[A treatment method for patients with reflux esophagitis (an invention, copyright No. 1607825)].

The authors developed a method of complex treatment of patients with reflux esophagitis consisting in introduction via a probe of cooled 0.25% collargol solution and rose oil after intake of dimetpramide and simultaneous electrostimulation of the lower third of the esophagus. This method of treatment allows to control rapidly the pain syndrome, enhance treatment time and prolonged remission to 24 hours and more.

[Primary prevention problems in digestive organ diseases].

Questionnaires were used for examining workers of one of the shops of an industrial enterprise. Out of 300 persons examined, only 160 did not note any deviations from normal. Before filling in a questionnaire 10 persons had been registered at a dispensary for alimentary diseases; 76 persons noted that they had sensed deviations from normal functioning of the alimentary organs despite the fact that they did not regard themselves as being ill. Profound clinical, instrumental, laboratory and x-ray studies revealed peptic ulcer in 25, chronic gastritis in 25, chronic cholecystitis in 13, chronic pancreatitis in 2, and chronic enterocolitis in 11 out of the 76 persons examined. As to 54 persons who indicated the signs of disturbed well-being in the questionnaires, a detailed examination failed to discover any morphological or steady functional disorders so that, these persons were attributed to a group with premorbid conditions: abdominal discomfort (23), dyspepsia (11), gastrointestinal dyskinesia (10), and asthenia (10).

[The use of pH measurement in the diagnosis of gastroesophageal reflux].

pH-metry carried out in 210 patients with gastroduodenal diseases has diagnosed reflux as the underlying condition in 48 and as a concomitant disease in 75 cases. The degrees of gastroesophageal reflux severity were distinguished; this helped plan differentiated therapeutic schemes for these patients. Trials of these therapeutic schemes promise a higher efficacy of the therapeutic measures for gastroesophageal reflux.

[The frequency of detecting Campylobacter pylori in biopsies of the gastric mucosa].

Campylobacter pylori were detected in 70% of patients with diseases of the digestive tract. It was found endoscopically that presence of Campylobacter pylori is always accompanied by changes of gastric mucosa, mainly in the antral region. There were no specific symptoms characteristic of Campylobacter pylori gastritis. There was a close correlation between the presence of Campylobacter pylori and activity of the inflammatory process (active gastritis in 73.6% of patients).

[Quantitative determination of blood loss from the gastrointestinal tract using Metol in chronic diseases of the digestive organs].

The authors have developed a method for measuring the blood loss with feces using an available and inexpensive reagent--methyl-4-aminophenol sulphate (Metol). The diagnostic value of the method in ulcerative and hemorrhagic lesions of the alimentary tract is demonstrated, as well as its advantages over other techniques. The method helps choose a differentiated course of treatment with due regard for the hemorrhage severity.

[The level of immunoglobulins in the blood serum of patients with chronic diseases of the gastrointestinal system].

A clinical, laboratory, and immunological analysis of 253 cases with chronic gastrointestinal diseases has revealed changed levels and ratio of serum immunoglobulins of the three principal classes in 46.6% of patients. These changes have been the most marked in chronic hepatitides, the least so in chronic inflammatory diseases of the intestine, in peptic ulcer, and particularly in chronic gastritis. These shifts have been nonspecific but associated with clinical and laboratory signs of an active process, and may be regarded as additional criteria for an individual assessment of the process, its activity, and for planning the treatment, immunocorrecting therapy included.

[Morphological semeiotics of the celiac-sprue syndrome].

Sprue is a specific form of the gluten intolerance followed by the atrophy of villous structure and lengthening of crypts of the small intestine crypts. Hereditary factor has been implicated in its development and the immune hypothesis considering the sprue as a manifestation of the hypersensitivity to gluten is the most popular among the existing pathogenetic theories of this disease. The epithelium of atrophied villi reveals the most striking alterations. The content of lymphocytes in the epithelium is increased, high percentage of them being in the state of blast-transformation. The cells of the suppressor-cytotoxic type predominate among intraepithelial lymphocytes, the transfer of lymphocytes into the damaged epithelium is enhanced. Altered epitheliocytes have a smaller surface of the apical plasmalemma and a decreased activity of the associated enzymes. Intercellular epithelial contacts are altered and the basal membrane is damaged. There is an increase in number of T-lymphocytes and plasma cells synthesising IgA, IgM and IgG, in the thickened tunica propria of the small intestine mucous membrane. Lymphocytes-helpers predominate among T-lymphocytes and the ratio lymphocytes-helpers to lymphocytes of the suppressor-cytotoxic type is going down. Restoration of morphological alterations in the small intestine occurs when gluten is excluded from food.
