[Development of pacemaker implantation].

The Morgagni-Adams-Stokes syndrome and the possibility of the excitability of a heart standing still by electric current are already known for a long time. Nevertheless, only in 1952 first success in the treatment in man was seen. In 1958 the first transistorized, working with battery cardiac pacemakers were implanted. The development of the impulse supply for the permanent and temporary stimulation, of the infloating technique, of the impulse production, the source of energy and the working method are demonstrated. In a tabulated survey the comparison of the cardiac pacemakers which are annually implanted in the GDR with those ones in the County Hospital St. George Leipzig is done.

[Extraction of the electrodes by continuous traction--an effective procedure in treating pacemaker wearers with secondary inflammatory complications].

Using a continuous traction method we succeeded in extracting 24 bacterially contaminated and fixed in the heart electrodensounds around 4 years after pacemaker implantation without complications worth mentioning in 7.5 days (1-27). The tissue attached to 20 sound points contained 9 times portions of the myocardium, 9 times remainders of the endocardium, 12 times particles of the connective tissue and 17 times suppurative inflammatory tissue. After extraction of the sound and aimed chemotherapy in all 24 patients the inflammatory pathological conditions rapidly and free from recidivations receded.

[Multicenter study of the results of the extraction of transvenously implanted heart-pacemaker electrodes using continuous traction].

The continuous traction of firmly grown-in collar electrodes from the right ventricle is a simple technique poor in complications with a high success rate, which can be recommended taking into consideration the indications in the failure of the immediate manual extraction and which can be performed in every centre for cardiac pacemakers. Apart from the consideration of adequate informations in literature this evidence above all depends on the evaluation of data of 29 cases from 10 centres for cardiac pacemakers.

[Resection of solitary pulmonary metastases].

From January 1957 to December 1981 45 patients underwent resections (lob-, bilob-, pneumonectomies) due to solitary metastases of the primary tumour in the lung. Concerning the 3- and 5 years survival rate resp. 41 patients could be followed up. The best results could be obtained in patients with metastases from carcinomata of the uterus, kidney and female breasts with an average survival coming up to 44.3%, 32.3% and 32.2%. (22% of our patients had a survival time of more than 5 years, 29% of more than 3 years). In our opinion the resection of solitary pulmonary metastases can be regarded as a surgical method which allows satisfactory results to be obtained.

[Wedge-lobe-resection of the bronchogenic cancer (author's transl)].

It is reported about 70 patients with bronchogenic cancer during the period of 1967 to 1978, which were treated by a wedge-lobe-resection of cardiorespiratory reasons. The results of this operation method don't stand behind those of lobe- respectively pneumonectomy. The reasons are the comparatively lower operation stress and the smaller loss of lung parenchyma. In critical use the economic method of treatment presents a true alternative treatment on patients with increased risk, though it should not remove the classic resection methods.

[Misplaced implantation of a pacemaker electrode in a coronary vein].

The wrong implantation of pacemaker electrodes into a coronary vein is in most cases accidentally diagnosed when thoracic X-ray pictures are made in two planes. It is to be excluded not with certainly in the sagittal ray-path of the thoracic X-ray picture and in the ECG of the extremities. The diagnostic criteria for the recognition of a false position are the end of the electrode which is in the frontal ray-path directed dorsally to the spine and a right bundle branch block in the ECG existing since implantation. Pacemaker-synchronous contractions of the diaphragm as well as an increased already at the time of implantation stimulus threshold may be references. Description of an own observation with an up to now long-term stimulation through a coronary vein without complications lasting 15 months, in which a correction of the position is not regarded as necessary due to the regular function of the pacemaker.
