A simple technique for shoulder arthrography.

PURPOSE: To present a systematic approach to teaching a technique for arthrography of the shoulder. Using an adhesive marker-plate with radiopaque coordinates, precise sites for puncture can be identified and the need for fluoroscopic guidance obviated. MATERIAL AND METHODS: Forty-six glenohumeral arthrograms were performed in 45 patients; in 1 case involving examination of both shoulders. The stages of the technique are described in detail, as are the fundamental aspects of achieving an effective glenohumeral injection. Pain intensity was measured in all patients using a verbal description scale. RESULTS: Shoulder arthrography was successful in all cases. Average time taken for the procedure was 7 min, with no difference in the respective times required by an experienced radiologist and a resident. The procedure was well tolerated by most patients, with slight discomfort being observed in a very few cases. CONCLUSION: The arthrographic technique used in this study is simple, safe, rapid, and reproducible, and has the advantage of precise localization of the site for puncture without need for fluoroscopic guidance. The procedure described in this study can be of help in teaching residents and can reduce the learning curve for radiologists with no experience in arthrographic methods. It also reduces the time of exposure to fluoroscopy.

Histology and carbohydrate histochemistry of the prostate gland of the Brazilian four-eyed opossum (Philander opossum Linnaeus, 1758).

The histology as well as glycogen and mucosubstances were described in the cranial, middle and caudal segments of the disseminate prostate of the Philander opossum. The prostate tubuli show external, medium and internal zones. The stroma presents few thin colagenous fibers while the reticular fibers form a well developed network around the glands. The elastic fibers are scant and only observed amidst the excretory ducts (internal zones). The cranial segment is formed by the narrowest tubuli of the gland. The external and medium zones are formed by cubic or cuboidal epithelium with acidophilic granules in the cytoplasm. The internal zone is lined by slightly acidophilic columnar cells. Several excretory ducts join to each other, before emptying in the urethral lumen. The middle segment is formed by largest and longest tubuli of the gland. The secretory epithelium of the external zone shows 2 morphologically distinct types of cells. The medium zone is formed by cells showing strong acidophilia at basal part and slightly basophilia in the apical part. The caudal segment is formed by cuboidal acidophilic epithelium and show the largest lumen of the gland. The luminal secretion of the gland is constituted by acidophilic fluid and numerous globules. Neutral mucosubstances were demonstrated in epithelium and lumen secretion of the 3 prostatic segments; epithelial glycogen was not found in the medium zone of the middle segment. Sialomucins were detected in the cranial and middle segments. Sulfomucins were not found in the prostate of the Philander opossum.

Histology, glycogen and mucosubstances histochemistry of the bulbo-urethral glands of the Philander opossum (Linnaeus, 1758).

The histology and histochemistry of glycogen and mucosubstances were studied in the 3 pairs of bulbo-urethral glands (Cowper's glands) of the Philander opossum. The glands are covered externally by connective tissue and below it by striated muscle. The stroma is scant in elastic fibers whereas the reticular ones form a delicate network around the adenomeres. The lateral and intermediate bulbo-urethral glands are formed by long and ramified tubuloalveoli presenting thin wall when filled with homogeneous secretion. The glandular ductus are lined with columnar epithelium and present a sinus at the final portion. The medial bulbo-urethral glands are the smallest and they are formed by long and ramified tubuli. The secretory epithelium shows acidophilic and clear cells. The acidophilic secretion in the lumen is globous or irregular. Neutral mucosubstances were demonstrated in all bulbo-urethral glands, whereas glycogen was found in the intermediate and the medial glands. With reference to acidic mucosubstances, the intermediate glands secrete sulfomucin while the medial glands elaborate sialomucin.
