Changing patterns in the vascular form of Ehlers-Danlos syndrome.

We report and analyze two cases of Ehlers-Danlos syndrome (EDS) type 4. The first manifestation of the disease was a spontaneous perforation of the colon in a 47-year-old man; he was successfully reoperated on five years later for the rupture of an abdominal aortic aneurysm. Abdominal pain demonstrated the syndrome in a 33-year-old woman in whom multiple abdominal aneurysms were found. A ligation of the anterior tibial artery for spontaneous rupture was performed five years later. Light and electron microscopic studies of the skin disclosed similar alterations in both cases. The diameter of the collagen fiber bundles was reduced and the diameter of collagen fibrils was increased. It appears that EDS type 4 might be less characteristic than has been previously described. Classification of the different types of EDS according to electron microscopy is not possible.

[An oculoglandular syndrome and ophthalmia nodosa].

A case of ophthalmia nodosa associated with an oculoglandular syndrome is described. The patient, scratched by a cat six-and-a-half months earlier, presented a positive cat-scratch disease antigen test. The possibility that the oculoglandular syndrome is due to the reactivation of a latent virus, the cause of cat-scratch disease, is discussed.

[Correlations between steroid receptors and the histopathologic differentiation of carcinoma of the breast in women].

This retrospective study on 545 primary mammary carcinomas shows highly significant negative correlations between the presence of oestrogen and progesterone receptors on the one hand and nuclear grading, degree of necrosis or of inflammatory infiltration and histological grading on the other hand. The relationship between receptors and histological grading is not as highly significant as reported by others because of the criteria of grading that were selected. All these correlations disappear when only premenopausal patients are considered. Receptors are most frequently positive in tubular and lobular carcinomas but are rarely found in medullary carcinoma. The presence of receptors bears no link with the size of the tumour. When a tumour is too small for both, histopathological and biochemical analysis to be performed, it is possible to predict the existence of positive receptors thanks to histological criteria of tumour differentiation with a probability of 75% for 67% of patients.

[A rare variety of polyendocrine neoplasms: the MEN IIb and MEN III syndrome].

The MEN IIb syndrome is a rare form of multiple endocrine neoplasia, clearly defined more than 15 years ago, since which time more than 100 cases have been identified. It has a characteristic phenotype appearance which is unique and should permit more frequent diagnosis. The patient who is the subject of this study had passed through several different hospital departments without his very characteristic appearance being recognized or a diagnosis established. The marfanoid aspect of such patient is however very obvious, with kyphosis, skeletal asymmetry and funnel chest. The facial appearance of blunt features and thick lips is even more specific. The tongue and digestive mucosa are covered with small polypoid lesions which are submucosal ganglioneuromas. The visible presence of fine hyperplastic nerve fibres on the cornea is pathognomonic. This syndrome occurs sporadically but is generally transmitted genetically in an autosomal dominant manner. A medullary thyroid carcinoma, followed or preceded by pheochromocytoma (often bilateral), is associated with these dysmorphic signs. The tumours are themselves preceded by C-cell hyperplasia or adrenal medullary hyperplasia. The ganglioneuromas which predominate in the digestive tract exist from birth. The accompanying symptoms may, initially, be mistaken for inflammatory bowel disease. The prognosis is poor, since it relates to the evolution of the medullary thyroid carcinoma which is particularly malignant and presents at an early age. We have been following the course of this unusual disease of the APUD system in a young man, now aged 26 years, for over 2 years.

[Multifocal pulmonary bilharzioma due to Schistosoma mansoni].

A case of mixed schistosomiasis in an African female immigrant from Cameroon is reported. Schistosoma mansoni was found in a symptomatic double abscess of the lower lobe of the left lung (1981). S. haematobium was present in the stool and the rectal mucosa but the patient had no symptoms. In Africa the patient had been treated for urinary and intestinal bilharziosis with niridazole in 1979, at which time she did not present pulmonary symptoms. After her arrival to Switzerland (1980) she complained of thoracic pain and bloody purulent sputum. A solitary pulmonary infiltration in the left lower lobe and eosinophilia were noted. Tuberculosis was suspected and, despite negative bacillary findings, trial treatment with tuberculostatics was started. As a smaller infiltration appeared beside the first, left lower lobectomy was performed followed by niridazole treatment. The exceptional diagnosis of pulmonary bilharzioma should be considered in patients with a solitary pulmonary infiltration who come from an area endemic for bilharziosis or who show clinical or laboratory signs of a present or past bilharzial infection.

[Reintervention for recurrent angina pectoris after aortocoronary bypass].

In this department between 1969 and 1980, 1116 angina patients underwent aorto-coronary venous bypass operation for revascularization of the myocardium. Thirty-four (3%) of these patients later underwent reoperation for anginal recurrence. Recurrence of angina was due principally to obstruction of an aorto-coronary venous bypass graft (48.6%) and progression of atheromatous disease in non-grafted coronary arteries. 9% of reoperated patients had a myocardial infarction postoperatively. Overall mortality was 6%. Quality of life must be considered excellent when it is observed that 87% of these patients enjoyed symptomatic improvement and as many as 63% were still asymptomatic following an average observation period of more than three years (38.7 months). On the basis of these encouraging results, all patients with recurrence of angina should undergo further coronary investigations with a view to reoperation.

Lytic lesions of the femoral neck in rheumatoid arthritis simulating pigmented villonodular synovitis or malignancy.

Large lytic lesions, relatively asymptomatic, involving the femoral neck and the base of the head are described in two patients suffering from a classical seropositive rheumatoid arthritis. Histological examination failed to reveal signs of malignancy, infection or pigmented villonodular synovitis. There were no rheumatoid nodules but a chronic hypertrophic villous synovitis was found. Rheumatoid synovium may invade the superior extremity of the femur; this fact is important in the differential diagnosis of destructive lesions of the femoral neck in RA.

[Inflammatory pseudotumor of the lung. Apropos of a case].

A case of inflammatory pseudotumor of the lung is reported and the essential features are recalled. Depending on morphology or pathogenesis this disease has received various designations, e.g. xanthogranuloma or post-inflammatory tumor. When not found accidentally this relatively rare lesion involves few very specific symptoms and usually occurs in young people. Radiographic and endoscopic investigations are only exceptionally of help in specifying the nature of the disease, and only histology provides any diagnostic certitude. The treatment for this lesion, the prognosis of which is excellent, is surgical.

Intraepithelial carcinoma of the oesophagus: endoscopic morphology.

The endoscopic detection of 18 "early" hypopharyngo-oesophageal carcinomas, has allowed us to perform a detailed study of the morphological correlation between endoscopy and histology (in 10 cases). We have thus defined 4 different endoscopic types of intraepithelial carcinomas; their morphology, mapping and evolution are greatly variable. In high risk groups (heavy smoking and alcohol consumption, ENt-cancer) the multicentricity of intraepithelial carcinomas (80%) entails a thorough endoscopic screening of the upper digestive tract (mouth, pharynx, oesophagus) and of the lower respiratory tract (larynx, trachea and bronchi).

[Liver tumors and oral contraceptives].

Benign liver cell tumors in young women are reported with increasing frequency. An association between benign tumors and oral contraceptive use was first suggested in 1973 by Janet Baum. During the past ten years we have observed 15 benign liver cell tumors, all of the focal nodular hyperplasia type. Ten of the 15 women were taking or had taken oral contraceptives. This association was found not to be statistically significant (0.06 greater than p greater than 0.05). We also diagnosed 2 liver cell carcinomas in women who had taken contraceptives.
