[Single-layer anastomosis of the colon].

Presented are the analysis of 134 single-layer colon anastomosis done between years 1983 and 1993. Essential parameters of those are compared with two-layer colon anastomosis - 65, done at the same period, as well as with wider series done in the world in a last couple of years. Main task of the analysis is to point to contemporary developments of suture technique in the world and at home, which is being simplified in technical terms, characterized by time and material savings without affecting anastomosis quality. Those characteristics have been attained due to good preparation of intestines, improved materials, antibiotics and due to finding out that submucosis and subserosis are carriers of suture firmness. On processed material when single-layer suture was used (134) we had average of 8.2% clinically evident fistulas - single-layer individual (78) 3.8% a single-layer continuous (58) 14%. With two-layer suture technique (65) we had 14% clinically evident fistula. Average mortality with both techniques was 4%.

[Diaphragmatic rupture in present-day injuries].

In this report the authors took all material found on KBC Zemun for period from 1968 to 1988 on 43 causes traumatic rupture of diaphragms treated on this clinic. On their own experience and taking the experience of the others the authors found that: Traumatic rupture of diaphragm is a consequence of thoracoabdominal compressive trauma, which it accompanies in 3% of cases. Any traumatic rupture of diaphragm is an acute surgical state which requires an urgent surgical treatment. No other treatment is justified. Radiological method is applied in diagnosing the traumatic rupture of diaphragm. The surgical operation ought to secure a good detection of accompanying injuries. They are surgically taken care of by breaking the communication between the two cavities and, by providing their proper drainage. The major factor causing this type of traumatism presents the traffic accidents. The death rate with these traumas is very high and largely depending on accompanying injuries, reaching 20.9% in the material that we studied.

The effects of local application of 1% dopamine on intraocular pressure and pupillary diameter.

This paper deals with the experimental results of the effect of 1% dopamine on intraocular pressure, pupillary diameter and hemodynamic parameters after its local application in the rabbit eye. Our results show that locally applied dopamine caused an initial increase of intraocular pressure (IOP) (1 h), followed by its decrease that was the most pronounced 3 hours later. Mydriasis was an immediate effect of dopamine, which persisted until the end of the second hour. There were no changes in blood pressure and heart rate. Although the exact mechanism of dopamine effect on IOP is not known yet, our results point out that this is mainly due to the decrease fo aqueous humor formation.

[Role of nimodipine in the therapy of subarachnoid and intracerebral hemorrhage].

Syndromes of intracranial hemorrhage, and particularly subarachnoidal, i.e., intracerebral hemorrhage (SAH and IH) present clinical entities that are the most severe conditions in neurology. Timely recognition, diagnosis and adequate therapy are imperative. The most important factor that aggravates an already difficult prognosis of those entities is cerebral vasospasm. Upon the presented facts, the aim of this investigation was to establish the value and role of administration of selective calcium channel blocker--nimodipine in patients with SAH and IH compared to the degree of neurological and functional impairment, as well as the recovery of the function of consciousness compared to the patients with those syndromes from an earlier period, who were not treated with this medicament. Investigation comprised 30 patients who received nimodipine and 20 patients without this agent in therapeutic program. Results of the investigation confirmed significant difference concerning the neurological recovery, improvement of functional capability and recovery or consciousness disturbances, respectively, in patients who received nimodipine compared to the group without this agent. It can be concluded that nimodipine as calcium channel blocker with multitopic pharmacological effects on mechanism of SAH or IH development, respectively, as well as on the development of complications of those syndromes, particularly to the development of vasospasm and reactive ischemia, with the improvement of hemorrheologic disorders deserves to be included as the unavoidable segment of therapeutic program of SAH and IH syndrome immediately after clinical phenomenology is revealed.

The value of ophthalmoscopy in the diagnosis of arterial hypertension in patients with ischemic brain disease.

Beside dyslipoproteinemia, one of the key risk factors for the onset of brain atherosclerosis, as well as ischemic brain disease (IBD) is arterial hypertension. Significant number of patients is not aware of their hypertension, and a paradoxical blood pressure decrease can occur at the onset of IBD, due to the failure of autoregulation mechanisms. Likewise, valid anamnestic data can not frequently be obtained due to difficulties in communication with patients. Regarding these facts, our hypothesis was that ophthalmoscopy in patients with IBD had the greatest sensitivity in the diagnosis of hypertensive disease, its duration and severity. For that reason, the purpose of this study was to determine the significance of ocular fundus examination in those patients with IBD who were not aware of their hypertension, or the high blood pressure was not registered at the admission. Study comprised 140 IBD patients selected upon the following criteria: ophthalmoscopy was performed by the same ophthalmologist, and IBD was diagnosed according to clinical criteria and by brain computerized tomography. Results of the study demonstrated that 26 (18.6%) patients, although not aware of having hypertensive disease, had grade I hypertonic fundus, 14 (10%) had grade II, and 8 (5.5%) had grade III hypertonic fundus, which indicated the high sensitivity of ophthalmoscopy in the diagnosis of hypertensive disease, as well as its duration and severity. This is particularly important in patients with negative history of hypertension, and also suggests the significance of routine ophthalmoscopy in normotensive patients.

The significance of determination of the fraction of creatinine-phosphokinase in patients with acute ischemic brain disease.

Heart and brain vascular diseases are the leading causes of mortality in the world. Cardiac complications can frequently occur during the development of cerebral ischemia. The aim of this study was to establish the possible changes in fractions of creatinine-phosphokinase as the sensitive laboratory index of parenchymal lesion of brain parenchyma and the presence/absence of risk factors for ischemic brain and heart disease. The study comprised 80 patients with acute ischemic brain disease (AIBD), without the history of previous coronary disease. Blood samples were taken in all patients within the first 48 hours from AIBD onset aiming to determine a total (muscular MM) and heart fraction of creatinine-phosphokinase (MB), and brain parenchyma ischemia was confirmed by CT or MR scan of the head. A detailed history of the risk factors for ischemic brain disease (IBD) and ischemic heart disease was taken from all patients with AIBD, and the profile of glycemia and lipid status were determined, and blood pressure was measured 6 times a day. Independent variables in statistical analysis were: age, degree of severity and the side of neurologic event, size of ischemic lesion and maximal values of systolic and dyastolic pressure. Dependent variables were the values of fractions of creatinine-phosphokinase (CPK). Control group (n = 40) comprised patients with neurologic diseases of non-vascular origin. All parameters as well as their interrelations were statistically analyzed. The results revealed significant correlation of the increased levels of CPK of MM and MB fraction with the size and place of ischemic lesion in the right cerebral hemisphere, which was highly significant for MB fraction in the total group of patients with AIBD, and for MM fraction, only for cases of more severe IBD. Highly significant increased values of those fractions were also observed compared to the control group of patients.

[Effect of aging on visual evoked potentials].

Experimental and clinical studies, aiming to establish the influence of aging on latence, amplitude and types of waves of visual evoked potentials (VEP) are numerous. It was concluded that P-100 latence prolonged with aging, particularly after the age of 55, while P-100 amplitude had been gradually decreased during the life. During the aging, senile changes in the eye and optic nerve (for example, senile miosis, degenerative retinal changes, geniculostraital defect) can be reflected to the changes in VEP (P-100). Aim of our study was to establish if the aging influences the alteration in latence and wave amplitude in VEP, as well as to standardize normal values of VEP in different ages. VEP was registered in 135 subjects classified into 3 groups. Group I comprised 49 subjects, aged 17-30 (average age 20.92 years), group II comprised 34 subjects aged 31-54 years (average 42.55 years). Group III--over 55 years (average age 69 years) comprised 52 subjects. There were 55 females and 80 males. Results have shown that the mean value of amplitude for both eyes in the first two groups of younger subjects was 9.03 microV and 8.91 microV, respectively, while in the group of older subjects, it was slightly decreased--8.54 microV. P-100 latence prolonged insignificantly until the age of 55, and afterwards in every 5 years it increased for 1.5 ms. Amplitude was insignificantly lower latence in VEP was insignificantly prolonged until the age of 55, and afterwards, the increase was significant. Amplitude was decreased insignificantly, gradually during the life.

[Vitreoretinal proliferation as a cause of tractional retinal ablation after severe perforating injury].

The main idea of this paper was to investigate the influence of proliferative reaction in vitreous body, in case of serious injuries of the eyeball on the development of complications. It was shown that the presence of hemophthalmia which intensified PVR process and anteroposterior traction influenced the intensity of PVR and also the development of retinal ablation. The complete perforation of the posterior wall of the eye ball (duplex perforation) with the small foreign body would produce bigger scar on the posterior wall than in case of singular perforations, which would usually be enough to control anteroposterior traction towards vitreous body and prevent retinal ablation.
