[Multisite validation study of questionnaire assessing out-patient satisfaction with care questionnaire in ambulatory chemotherapy or radiotherapy treatment].

Patient satisfaction is now recognised as an important quality of care outcome which is particularly relevant in oncology. Adapted from the EORTC In-Patsat32, the Out-Patsat35 is a 35-item satisfaction with care questionnaire measuring cancer outpatients' perception of hospital doctors and nurses, as well as aspects of care organisation and services. This study assessed the psychometric properties of this scale. Patients undergoing ambulatory chemotherapy (CT) or radiotherapy (RT) in 7 cancer centres in France were invited to complete at home the Out-Patsat35 as well as EORTC QLQ-C30 for psychometric testing. Of 416 eligible patients recruited, 96% returned the questionnaire. Most patients (71% in CT; 69% in RT) completed this scale within 15 minutes and the mean rate of item omission was only 4.4%. Confirmatory analyses revealed good convergent validity and excellent internal consistency, although some subscales within the Out-Patsat35 were relatively highly correlated. Items and subscales of the Out-Patsat35 and of the QLQ-C30 were not significantly correlated, underlying that the two questionnaires are assessing quite distinct concepts. The subscales of the Out-Patsat35 were not related to age, gender and education, suggesting a cultural evolution in French cancer patients towards a greater homogeneity in their opinion toward care. This study supports the acceptability to patients, and the psychometric properties of the EORTC Out-Patsat35 questionnaire.

[Buruli disease and plastic surgery].

The Buruli ulcer is a skin infection with Mycobacterium ulcerans which progresses silently. This infection affects mostly women and children who live near stagnant waters. Buruli ulcer is disease that has terrible consequences if not promptly diagnosed and treated. It destroys progressively skin tissues and consequently leaves very important scars. There is no efficient medical treatment. This presentation proposes to take care efficiently of the Buruli ulcer by simple plastic surgery techniques. We relate our experience of a mission in Benin, in the context of the national programme of struggle against the Buruli ulcer.

Carotid chemodectomas. Experience with nine cases with reference to preoperative embolization and malignancy.

The medical records of nine patients (five female and four male, mean age 58 +/- 5 years) presenting with a carotid chemodectoma between 1983 and 1995 were reviewed. In two cases (22%) the diagnostic was not suspected at the time of initial presentation. The most common complaint was a swelling in the anterolateral region of the neck. One patient (11%) presented with a preoperative peripheral nerves deficits (vagus and hypoglossal palsies and Horner's syndrome). Two tumours were embolized preoperatively with polyvinyl alcohol particles. Complete surgical excision was possible in each patient and the plane of resection was adventitial. In three cases, early ligation of the external carotid artery facilitated the resection. In two patients, the vagus nerve was sacrificed because of tumour involvement. No operative mortality was observed and no vascular complication occurred. In addition to the patient with preoperative neurologic symptoms, three patients developed peripheral nerve deficits (vagus and hypoglossal nerves) postoperatively. Two of these deficits were transient. These peripheral neurologic complications were observed with the largest tumour sizes. Two cases were malignant (lymph nodes and bony metastases). These two patients received postoperative radiotherapy. The mean follow-up period 63 +/- 19 months. No patient developed local recurrence during the follow-up. Two patients died during the follow-up, one for condition unrelated to their disease and the second from metastatic dissemination. In conclusion, carotid chemodectomas may be safely resected. The best way to minimize the rate of complications is to operate them at an early stage of evolution.

[The nose: representation and symbolism].

The nose is a primordial element in the facial recognition among the human species. This privileged place, confirmed by experimental psychology, finds a professional application in the creation of the photofit. The physiognomonist drift, that flourished during the last century, lacked a scientific basis and gradually gave way. On the other hand, it is interesting to observe the artist's representations of the nose: except for the realistis school the nose is upsetting. It is therefore either stereotyped or deleted. As for the caricaturist, he illustrates the nose shamelessly. This representation is certainly related to the symbolic force of the nasal appendage, not only a vector of diverse and variable odours but also a passage for the spirit and to the heart. The nose is therefore a significative element of relation, of contact and of expression.

[Assessment of extension and staging of oral cancers].

Cancers of the oral cavity are most frequent during the 6th to 8th decades and tobacco and alcohol are major risk factors. Diagnosis must be made by bimanual palpation and biopsy. Local invasion is best evaluated by CT-scanner and Magnetic Resonance Imaging for the primary tumor and by ultrasonography for neck nodes. For therapeutic and prognostic goals, staging of the tumor must be estimated before first treatment according to the AJCC and the UICC classification.

[A method for collecting data on kinship in demographic surveys: a compromise with the anthropological method].

"We present herein a solution to operationalize the concept of kinship using the experience of the IFAN-ORSTOM survey carried out in Dakar [Senegal]. Our results confirm the increased dependence of the younger generation on older generations [during life cycle changes or demographic crises]. The urban way of life does not seem to disorganize family structures in Dhaka. At present, it is through family solidarity that the shock of the crisis is absorbed." (SUMMARY IN ENG AND SPA)

More single women in African cities: Pikine, Abidjan and Brazzaville.

The authors examine increases in marriage age among women in Africa. They show that "this evolution towards later marriage is linked to schooling, but changes are also emerging among women who have not attended school....[The authors] show that the growing proportions of single women are not merely the result of marriage postponement, but a sign of more in-depth changes which are transforming African urban societies."
