[Visual function after pars plana lensectomy and intraocular lens implantation in dislocation of the lens into the vitreous body].

The authors evaluate the functional result of the surgery solving luxation of lenses into the vitreous body cavity with helping of pars plana lensectomia with implantation of intraocular lenses, they have been performed in The Department of Ophthalmology of The Teaching Hospital of The Charles University in Hradec Králové between February 1995 to February 1999. Our group consisted of 11 patients (11 eyes), 8 males and 3 females in the age from 28 to 81 years, with average age of 60 years, the patients were divided into 3 units. 1st unit is represented by 5 patients with traumatic luxation of lens into vitreous body cavity after its contusion with developing of the secondary glaucoma, in the 2nd unit there are 5 patients with this complication as a consequence of operation of cataract, the 3rd unit includes only 1 patient with the Marfan syndrome. Study proved the fact that due to using of new surgical methods it is possible to keep a satisfactory visual functions in patients with a serious eye complications.

[Results of treatment of retinal detachment with macular holes].

We analysed the results of PPV combined with the silicone oil or expansive gas endotamponade of the retina by 12 patients. The etiology of the retinal detachment with a macular hole were: myopia in 8 cases, emetropia in 4 cases. Reattachment and functional results with visual acuity from 0.5/50 to 6/24 we found by all emetropic eyes. By myopic patients we found 87.5% anatomical success during 40 months, functional result of these patients advises on the degenerative myopic changes.

[Retinal detachment in pseudophakia].

The author evaluates the prevalence and results of surgical treatment of retinal detachment (RD) after implantation of intraocular lenses in a group of 10,185 eyes operated at the First and Second Ophthalmological Clinic from 1988 to the end of 1993. Retinal detachment was observed in 0.4% of artephakic eyes. A striking risk factor for the development of RD was in particular loss of the vitreous body during cataract operation, a previous injury, myopia and YAG capsulotomy. The presence of an intraocular lens did not influence in a substantial way the selection of the surgical procedure. In cases with a complicated finding in the anterior segment and the retina the authors selected more frequently pars plana vitrectomy (PPV) with internal tamponade with an expansive gas or silicone oil (in 44.3% eyes). The retina became attached in 90.4% of the eyes, while in patients operated by cryosurgery anatomical success was achieved in 82.1% of the eyes and in the group operated by PPV with internal tamponade in 100% of the eyes. Functional improvement, as compared with the condition before surgery of RD, was achieved in 48.9% of successfully operated eyes. In the PPV group the authors observed, however, substantially poorer functional results as compared with cryosurgery. In complicated artephakic RD PPV with internal tamponade contributed to a striking improvement of anatomical results but the poorer functional results after PPV in complicated RD provide evidence of a poorer prognosis of artephakic RD, as compared with aphakic ones.

[Bilateral retinal detachment].

In 1977-1990 a total of 2,554 patients with rhegmatogenic retinal detachment were operated by the cryosurgical method, using silicone episcleral implants. The total number of patients comprosed 36 (1.4%) where the authors detected during the preoperative examination retinal detachment, usually in an initial stage, on the other eye. In 30 patients the authors operated bilateral detachment of the retina simultaneously and in the remaining six during the same hospitalization. The poorer eye was characterized by 1. more extensive retinal detachment, 2. a higher grade of proliferative vitreoretiopathy (PVR). As to the preoperative findings, the authors consider the following points important: 1. Almost 70% of the patients did not report subjective complaints of the better eye. 2. With regard o the pathogenesis the patients were divided into three groups: lattice degenerations in 61.1%, idiopathic oral dialysis 11.1%, other factors 27.8% of the patients. 3. Symmetry of the retinal defect up to 90 in two-thirds of the patients. 4. Type of operation. On the poorer eye the authors performed segmental plombage in 12 eyes (33.3%), cerclage in 5 eyes (139%) and reinforced cerclage in 19 eyes (52.8%). For the better eye the authors indicated segmental plombage in 12 eyes (77.8%), cerclage in 4 eyes (11.1%) and reinforced cerclage in 4 eyes (11.1%). Subretinal fluid was punctured on the poorer eye in 19 eyes (52.7%) and on the better eye in 7 eyes (19.4%). After an average observation period of 40 months the retina became anatomically attached in 86.0% on the poorer eye, and in 100% on the better eye. Bilateral retinal detachment in our group can be considered a model situation of retinal detachment which developed roughly at the same time and under equal endogenous conditions of the given organism. It may be thus assumed that a different extent of detachment and grade or proliferative vitreoretinopathy before operation was determined above all by the period of retinal detachment.

[Ocular changes in Gardner syndrome].

In hereditary adenomatosis of the large bowel (familial polyposis) extraintestinal manifestations of the disease are common: hyperostosis, dental anomalies, soft tissue tumours, desmoid tumours etc. Patients with marked extracolic signs are described as patients with Gardner's syndrome. Recently a further sign is described--foci of congenital hypertrophy of the pigmented retinal epithelium. The authors examined 22 patients with confirmed hereditary adenomatosis (Gardner's syndrome). The typical finding of pigmented foci on the fundus was recorded in 18 subjects (82%) incl. 9 subjects (50%) where the finding was bilateral. The authors examined also 25 children of these patients. In those a positive finding was recorded in 11 (44%). Ophthalmological examination can contribute in a significant way to detection of an asymptomatic subject with hereditary adenomatosis, in particular when seeking risk patients in an affected family. Evaluation of ophthalmological changes can also contribute to the solution of some special genetic problems of this disease.

[Pars plana vitrectomy with implantation of silicone oil in surgery forvery large retinal tears].

The authors evaluate the results of pars plana vitrectomy (PPV) with intravitreal implantation of silicon oil (ISO) in 19 eyes of 17 patients with gigantic tears of the retina. During the mean observation period of 22 months the operation was successful in 15 of 19 eyes (78%). The retina adhered on the entire surface in 12 eyes (65%), in three eyes (15%) there remained a defined flat elevation of the retina in the periphery of the lower half of the fundus which did not go as far as the macula. In successfully operated eyes the visual acuity was 0.4 in two eyes, visual acuity of 0.3-0.1 in 7 eyes and visual acuity less than 0.1, making spatial orientation possible, in six eyes. The cause of reduced visual acuity of successfully operated eyes were macular abnormalities which were associated with the fundamental disease, degenerative myopia, injury, impaired nurture of the macula during prolonged detachment, a membrane-forming process. As to late complications of ISO, the most frequent were glaucoma in 42% and complicated cataract u 78% phakic eyes. Extraction of a cataract with iridectomy VI and supplementation of silicon oil into the intravitreal space did not cause in any of the patients a relapse of detachment of the retina. The final evaluation of PPV with ISO in the surgery of gigantic lesions of the retina will be possible only after the long-term follow up of larger groups of patients.

[Is the prognosis of pars plana vitrectomy in diabetic retinopathy dependent on the general status of the diabetic?].

Results of pars plana vitrectomy (PPV) were compared in two groups of patients. The first group was formed from 105 eyes of 87 diabetics without clinical signs of the diabetic nephropathy or nephropathy without decrease of renal functions. The other group was formed from 64 eyes of 50 diabetics suffering from diabetic nephropathy in the stage of the renal insufficiency. In the first group PPV was successful during the mean observatory period of 26 months, in 59 from 105 eyes (56%) and the mean visual acuity was 0.12. In the second group PPV was successful, during the mean observatory period of 18 months, in 34 from 64 eyes (53%) and the mean visual acuity was 0.09. Number of the successfully treated PPV was steadily decreasing according to the prolonged period of the further observations, in both mentioned groups: from 85 (81%) after 3 months on 62 (41%) after the interval of 3 years. The mean visual acuity in both followed groups was, with respect to the length of observation, moderately increased. Diabetic nephropathy in the stage of the renal insufficiency was without negative effect on the prognosis of PPV in complicated diabetic retinopathy. The more advanced organ complications of diabetes were not evaluated as contraindications of PPV and the operation may significantly improve the quality of the life of an otherwise heavily ill diabetic patient.
