The authors developed a method for studying the whole arterial system, except for the coronary arteries, by means of ultrasonic techniques. It was based on the conception of the generalized nature of atherosclerosis presuming the existence of some specific types of the process which may be classified into 5 categories (according to DeBakey) depending on the main focus of distribution of the disease. The method was named pan-arterial ultrasonography. It includes a series of ultrasonic examinations performed by means of Doppler's ultrasonic scanning apparatus operating in B-regimen with various transducers for determining the arterial blood flow and imaging the various arterial segments. The authors examined successfully 716 patients by the method.
The authors analyse the existing indications for forming a femoropopliteal shunt and its late-term results. Critical ischemia with pains at rest or trophic changes, e.g. ulcers or gangrene, are absolute indications. The nature of the graft as such is the most important factor influencing the late-term results. The indications, the arteriographic findings, the region of the anastomosis, and operations performed on the femoropopliteal vessels earlier have a marked effect on the late-term results, particularly if synthetic prostheses are applied. The results obtained show that further investigations are necessary with careful recording of the variants for better understanding of the role of femoropopliteal shunting as palliative treatment of atherosclerotic occluding lesions of the inferior inguinal region.
A postoperative arteriovenous fistula of the upper thyroidal pedicle was detected 14 years after thyroidectomy. Treatment in this rare case was by elective surgical excision of the fistula.
The terms "Raynaud's disease" and "Raynaud's phenomenon" are often used interchangeably, as if they were a single problem. Because prognosis and therapy are different with each of these entities, 86 patients with episodic digit ischemia were evaluated over the past five years. All patients were evaluated similarly with plethysmography, angiography, and other studies for underlying causes of the digital ischemia. From the study, it was concluded that the majority of the patients had an underlying cause for the problem even though they satisfied the commonly accepted criteria for the diagnosis of Raynaud's disease. It is clear that the diagnosis of Raynaud's disease must be one of exclusion, because it is more uncommon than is generally appreciated by the medical community.
Venous thrombosis in childhood is an extremely rare condition. The etiologic factors in childhood are different than in the adult. Generalized infection seems to play the main etiologic role, and for this reason antibiotic treatment is usually necessary. Anticoagulant treatment is mandatory; however its application presents many difficulties. The administration of calcium-heparin subcutaneously for a long time is feasible, well-controlled, without side effects and highly effective. The authors therefore strongly recommended this anticoagulant treatment in childhood.
