[Abortions on demand in adolescents].

The pregnancy is an integral part in the life of teenagers. When the conception was appeared the adolescents have to choose between interruption of delivery. Psychological status, contraceptive use and sexual behavior, motivation for abortion, operative methods and complications were observed in 463 teenagers, aged 13-17 years, who had an abortion voluntary in the last 6 years (1988-1993). We were found an abortion rate to 2.33% in our Dept. Obstet. Gynaecol. and a trend to increase the number of delivery in teenagers. The medical complications of abortions were: perforatio uteri complicata (0.65%) and endometritis acuta (9.29%), in cases with pregnancies over 16-20 g.a. This investigation was characterized from the teenagers as an stress and will be able to be complicate with a psychological disorders.

[A patient with psoriatic arthritis and secondary amyloidosis].

A case of a man suffering from psoriasis vulgaris is presented. The onset of the disease was at the age of 5. From the age of 14 all joints were gradually affected by the type of severe, symmetrical psoriatic arthritis which led to aseptic necrosis of the hip joints. In the course of the disease, from 1988 on, proteinuria and nephrotic syndrome were found. The renal biopsy proved amyloidosis which is considered secondary, developed on the basis of psoriatic arthritis. The severe psoriatic arthritis with especially emphasized clinical activity is in this case the pathologic antigenic stimulus for the amyloid formation. The clinical course and the histologic findings from the renal biopsy point to serious structural disorders of the renal parenchyma which determine the progressing tendency toward chronic renal failure.

[Arterial hypertension in gouty nephropathy].

The incidence of arterial hypertension in patients with gouty nephropathy was studied. A reverse correlation dependence was established between the clearance of uric acid and the values of systolic and diastolic pressure, directing the clinicians to controlling the hyperuricemia, in parallel with the prescription of hypotensive agents. Attention is drawn to the higher incidence of hyperlipidemia in patients with gouty nephropathy and hence--the possibility of more frequent coronary incidents. Renal insufficiency advanced earlier in the patients with gouty nephropathy and arterial hypertension.

[Patient with familial Mediterranean fever and amyloidosis].

A patient is described, with genetic form of amyloidosis with family Mediterranean fever. The secondary amyloidosis, that developed as a complication, has been preceded by attacks of acute articular and abdominal pains of several years. Elevated level of ethicholanolon in urine was established as well as extremely high values of serum fibrinogen, admitted to be pathognomonic signs of family Mediterranean fever. The eposition of amyloidosis is confirmed via rectal and renal punch biopsy. In spite of the diffuse character of the deposited amyloid in the renal tissue, there were still no signs of glomerular sclerosis and clinicalf--normal depuration renal function was observed, with normal creatinine clearance and normal nitrogenous bodies in serum. A favourable effect of colchicine therapy was observed in the patients both as regards the acute attacks of the disease and as regards the renal involvement.

[Para-amyloidosis in 2 patients with malignant diseases].

Two cases are described with paraneoplastic amyloidosis, advancing in patients with a basic etiological moment--myeloma and hypernephroma. The clinical picture, in both patients was dominated by the nephrotic syndrome and rapidly progressing renal insufficiency, leading to the fatal end within a very short term. That term in the patient with hypernephroma was only several months after making the diagnosis, and in the patient with myeloma--up to two years. Renal punch biopsy was performed to both patients during their life time.

[Case of Fabry's disease].

A patient is described, admitted to the clinic on the occasion of proteinuria, edemas and arterial hypertension. The clinical and paraclinical data formed the picture of a renal involvement of mainly glomerular type with proteinuria to 5 gr%0, nonselective type, edemas, hypertension and data of chronic renal insufficiency I degree (serum creatinine to 3.8%, blood urea to 112 mg%). What impresses are the extrarenal manifestations as paresthesia of the lower limbs, with a sense of numbness to acute burning pain, a rich psychonervous symptomatics. The skin efflorescence, followed up and observed at the clinic of dermatology and the puncture biopsy material from the kidney revealed specific signs of Fabry syndrome.
