octenidine (undiluted, 1% and 0.1% diluted) were added to concentrated hydatid cyst sediments for 5, 10, 15, 20, 25, 30, 45, and 60 min, and scolicidal effects of octenidine were compared with 20% saline and control group for the same times. It was found that undiluted octenidine had a strong scolicidal effect at 15 min compared to saline at 20%. One percent octenidine had a scolicidal effect at 30 min. However, 0.1% octenidine did not have enough scolicidal effect in 1 h. It was concluded that undiluted and 1% diluted octenidine might be used for scolicidal purpose in the treatment of hydatid disease.
