[Chronic abdominal pain: lipoma of the small intestine].

The authors present a case of submucous lipoma of the jejunum in a 6 years old boy. They point out the rarity of these tumors in children and the unusual aspect of the lesion, the circular, stenotic shape has never been noted elsewhere.

[Internal hernia in the duodenal fossae. 3 cases].

Para-duodenal strangulated hernia is an unusual operative finding. The entraped position of intestinal loops in a vascularized pouch allow to recognize the lesion, the site of the neck of the hernia point out its anatomic form. Pre-operative diagnosis seems difficult to access in spite of numerous investigation'means. The strangulation is generally few tight, so the treatment is very easy outside of a carefully dissection of the neck of the left para-duodenal hernias.

[Abdomino-thoracic digestive duplications. General review apropos of 2 cases].

The authors present two personal observations of thoraco-abdominal duplications of the foregut and analyze 48 others cases collected in the literature since the first one related by Budde in 1912. Anatomic study, evolutivity of the malformation, fully expression of notochordodysraphia, allow an analysis of symptoms, which must actually lead to an acute diagnosis. Total excision in a single stage through a double thoracic and, after, abdominal approach is the logic treatment.

[Acute intestinal invagination in infants and children. Analysis of 125 cases treated surgically].

125 children were treated for acute intussusception with :-- Barium enema to establish the diagnosis and to do partial reduction--Even when total reduction is radiologically obtained a surgical control is done with routine appendicectomy.--The rate of recurrence is 3,2% occurring from 1 to 2 years after surgery. In 18% of our cases (23 patients) an organic etiology was discovered at surgery but in one patient only this was documented ona barium enema which demonstrated the necessity of systematic laparotomy.

Acute Disease↗

[Duodenal duplications. Apropos of 3 cases].

The treatment of three cases of double duodenum and review of 30 cases from the literature show the excess of certain surgical attitudes. Duplication is a benign lesion, most often found in the infant or newborn and for which treatment must remain simple. Mutilating excision procedures should be avoided: gastroduodenectomy, duodenopancreatectomy. Total resection, which is rarely possible, or subtotal resection with destruction of active mucosae, and internal duodenal or rarely jejunal bypasses are satisfactory forms of treatment in the immense majority of cases.

[Acute non-traumatic pancreatitis in childhood. Report of 9 cases (author's transl)].

The authors report 9 cases of acute non traumatic pancreatitis in children. The most common symptom in their patients is atypical abdominal pain often associated with vomiting (5 cases) and shock (4 cases). Among laboratory investigations: --High serum amylase level (average: 1 045 UI/l) is constant, associated with hypocalcemia and hyperglycemia in 3 patients. --Radiographic findings on plain film of the abdomen are diagnostic in 4 cases. --Abdominal ultrasound is the most reliable test and positive in the only patient on which performed. Among etiologies, drug induced pancreatitis is the most common (5 cases) due to combined Prednisone-L-Asparaginase (4 patients): --A duodenal ulcer and a case of choletithiasis are reported. --In 2 patients no determinant factors are found. A good response to parenteral nutrition, gastric suction and antisecretory agents is observed in 7 cases. 2 leukemic patients died shortly after the acute episode.

[One case of complete reduplication of the bladder (author's transl)].

We report one case of complete reduplication of lower urinary tract associated with bilateral vesico-urethral reflux, hip dislocation, incomplete sacral agenesis and neck malformation. The two bladders are physiologically functional and we only operated the bilateral reflux by Leadbetter-Politano procedure. A review is presented of the twenty-two published cases, with the classification.

Effects of acetylsalicylic acid and indomethacin on growth hormone secretion in man.

To investigate the possibility that prostaglandins (PG) take part in the control of growth hormone (GH) secretion in humans, we have studied the effects of protracted and acute administration of acetylsalicylic acid (ASA) and indomethacin (ID), two PG synthesis inhibitors, on basal and insulin-stimulated GH secretion in normal volunteers. In eight subjects, oral administration of 3-2 g daily of ASA for 4 days clearly reached GH response to insulin hypoglycemia (p less than 0.01, ANOVA). In six additional subjects, GH response to hypoglycemia was not modified by a 4-day oral treatment with 300 mg daily of ID. The pattern of plasma free fatty acids (FFA) and blood glucose during the insulin tolerance test was not significantly affected by ASA treatment. After ID the O time value of the above parameters was somewhat higher than under basal conditions, while the drop of blood glucose, but not to FFA, was slightly more pronounced. Acute oral administration of 1.5 g ASA in 12 subjects did not appreciably modify baseline plasma GH, FFA, and blood glucose levels. By contrast, a single oral dose of 100 mg ID in 12 subjects caused a moderate but significant rise (p less than 0.05) of plasma GH levels together with a clear elevation (p less than 0.01) of plasma FFA and blood glucose levels with respect to a group of controls treated with a placebo. Collectively these results are compatible with the possibility that PG play a physiologic stimulating role in the control of GH secretion, although an effect of ASA and ID unrelated to PG inhibition cannot be ruled out, In any event, in view of the number of endocrine and metabolic alterations induced by ASA and ID, these drugs seem to merit further study.

[Bacterio-parasitic interactions. Enterobacteria and schistosomes (Salmonella - schistosome association)].

The description of a case imported from Chad of an association of Salmonella typhi - Schistosoma haematobium and, in this connection, a new revised general account. The association Salmonella-Schistomas is wide-spread. It corresponds to a salmonello - schistosomo - micro - association by the fixation of precise bacteria in the case of a bacteriemia on the cutaneous surface of male schistosomes in clearly defined places. These Salmonella constitute a permanent antigenic solicitation and they are capable of mobilization and rejection (bacteriuria after passage through the blood-stream) whereas bacteriosis remains asymptomatic. In fact, on the practical level, a clear and absolute distinction must be made between:--the bacteriemia with the concomitant bacteriuria, which are largely physiological in nature, very frequent and originating from various germs, connected with the emunctory function of the kidneys and which can be observed in any subject.--the schistosomal bacteriuria which are specific entities and evidence of a veritable micro-association of a symbiotic nature between the Salmonella and the schistosome, a micro-association which the bacteriuria reveals unexpectedly whereas the clinical symptoms are absent, not clearly discernible or totally misleading.

[Bilateral persistence of the common mesonephric duct in children. Apropos of a case. Review of the literature].

We report a case of bilateral persistence of the common mesonephric duct in a child. This anomaly is an error of implantation of the vas deferens in the distal part of the Wolffian duct. This abnormality is extremely rare (6 cases have been reported in the world literature). The diagnosis is usually incidental during laparotomy or on retrograde cystography (vesico-ureteric and vesico-deferential reflux). It is simple to treat: bilateral deferential ligation in view of the context of multiple malformations, in order to prevent ascending genital infections (prostatitis, epididymitis, testicular abscess). Only one case has been reported in adults, presenting in the form of a septic complication. This diagnosis must be considered in a context of atypical urogenital infection in a subject with other congenital abnormalities.
