A simple technique for hemoglobin estimation to screen for anaemia.

Drabkins method has been modified enabling detection of anaemia in a large population. 132 samples of EDTA blood were subjected to hemoglobin estimation by (1) Direct Drabkin (DD) (2), Filter Drabkin (FD) and (3) Special Filter Drabkin (SFD). Hemoglobin estimations by DD and FD compared well on statistical analysis. SFD with a punch diameter of 10.6 to 10.7 mm compared well with DD and is ideal for screening anaemia in field studies.

Anemia

Healing of non-vascularised diaphyseal bone transplants. An experimental study.

Four different experiments were performed to study the healing of a large, non-vascularised, diaphyseal, bone segment in adult cats. In the first experiment, a 4 cm segment of tibia with its periosteum was excised and replaced in its bed. The other experiments were similar, except that in the second, the periosteum of the segment was removed, in the third its medullary canal was blocked with a Silastic rod, and in the last group the segment was isolated from its muscle bed by a Silastic sheet. The reparative processes were quantified by estimating the resorption index, the cortical new bone formation index, the callus encasement index, and the osteocyte count. Bone resorption and apposition occurred in the segment even when the periosteum was absent or the medullary canal was blocked, with osseous union at both ends by eight to 12 weeks, provided the segment was not isolated from its muscle bed. Thus, the muscle bed played a significant role in these reparative processes.

The lateral condylar prominence. A complication of supracondylar osteotomy for cubitus varus.

We reviewed 27 patients who had supracondylar closing wedge osteotomy for cubitus varus. There were 10 excellent and 12 good results. However, of these 22 patients, 14 had a significant bony prominence over the lateral condylar region caused by lateral displacement of the elbow when closing the osteotomy. This prominence was less obvious in patients who had their osteotomy at a young age, but worse after operations near or after skeletal maturity. This difference appeared to be due to remodelling.

Adolescent

The estimation of dermal collagen in osteoporotic patients by a histomorphometric method.

Sections of skin biopsies from the dorsum of the hand of patients with symptomatic osteoporosis were compared with those of carefully selected age-sex matched controls. The amount of dermal collagen in the osteoporotic individuals estimated by a histomorphometric method was found to be significantly less and severe degrees of collagen loss could be readily identified by microscopic examination. Distinct morphological abnormalities of the collagen fibres could also be observed in most patients with osteoporosis. This loss of dermal collagen is suggested to be the main factor responsible for the decrease in skin fold thickness and the frequently striking transparency of skin observed in association with osteoporosis.

Aged

Pseudotumours due to oil palm thorn injury.

Three cases are reported in which pseudotumours developed in the hand following injury by oil palm thorns. In one patient there was a periosteal reaction of the proximal phalanx adjoining the site of injury. The lesions were excised, and the embedded oil palm thorn was identified in all three. The clinical and pathological features of the lesions and problems in their diagnosis are described. In every case the local tissue reaction was out of proportion to the size of the foreign body, raising the possibility that a toxic substance might have been released by the thorn.

Adult

Humeral torsional deformity after supracondylar osteotomy for cubitus varus: its influence on the postosteotomy carrying angle.

Humeral torsion was measured in 23 patients by computed tomography (CT) scan after supracondylar osteotomy for cubitus varus. The opposite unoperated humerus was used as the control. The mean torsion in the unoperated humerus was 70 degrees. Patients who did not have "derotation" of the distal fragment during osteotomy had approximately 30 degrees more internal torsion on the operated side than on the opposite side. The carrying angle after osteotomy was not influenced by lack of correction of torsional deformity in the humerus, and correction of torsion during osteotomy was not essential for correction of the carrying angle.

Adolescent

Comparative study of hemoglobin estimated by Drabkin's and Sahli's methods.

Hemoglobin was estimated by Sahli's and Drabkin's method in samples collected by finger prick and venepuncture. Statistical analysis revealed a significant difference in the hemoglobin values obtained by the two methods (P less than .01 & P less than .05 respectively). Sahli's method carried out by two different groups of workers in samples of blood collected by venepuncture showed statistically significant difference (P less than .001). This could be due to the inbuilt errors of Sahli's method including a subjective bias due to visual comparison. When same samples were subjected to Drabkin's method by the same group of workers no significant difference in the results were obtained (P less than .05). This is due to reliability of the Drabkins method. The aim of this comparative study of hemoglobin estimation by Sahli's and Drabkin's methods is to emphasize the sensitivity and reliability of Drabkin's over Sahli's and utility of Drabkin's method in undergraduate teaching schedule.
