[Immunopathologic mechanism in some ocular diseases].

OBJECTIVES: The goal of the present study is to elucidate the pathogenic mechanisms that determine the ocular involvement in some systemic diseases. MATERIAL AND METHODS: We have studied 45 patients with ocular diseases and concomitant systemic diseases (systemic lupus erythematosus-SLE, ankylosing spondylitis, Reiter syndrome, juvenile rheumatoid arthritis, Sjögren syndrome). The patients studied have been compared with a control group represented by 20 healthy individuals. In each case we have determined the serum levels of the following humoral markers: the C3 complement fraction, circulating immune complexes, autoantibodies (antinuclear autoantibodies, anti-double stranded DNA autoantibodies, anti-smooth muscle autoantibodies, anti-neutrophilic cytoplasm autoantibodies, anti-cardiolipin autoantibodies, anti-SSA, anti-SSB, rheumatoid factor). RESULTS: The autoantibodies detected in certain cases of uveitis suggest the existence of an autoimmune etiology; the antibody titers have been high in some severe cases of rheumatic uveitis, especially in patients with ankylosing spondylitis. We have found that there is a correlation between the presence and the titer of certain autoantibodies, and the onset and evolution of the disease. Specifically, the antinuclear autoantibodies may be considered a predictive factor for the development of chorio-retinopathy of SLE. Uveitis mediated by immune complexes has been seen in rheumatoid arthritis, ankylosing spondylitis, and SLE, these being the same systemic diseases where there is an important pathogenic role of immune complexes. CONCLUSIONS: The abnormal immunologic function, that was observed in the ocular inflammatory diseases studied, may be synthesized by the association between two immunopathologic phenomena; both of them are finalized by the non-self transformation of the antigens linked to various eye structures. These are represented by autoimmunity (of humoral, cellular, or mixed type), and the immediate type III hypersensitivity reaction. This study provides important data regarding the involvement of autoimmune diseases in the etiology of ocular diseases.

[Humoral and cellular immunity investigation in idiopathic uveitis].

OBJECTIVES: There is an amount of controversy regarding the patho-physiology of uveitis. Due to the fact that the eye (and especially the highly vascular tissues such as the uvea and the conjunctiva) is considered a special "target" for the immunopathologic reactions, the goal of our study is to contribute to the understanding of the causal role of the immune reactions in the development of idiopathic uveitis. MATERIAL AND METHODS: We have studied 48 patients with idiopathic uveitis and 20 control subjects (with no ocular symptomatology) for the following humoral and cellular immune markers: IgM and IgG, the C3 complement fraction, circulating immune complexes, autoantibodies (antinuclear autoantibodies, anti-double stranded DNA autoantibodies, anti-smooth muscle autoantibodies, anti-neutrophilic cytoplasm autoantibodies, anti-cardiolipin autoantibodies, anti-parietal cell autoantibodies), and lymphocyte subtypes. RESULTS: Changes in the serum levels of the immune humoral and cellular markers have been noted in 39 (81.25%) of the patients studied. C3 was decreased in 12 (30%) of all cases; the immune complex level was increased over 50 U/mL in 20 patients (51.28%). The presence of the above-mentioned autoantibodies was seen in 19 patients (48.71%), with different incidence. By flow cytometry the percentage of CD4+ (T helper lymphocytes) has been found to be decreased, and the percentage of CD8+ was found to be increased in 8 cases (20.51%). The opposite, the increase in the percentage of CD4+ lymphocytes, and the decrease in the percentage of CD8+ cells was seen in 25 (64.10%) of the studied patients. Taking into consideration the clinical symptoms and the level of the immune humoral and cellular parameters, the cases included in the study have been included in two groups: anterior uveitis (acute and chronic), and posterior uveitis. CONCLUSIONS: By comparing the results, we conclude that the autoimmunity and the immediate hypersensitivity mediated by immune complexes may be involved in the appearance and the development of the intraocular inflammation in certain cases of idiopathic uveitis.

[Epidemiological aspects in breast cancer].

Breast cancer represents one of the main public health problem, with a special epidemiological interest, both at the national and international level. A retrospective epidemiological study during the last 11 years (1990-2000) concerning new cases and incidence of the breast cancer at the national level, in Dolj county and in the 1st Surgical Clinic Craiova was made, by using of a comparative analyses which parameters were processed by modern statistic methods. Time evolution equation of the new cases all over the county showed us that more than 208.52 new cases than in the previous year appeared every year. If such an increased rhythm were held, about 6800 new cases would be every year up to 2100. As concerning breast cancer frequency, both at national and local level, a significant increase can be noticed during the last period. At national level the incidence increased from 3.3@100000 in female in 1950 to 29.2@100000 in 1990 up to 46.2@100000 in 2000. We noticed an increase of the incidence with 58.2% at the national level and 86% in Dolj county, in 2000 compared to 1990. By using a comparative analyses on age grouped we could establish a significant frequency of cancer in the patients of 40-49, 50-59 and 60-69 years old, with two frequency peaks for 45-49 and 60-64 subgroups, both at national and county level and also in 1st Surgical Clinic.

[Diagnosis characteristics features in breast cancer].

Representing about 20% of all woman's cancers in all the world, breast cancer is a multifactorial disease with unknown mechanism facts which may explain the significant growth of incidence and mortality in the last few years. In the absence of screening program in majority cases the diagnosis is discovered too late. In this present study we have made a retrospective study on 487 patients during 1996-2000 at the 1st Surgical Clinic from Craiova. The purpose of this study was the defining of the population with an increased risk which has to be monitorized. In order to accomplish it, we have carefully supervised some of clinical parameters: sex, age, social environment, physiological and pathological antecedents, heredocolateral antecedents, and the constitutional type. The study method was the comparative analysis, and the evaluation according to the modern statistical methods. The risk factors, which can be surely used in defining the groups of population with an increased risk, which should be supervised for an active discover of breast cancer, as the resulted from our study, are: the age decades V, VI, and VII with maximum incidence in subgroups 45-49 and 60-64 years old; early monarch (67.15%); delayed menopause, over 50 years old (84.4%); long exposure to sexual hormones during the reproductive period (the period of menstrual cyclicity over 32 years old); the pathology of the benign tumor, the biggest risk belonging to the proliferative tumours (intraductal papiloma); hyperestrogenism. The only improving method of the therapeutical results in breast cancer is an early discover of it, which can be accomplished only by the implementation of a national program of active discover, in which the essential part belongs to the primary care helped by a widely-broadcast medical education.

Assessment of Gleason system use on different types of prostatic tissue samples.

One of the well-known and accepted methods of prostatic adenocarcinoma grading is Gleason system. The authors made a retrospective analysis of 221 prostatic adenocarcinomas divided into three groups (transvesical prostatectomies, transurethral resections and needle biopsies) following the type of surgical procedure used for drawing the tissue. Gleason scores and comparison between odd and even Gleason scores were assessed in the entire group and in each subgroup. High scores prevailed, meaning a tendency towards a low grade of differentiation. Even scores also prevailed meaning, on one hand, that, often, the examined specimen reveals only one pattern and, on the other hand, that surgical procedures as transurethral resections and unique needle biopsies cannot offer a sufficient material for examination, the multicentricity of prostatic carcinoma being well known.

Comparative study between prostatic carcinoma G1-G3 and Gleason grading systems.

The need of an accurate and prognostically valuable method for grading the prostatic carcinoma resulted in, in the last decades, a large number of systems, none of them succeeding in being unanimously accepted by the pathologists family. The authors selected and reassessed using Gleason system a group of 221 cases with prostatic adenocarcinoma previously diagnosed using a combined G1-G3 system. The results were converted from Gleason system to G1-G3 system and then compared with the initial diagnostics. The group was divided, following the type of surgical procedure used for drawing the tissue, into three groups: transvesical prostatectomies (TVP), transurethral resections (TUR) and needle biopsies (B). The concordance between the two systems was good, even the number of TUR and B specimens was significant (85% of the studied group) showing that any of them can be successfully used in current practice, depending only on pathologist's experience.
