Clinical experiences in the Gambro-preservation unit: analysis of 101 human cadaver kidneys.

101 human cadaver kidneys were preserved by means of hypothermic pulsatile perfusion in the Gambro perfusion unit with human albumin as perfusate. Perfusate flow and some biochemical parameters were assessed during perfusion and correlated with renal function after transplantation. There is a good correlation between perfusate flow and recovery of physiological renal function. LDH released to the perfusate as viability test is of limited value: a direct correlation was only found between LDH activity and perfusion time, and LDH activity and ischemia time. Determination of pH and lactic acid level 1 h after beginning of perfusion is without practical value according to our experience. The quality of albumin in perfusion solution influences the immediate graft function.

[Quantitative angiographic studies for the differential diagnosis of early acute kidney failure of ischemic and rejection-damaged grafts].

On 22 auto- or homotransplanted kidneys of dogs a quantitative angiographic evaluation of the corticalis perfusion was carried out. For this purpose four special parameters were evaluated. With the help of the linear discriminance analysis we succeed in a sufficient secure separation between autografts impaired by ischaemia or shock on the one hand and homografts impaired by rejection on the other. It is referred to the prognostic significance of angiography in human graft kidneys with disturbed function.

[Flow volume-velocity relationship in the circulation of acutely failing kidney transplants (experimental study)].

The volume velocity-relation of renal blood flow was examined in transplanted dog kidneys during the oliguanuric phase of the acute renal failure (ARF). Under these circumstances it is possible to separate the ARF in consequence of ischemia or hypoxia of the transplant or in consequence of rejection. A significant relation between the total blood flow and the mean flow correlation exists in the acute functional failure of ischemically injured autotransplants as in the case of autochthonous kidneys. This velocity is also kept in the acute insufficiency of autotransplants induced by shock although there exists a stronger redistribution of cortex circulation than after the ischemic insultus. Extreme low or high circulatory velocities without a relation to the blood flow originate from the deficiency of extensive vascular regions and the formation of arteriovenous shunts in acutely failing rejected homotransplants.

[Clinical and experimental results on the significance of kidney damage for long-term prognosis after kidney transplantation].

In the clinical study the influence of the differently long times of warm ischaemia and of the duration of the acute renal insufficiency post transplantationem on the late prognosis of the grafts and the survival of the patients as well as on the incidence of complications in 93 patients who first underwent a transplantation of the CD-kidney and who more than two months after the transplantation lived with a working kidney was examined. A direct relation between the duration up to the beginning of the function of the kidney and the long-term prognosis of the patients was proved. In the group with immediate beginning of the function of the kidney at an average WIZ of 13 minutes the creatinine clearance was significantly higher and the incidence of severe infections in the late phase was lower than in the groups with longer WIZ and very retarded beginning of the renal function. Haemodynamic investigations on the autograft model of the dog after 30 minutes WIZ resulted in a pathologically increased renal vascular resistance and in a restricted filtration function up to 4 weeks post transplantationem.

[Viability testing during preservation of dog kidneys with an albumin-containing perfusion solution].

In models experiments on kidneys of dogs the prognostic significance of hydrodynamic, metabolic and enzymatic parameters in the machine preservation with an oxygenated-albumin-containing solution was tested. While the hydrodynamic and metabolic measuring values did not give a clear reference to different ischaemic restrictions of the functional capacity after autotransplantation, the activities of lactate dehydrogenase and N-acetyl-beta-D-glucosamidase in the perfusate proved to be suitable criteria of vitality. In this case the determination of lactate dehydrogenase is to be preferred on account of the smaller methodical expenditure.

[Effect of phenoxybenzamine on the renal and general circulation in the hypovolemic hypotonic dog].

For the estimation of the haemodynamic and metabolic effect of 1 mg/kg phenoxybenzamine in a kidney donor with craniocerebral trauma model experiments were performed on dogs with hypovolaemic hypotension. The results show that phenoxybenzamine leads to vasodilatation in a constant aortic pressure of 80 mm Hg in the systemic circulation as well as in the renal circulation with different speed of reaction. Thus the arterial and venous O2 and CO2 partial pressure normalizes and the diuresis increases. On the other hand, the hypotension-conditioned metabolic acidosis and the symptoms of an acute renal failure may not be influenced.

On the therapy of urolithiasis in chronic renal failure.

A resigning attitude in respect to patients suffering from nephrolithiasis and manifest renal failure is not justified anymore in view of the advances in diagnosis and therapy. During the period 1964-1975, at the urological clinic of the hospital Friedrichshain 188 patients suffering from nephrolithiasis and renal failure have been treated: 120 surgically and 68 conservatively. The operative-therapeutical conception is outlined. The pre- and postoperative hemodialysis therapy is described in detail and the different indications of hemodialysis according to the stage of renal failure are elaborated. Tabulated data demonstrate the fate of all surgically treated patients.

[Value of hemodynamic parameters in determining kidney transplant function (animal experiments and clinical observations)].

From the demonstrated results is to be concluded: 1. The haemodynamic and functional inquiries on animal experimental and clinical kidney grafts suppose that the tubules and the renal vascular system are comparably affected by the damaging influences of the different preservation methods. 2. This pathogenetic relationship is apparently maintained also in the regeneration phase; i.e. the tubular restitution is connected with an improvement of the blood-supply of the organ. 3. The discrepancy between the morphologic and functional findings of storage and perfusion preserved kidneys shows that the efficiency of the graft is not only defined by definitive structural changes, but also by other factors, such as metabolites, enzymatic and humoral influences, changes of the local blood composition, regional conditions of blood-supply, and so on. 4. Due to the significant relation between restrictions of the renal blood-supply and the renal function after transplantation haemodynamic parameters are suitable for the judgment of renal preservation methods and for the prognostic estimation of renal grafts.

Intraoperative blood flow in transplanted human kidneys and its relation to subsequent function.

The value of intraoperative blood flow measurement during clinical kidney grafting to predict subsequent function was studied in the course of 19 consecutive transplantations. The results indicate that when blood pressure and heart rate of the recipients are within normal limits there exists a significant relationship between renal blood supply and recovery of the kidney function. The operative blood flow into the graft correlated, moreover, with the state of presensitization of the recipients, as determined by antibody-against-panel assays. The possible causes of these relationships are discussed. It is emphasized that actual viability of allotransplanted kidneys depends on both pre-and intraoperative pathophysiological processes.

[Significance of LDH determination in renal perfusions as criterion of vitality in kidney transplantations (author's transl)].

The activity of LDH in perfusate on human kidney and in experiments on dog kidney was investigated during the preservation by the Gambro perfusion apparatus. The enzyme activity of perfusate was characterized by a quotient of volume activity in relation to the perfusion flow rate. With this method we are able to distinguish the values of damaged and normal kidneys. A correlation was established between this quotient and the function resumption day in human kidney transplantation.
