Ultrastructure of periprosthetic Dacron knee ligament tissue. Two cases of ruptured anterior cruciate ligament reconstruction.

Light- and electron-microscopic investigations were performed on two failed Dacron ligaments that had been removed from 2 patients shortly after failure of the implant 2-3 years after reconstruction of the anterior cruciate ligament. Two different cell populations and matrices were correlated with closeness to the Dacron threads. Fibroblasts surrounded by connective tissue with collagen fibrils were located far from the Dacron threads. Roundish cells, appearing to be myofibroblasts surrounded by a more lax connective tissue and elastic fibers, were found close to the Dacron threads. The presence of myofibroblasts and the matrix differentiation could be attributed to the different mechanical forces acting on the Dacron and on the connective tissue because of their different coefficients of elasticity. The sparse occurrence of inflammatory cells in the synovial membrane and in the connective tissue surrounding the Dacron supports the biologic inertness of this artificial material. However, the repair tissue was not structured to resist tension stresses.

Ultrastructural aspects of human nonunion.

A histological study on the tissue of nonunion of tibias of two young patients was performed to evaluate the ability of cells to start the mineralization of the matrix. The observations can be summarized as follows: 1) Tissue vessels often appear occluded by thrombotic material; 2) Fibroblasts and chondrocytes found in the nonunion tissue seemed normal, with a good secretion apparatus; 3) The cell membranes were able to produce matrix vesicles; 4) Matrix vesicles and cell membrane looked positive to ALPase reaction, 5) Hydroxyapatite crystals could be observed in the cell matrix or inside matrix vesicles. It may be concluded that cells populating nonunion tissue are well equipped to induct the mineralization of the matrix, but the absence of a blood supply, enough to bring them a normal calcium amount, is the real reason for the nonunion.

[Variations in the rate of erythrocyte sedimentation in the postoperative orthopedic and traumatologic course and their clinical significance].

The authors report their results on E.S.R. variations within the first month post-surgery in 38 traumatologic and orthopaedic patients, to evaluate the time of normalization and its attendibility in case of infection. Normalization was found within the first month postoperatively, without significant differences between orthopaedic and traumatologic patients. They think that the E.S.R. becomes significant on the control of postoperative subacute infection after the third postoperative day and propose E.S.R. evaluation after this period.

[Posterior osteosynthesis in C1-C2 instability].

This is a retrospective study of 8 cases of instability of the C1-C2 segment caused by R.A. or non-union and treated by posterior stabilization with wire loops and bone grafts. The authors reported good results in patients with mild recent neurological involvement and/or radiographic instability of the segment (posterior subluxation of the dens greater than 1/3 of the A-P diameter of C1 or tilting of the dens greater than 30 degrees). The death of a patient with non-union of the dens and severe neurological involvement due to respiratory palsy leads the authors to regard severe long-term neurological involvement (quadriparesis) a contraindication for this type of procedure.

Long-term follow-up of 53 cases of chronic lesion of the anterior cruciate ligament treated with an artificial Dacron Stryker ligament.

Fifty-three cases of chronic lesion of the anterior cruciate ligament were followed up clinically and radiographically, and treated consecutively with an artificial Dacron Stryker ligament. An average of 29 months (range 12-53) after surgery, the authors report a high incidence of residual laxity (70.5%) and a 13% incidence of complications. There were no cases of evident synovitis, even if the authors do report a high percentage (85%) of periligamentous femoral osteolysis. Based on the results obtained, the authors believe that the Dacron Stryker ligamentous prosthesis is only indicated in cases where a biological ligament fails in patients engaged in competitive sports who request immediate activity for a limited period of time.

Herniated lumbar disc in the child: a description of two cases.

The authors report 2 cases of herniated disc in subjects under 10 years of age. Spinal stiffness more than nerve root symptomatology is the dominant element in the clinical picture: the diagnosis was based on CT scan and on radiculography. In both cases surgical treatment resulted in resolution of the symptoms which was maintained in long-term follow-ups.

Rokitansky-Küster-Hauser syndrome with ectrodactyly.

This paper describes an 18-year-old patient with Rokitansky-Küster-Hauser (R-K-H) syndrome. In this case, apart from the usual alterations associated with the R-K-H syndrome, such as aplasia of the Müllerian ducts, renal agenesis, ectopic kidney and anomalies of vertebral column, ribs and hips, rare skeletal, unilateral abnormalities of the left hand and foot were present, such as ectrodactyly. This malformation, seen in prepubertal age, had led to an incorrect diagnosis of acrorenal syndrome.

Osteoid osteoma of the body of the first sacral vertebra. Case report.

A rare case of osteoid osteoma localised in the anterior aspect of the first sacral vertebra is described. Scintigraphy and CT scan are essential for a correct diagnosis in this condition. Because of the site of the lesion, surgery was performed by an anterior transperitoneal approach.

The effects of methylprednisolone acetate in the treatment of bone cysts. Results of three years follow-up.

The treatment of bone cysts by topical injection of methylprednisolone acetate was initiated at the end of 1973, and the late results are reported in this paper. In seventy-two cases followed up for one to three years favourable results have been obtained in about 90 per cent. The technique of local injection and the surgical equipment employed, in the case of focal recurrences, are considered. With this method, surgical treatment of bone cysts in youth is seldom necessary.
