Acne urticata associated with chronic myelogenous leukemia.

A 56-year-old woman presented with a 6-month history of acne urticata, an intensely pruritic papulopustular eruption. Skin biopsy was nondiagnostic. Touch preparations of individual lesions were remarkable for numerous eosinophils. Peripheral blood count, bone marrow examination, and Philadelphia chromosome positivity confirmed a diagnosis of chronic myelogenous leukemia. A serum histamine level was markedly elevated. Treatment with hydroxyurea and antihistamines led to resolution of the skin eruption and improvement of the pruritus.

The biology of the pigmentary system and its disorders.

Melanin is the major determinant of skin color in humans. Abnormalities and diseases affecting pigmentation interrupt the production of distribution of melanin. The authors discuss the many diseases of hyper- and hypopigmentation.

Adrenal Insufficiency↗

Low-sulfur hair syndrome associated with UVB photosensitivity and testicular failure.

A 16-year-old male patient had a syndrome that included growth retardation, mental retardation, hair shaft abnormalities, neurosensory hearing loss, contractures of the fifth fingers, ultraviolet B (UVB) photosensitivity, small testicular size, and elevated levels of follicle-stimulating hormone. Reduced hair sulfur content and a characteristic pattern of light and dark birefringent bands, seen when hairs were viewed with polarization microscopy, were typical of the low-sulfur hair syndrome. UVB photosensitivity and testicular failure have not previously been identified as components of this syndrome.

Pemphigus foliaceus and proteinuria induced by alpha-mercaptopropionylglycine.

Pemphigus foliaceus and proteinuria occurred in a patient with cystinuria treated with alpha-mercaptopropionylglycine. This drug is similar to D-penicillamine in structure and function, and it is being used in patients who are unable to tolerate D-penicillamine. Although side effects of D-penicillamine are common, no major adverse reactions have been reported previously with alpha-mercaptopropionylglycine. Pemphigus foliaceus and proteinuria occurring during therapy with alpha-mercaptopropionylglycine and D-penicillamine are discussed.

Plasma androgens in women with acne vulgaris.

We have studied a group of young adult women of mean age 23.8 +/- 6.5 (SD) years with only acne (A, n = 46), only hirsutism (H, n = 10), and acne plus hirsutism (A + H, n = 19) who sought dermatologic care. We measured the androgens, total and free testosterone (T), free 17 beta-hydroxysteroids (17-beta), dehydroepiandrosterone sulfate (DS), and the androgen precursors 17 alpha-hydroxypregnenolone (17-Preg) and 17 alpha-hydroxyprogesterone (17-Prog), as well as testosterone-estrogen binding globulin in all patients. Plasma hormone levels of the patients were compared to those of 23 controls of mean age 25.6 +/- 6.6 years who had neither acne nor hirsutism. Mean levels of all hormones measured, except 17-Preg, were elevated in the women with acne. Fifty-two percent of Group A, 60% of Group H, and 63% of Group A + H patients had at least one abnormal hormone level. The most frequently elevated plasma androgens in all the women with acne were: free T 25%, free 17-beta 23%, and DS 19%. Total T was high in only 12%. Elevations of plasma androgens were present in some women who did not have hirsutism or irregular menses. Identification of endocrine abnormalities in women with acne may potentially offer an opportunity for hormonal therapy.

Lupus erythematosus with perforation of the pinna.

Cutaneous inflammation occurred in a 56-year-old woman who had chronic discoid lupus erythematosus. Lesions on the pinna resulted in perforation. This finding has not been reported previously in association with lupus erythematosus.
