[Histologic aspects of seborrheic keratosis].

The histopathological aspects of seborrheic keratosis have been studied in order to establish a basis to their classification upon structural morphology and cytology. The 210 seborrheic keratosis studied could be classified in several patterns among which we must point out the three most important ones: hyperkeratotic, adenoid and acanthotic. This last one can offer two very different images: uniform acanthotic (this one including at the same time, a variant of "small basal cells") and reticular acanthotic. Special mention has to be made as well to the activated and pure forms which, though being the result of evolutive accidental phenomena, no doubt they have very particular and distinctive histopathological aspects. Further, other variants are discussed such a clonal and acantholytic seborrheic keratosis, as well as the significance of inverted follicular keratosis and of melanoacanthoma.

[Apocrine nevus].

We present the case of a 32-year-old woman with a congenital lesion on her left cheek that histopathologically was composed of numerous mature apocrine glands. The diagnosis of pure apocrine nevus, a rare lesion of that our case probably represents the sixth described in the literature, was made.

[Hyperkeratosis lenticularis perstans (Flegel)].

The case of a female patient, 67 years old, with hyperkeratotic papules in the lower extremities which are characteristic of hyperkeratosis lenticularis perstans is reported. The clinical, histological and ultrastructural findings are shown. We have recorded the absence of Odland bodies in the area of the lesion, the persistence of desmosomes in the stratum corneum and lymphocytes with cerebriform nuclei within the inflammatory cell infiltrate. She was satisfactorily treated with etretinate and topical 5-fluorouracil We have reviewed the existing literature on this subject.

[Squaric acid dibutyl ester in the treatment of severe alopecia areata].

The results of the treatment with topical applications of squaric acid dibutylester in seven cases of severe alopecia areata are reported. In adult patients this form of therapy is very effective to achieve a total hair regrowth, but its use should be strictly limited to the most serious cases, recalcitrant to other therapeutical methods.

[Dermatologic aspects of collagen disease (author's transl)].

In this brief review the dermatologic aspects of collagen diseases are stressed. Cutaneous manifestations are the main clinical features from the standpoint of diagnostic evaluation of these disorders. Juvenile rheumatoid arthritis, lupus erythematosus, dermatomyositis and scleroderma are considered.

Woringer-Kolopp disease: pagetoid reticulosis.

A typical clinical and histologic case of so-called 'pagetoid reticulosis' or Woringer-Kolopp disease is reported. This is a chronic cutaneous condition usually with a single focus or at most with a few patches confined to a circumscribed region. A disseminated form has been described as Dupont-Vandaele type, but its relationship to the classic Woringer-Kolopp disease is uncertain. The nosological location of pagetoid reticulosis has not been established, but there are reasons to regard it as a particular and well-defined entity. In relation to the nature of this condition and to the origin of the 'pagetoid cells', it is tempting to consider the possibility that Woringer-Kolopp disease might be a lymphoproliferative epidermotropic disorder, classifiable as a benign form of 'cutaneous T-cell lymphoma', but the early histologic changes are rather indicative of an epidermal origin.

[Macular amyloidosis].

Fifty-five patients with presumptive macular amyloidosis were investigated for the presence of amyloid in dermal tissue. In twenty-six of these patients the diagnosis could be confirmed by the demonstration of amyloid deposits by optical and/or electron microscopy. In this paper attention is called to the frequency of oligosymptomatic forms of macular amyloidosis, which are often overlooked in spite of the usual existence of itching. These inconspicuous forms are termed "minor macular amyloidosis", and they consist of tenuous brown macules on the upper back, irregularly interspersed with the normal skin and with speckles of whitish discoloration. The diagnosis must be verified by the finding of dermal deposits of amyloid. For that purpose the electron microscopy is a much more reliable procedure than conventional methods or thioflavine T.

Amyloid↗

Mixed (lichenoid and macular) cutaneous amyloidoses.

Between October 1972 and Paril 1974 a total of four patients suffering from lichen amyloidosus were admitted to the wards of the Department of Dermatology, University Hospital, Valladolid. All these patients also showed a macular amyloidosis associated with the lichenoid eruption. The authors emphasize that macular amyloidosis is quite commonly found as an associated and closely related condition in lichen amyloidosus.
