[Vaginal ecosystem].

The vagina is original biotype with its own ecosystem, according to medical ecology science. This ecosystem has dynamic, but very unstable equilibrium. Disturb equilibrium is known as a disbiosys. It was discussed different components of this ecosystem: morphology of vaginal walls, vaginal liquidity, lacto-acid and residental flora, "invader" microorganisms, vaginal acidity, immune processes. It was shared our own experience with medicine Polygynax, remedy of Laboratoire Innotech International (Paris, France). Polygynax has such an advantage - rapidly restore disturbed ecological equlibrium in case of bacterial vulvovaginitis, caused mainly of intestine pathogenic flora.

[Treatment of bacterial vaginosis with high dosage metronidazole and lactic acid].

INTRODUCTION: Bacterial vaginosis (BV) is one of the most frequent vaginal infections in women of reproductive age. This condition is a disbalance of vaginal flora that is expressed in a considerable displacement of normal lactobacilli flora by opportunistic and facultative microorganisms. Despite the moderate or missing clinical manifestation this dysbacteriosis is linked with many complications in obstetrics and gynecology. AIM: To evaluate the effect of treatment of bacterial vaginosis with the product Arilin rapid containing vaginal Metronidazole in high doses and a following vaginal application of lactic acid in a part of the patients. MATERIALS AND METHODS: For the period May 2001-August 2001, bacterial vaginosis was established in 45 patients of the Specialized hospital for active treatment in obstetrics and gynecology "Maichin Dom", Sofia and the Military medical academy-Sofia, gynecology ward. The diagnosis of bacterial vaginosis was established in accordance to Amsel's criteria and the results that were obtained from samples colored by Gram. For the treatment of BV were used local Metronidazole (Arilin rapid) and vaginal tablets containing lactic acid. The patients were divided into 3 groups. In the first group (18 women) vaginal suppositoria of 100 mg Metronidazole (Arilin rapid) were used for 2 days, in the second (20 women)--Arilin rapid x 1 glob./2 days and a following therapy with vaginal tablets containing lactic acid x 1 glob. for 7 days. The third group (7 women) was the control receiving placebo therapy--shampoo Dercome Femme containing lactic acid for external application. RESULTS: In the group receiving Arilin rapid we observed a normalization of vaginal flora in 71.43% (5/7), intermediate vaginal flora in 14.3% (1/7) and no change caused by therapy in 14.3% (1/7). In the second group, where lactic acid was included, cure was established in 94.1% (16/17) and in only one (5.9%) case was established intermediate vaginal flora. In the control group in 85.7% (6/7) of the cases was registered a lack of effect. CONCLUSION: Treatment with Arilin rapid showed good results in 71% of cases. Adding lactate to therapy improved the therapeutic effect and achieved cure in over 94%. The following application of vaginal lactate is an additional step helping the recolonisation of the vagina with lactobacilli and is a contribution to the struggle with recurrent forms.

[The microorganisms associated with bacterial vaginosis as a cause of tubal sterility].

Bacterial vaginosis is one of the most widespread disturbances of the normal state of the vagina of women of reproductive and menopausal age. This condition is highly disregarded due to its moderate and not always clear clinical manifestation. The disturbed proportion of microorganisms in vagina is shown in the strong decrease or lack of lactobacilli and a spread of associated with bacterial vaginosis microorganisms. G. vaginalis, Mobiluncis spp., Bacteroides spp. (without B. fragilis), Peptostreptococcus spp., Mycoplasma hominis, Ureaplasma urealyticum. The condition is a predisposition to a disturbance of natural defense mechanisms and to a spread of infectious agents towards the upper female genital tract. This is a predisposition for the development of tubal patency and motility disorders, ovarian function disturbances, endometrites. The treatment of this condition as an important step in female sterility prophylaxis is commented.

[The probiotic Lactobacillus acidophilus--an alternative treatment of bacterial vaginosis].

The probiotics or biotherapeutical agents are microorganisms that have an antagonistic activity regarding pathogens in vivo. Therapeutic application of these microorganisms is mostly in cases of infections of non-sterile mucous surfaces--vagina and large intestines. With the help of the probiotic L. acidophilus bacterial vaginosis can be treated successfully in a natural way. The lack of systemic side effects makes it a drug of choice in the treatment of pregnant women. More studies in this direction and development of new strategies for treatment of bacterial vaginosis in pregnant and non-pregnant women are needed.

Expression of the nuclear protein mitotin in differentiating in vitro HL 60 cells.

Mitotin is a 125 kDa/pI 6.5 nuclear protein specific for proliferating cells and markedly increased prior to and during mitosis. This study presents evidence for the expression of this protein during dimethylsulfoxide (DMSO) induced differentiation of human promyelocytic leukemia HL 60 cells. The expression had been followed at two levels: as antigen, using a specific antimitotin monoclonal antibody and as mRNA, using a specific cDNA probe. The results from the immunofluorescent study show a gradual disappearance of mitotin in differentiating HL 60 cells starting from the fourth day after DMSO induction. On the other hand, the changes in the expression of mitotin mRNA were much more dramatic. This mRNA is expressed at a high level during the first three days of differentiation but shows a striking decrease after the fourth day. This correlates with the rapid changes in the number of blast cells in the differentiating HL 60 cell population. Therefore, the expression of mitotin mRNA can serve as a marker for the changes accompanying the termination of cell proliferation in differentiating cells.

Synthesis and in vitro activity of new cephalosporin derivatives containing a benzoxazolone ring.

New cephalosporin derivatives containing a benzoxazolone ring in the side acylamino chain are synthesized using chlorides of non-substituted and halogen substituted (2-benzoxazolone-3-yl) acetic acids for the acylation of the 7-aminodesacetoxy-cephalosporanic acid and 7-amino-cephalosporanic acid amino group. Some of these new compounds exhibit a biological activity higher than that of cephalexin and cephazolin against clinical strains of Staphylococcus aureus, Staphylococcus epidermidis and Streptococcus group A and B.

[Clinical, morphological, cytochemical and ultrastructural studies of a female patient with basophilic leukemia].

The morphological, cytochemical and ultrastructural characteristics of basophilic cells in patients with chronic myeloleukosis were studied. The basophilic elements, dynamically followed up, from all phases of maturation, represented from 48 to 83 per cent of the cellular population in the peripheral blood and marrow. The cytochemical and ultrastructural investigations confirmed their belonging to the basophilic line. Certain changes were found, being an evidence of disorders in the maturation and metabolism of the cells. The high basophilic index, combined with a low neurophilic one, suggest the participation of the basophilic line in the leukosis process. On the base of the results obtained, the conception of the existence of basophilic leukosis is admitted.

A new translocation in chronic myelogenous leukemia.

A reciprocal translocation involving chromosomes Nos. 3 and 22 has been found in a patient with seemingly Ph-negative chronic myelogenous leukemia (CML). G-band analysis revealed, that deletion in No. 22 occurred at the same point, as in the typical cases of the disease. It was concluded, that breakage in No. 22 at a specific site with spatial disjunction of the resulting segments might be the crucial cytogenetic event in the genesis of CML, the Philadelphia chromosome not being an obligatory result of the rearrangement.
