[Appendicular invagination and post-appendectomy invagination].

Intussusception of the appendix is an uncommon event (200 cases previously reported) and intussusception after appendicectomy is rare (20 cases previously published). Two recent cases are reported, the first with primary appendiceal intussusception and the second ileocolic intussusception after appendicectomy. In the second case the final pathology report revealed an intussuscepted caecal duplication responsible for the ileocolic intussusception. This event has not been previously reported. Symptomatology, investigation, classification, differential diagnosis and surgical tactics are discussed.

[Surgical treatment of acute necrotizing pancreatitis].

15 cases of acute necrotizing pancreatitis are reported. The necessity of early diagnosis and operative treatment are emphasized. Axial computerized tomography is a major progress in the diagnosis. As for the operative treatment we advocate a wide drainage of the pancreatic area, a gastrostomy for suction, a cholecystostomy and a jejunostomy for nutrition. Postoperative mortality is, however, around 27%.

[Axial tomodensitometry in acute pancreatitis].

Early surgery for the treatment of acute necrotizing pancreatitis is important. With the CAT-scan we evaluated the extent of the lesions of the pancreatitis in 26 patients with acute necrotizing pancreatitis. Due to severe clinical symptoms and the CAT-results 12 of these patients had to be operated. We found a good correlation between intraoperative extent of disease and the CAT-scan.

[Lithiasis of the main bile duct: change in surgical approach].

Should the papilla Vateri be explored during surgery for gallstones of the main biliary tract? A group of 672 choledochotomies were analyzed retrospectively; out of precautions we approached the papilla Vateri in 75% of these cases. This group was compared to a prospective study of 200 consecutive choledochotomies of which 21% required dilatation or sphinectotomy of the papilla Vateri. The number of remaining gallstones was approximately the same in both groups. There were 5 deaths due to postoperative acute pancreatitis in the first series and none in the second. Dilatation of the papilla or sphincterotomy should no longer be performed, routinely in surgery of the biliary tract.
