Bullae in skin grafts.

We describe eight patients with grafted leg ulcers, in whom subepidermal bullous lesions appeared after 4-8 weeks either at the donor or the grafted site. The blisters were seen in the centre as well as just outside the graft. They remained for some days and then disappeared. Histology revealed a normal epidermis and a dermis with a moderate lymphocytic infiltration around the capillaries. No clinical signs of pemphigoid were found and friction does not seem to have been a contributing factor. We have at present no explanation for the appearance of the blisters.

Prospective evaluation of the protected specimen brush for the diagnosis of pulmonary infections in ventilated patients.

To determine the relative cultural accuracy of the bronchoscopic protected specimen brush (PSB) in ventilated patients, we compared this method in the same area of the lung with histologic and bacteriologic examinations of pulmonary specimens. The procedures were concluded just after death when the ventilator was still functioning. Samples via the PSB were taken from a branch of the anterior basal bronchus of the left lower lobe, and immediate left thoracotomy was performed to obtain peripheral specimens from the corresponding segment for quantitative cultures. Twenty-six patients were studied; 6 had histologically proved pneumonia. Using quantitative cultures, a correlation was evidenced between PSB and lung cultures, especially in the subgroup of patients who received no antibiotic during the week preceding their death. No pneumonia was observed when PSB cultures isolated no organisms in concentrations greater than 10(3) colony-forming units (cfu) per ml. The PSB cultures at a level of 10(3) cfu/ml identified every patient with histologically proved pneumonia and every microorganism present in concentrations of 10(4) cfu/g or greater in the lungs. Moreover, in the subgroup of patients receiving no antibiotic, only 4 microorganisms were isolated by the PSB in concentrations greater than 10(3) cfu/ml and not found in the lung.(ABSTRACT TRUNCATED AT 250 WORDS)

Theoretical models in mechanics of the left ventricle.

Different rheological concepts and theoretical studies have been recently presented using models of myocardial mechanics. Complex analysis of the mechanical behavior of the left ventricular wall have been developed in order to estimate the local stresses and deformations that occur during the heart cycle as well as the ventricular stroke volume and pressure. Theoretical models have taken into account non-linear and viscoelastic passive properties of the myocardium tissue, when subjected to large deformations, through given strain energy functions or stress-strain relations. Different prolate spheroid geometries have been considered for such thick shell cardiac structure. During the active state of the contraction, the rheological behavior of the fibers has been described using different muscle models and relationships between fiber tension and strain, and activation degree. A forthcoming approach for bridging the gap between the knowledge of the muscle fiber microrheological properties and the study of the mechanical behavior of the entire ventricle, consists in including anisotropic and inhomogeneous effects through fiber direction field.

Short contact anthralin therapy of psoriasis with and without UV-irradiation and maintenance schedule to prevent relapses.

A short term anthralin application schedule was used at home in 42 patients with severe psoriasis. In another group of 21 patients treated at our day care center, UV-B irradiation was added when the surplus of anthralin had been washed off. Good healing results within 3-8 weeks were noted in most patients. To prevent relapses 31 patients continued the anthralin treatment once or twice a week after healing. In 15 cases this was combined with UV irradiation. One patient relapsed after a severe infection, but 16 others were observed for 6-9 months and 14 for 3-5 months without relapses. Irritation of non-affected skin is common especially at the beginning of the treatment and some staining of clothes and linen can occur. With proper information about this, the treatment is well accepted by the patient who generally finds it reasonably simple without the need of special protective dressings.

[A special type of mechanical acne: fiddler's neck dermatitis].

Seven cases of trauma-induced dermatitis of the neck, are described. They were all the result of violin playing (Fiddler's neck). The histological, clinical and the therapeutic characteristics of our cases are similar to those of mechanical acne.

[Cardiac transplantation: noninvasive detection of rejection by echocardiography].

The main objective of follow-up of cardiac transplant patients is early recognition of acute rejection of the allogenic myocardium. Two patients followed up by the usual protocol also underwent routine M-mode and 2D echocardiography. The data was digitalized and memorized to allow systematic study of the pericardial and cardiac structures, and a comparison of their evolution. Acute rejection was associated with an increase in the acoustic density of the myocardium, especially of the interventricular septum, and an increase in myocardial mass as calculated from M-mode data; an increase of 5 p. 100 of this parameter with respect to an average reference value was judged to be significant. Comparisons were made with electrocardiographic data, especially with an index of voltage. The variability of this index which may be unrelated to rejection was apparent; in addition, there was an interval of about 5 days between the echocardiographic changes and the recording of significant reductions of the electrocardiographic index. Previously reported histological changes explain the precocity and reliability of the echocardiographic results obtained by a non-invasive and reproducible method.

[Comparative echographic and scanographic study of a series of 19 hepatic angiomas].

The correlation of sonography with CT was studied in a series of 19 hepatic angiomas. CT is more specific, since it can demonstrate the vascular nature of the lesion. But it is less sensitive: in three cases it remained negative even after bolus injection of contrast medium. A follow-up strategy, and a decision policy regarding the different examination procedures is discussed.

[Rupture of the abdominal wall in highway accident victims wearing seat-belts].

Rupture of the rectus abdominis muscle represents one of the rare specific lesions due to seat-belts. Two cases are presented which illustrate diagnostic difficulties and the value of the warning provided by these lesions, which are very frequently associated with severe contusion of the small intestine or colon.

[Bullous pemphigoid induced by photochemotherapy of psoriasis. Apropos of 2 cases, with a review of the literature].

PUVA-induced generalized bullous pemphigoid is reported is 8 patients with psoriasis and compared with the bullous pemphigoid described in 25 patients treated by other techniques (Goeckerman, UVB, anthralin, sun exposure, topical steroids). Furthermore they must be differentiated from all the other types of bullae induced by photochemotherapy in psoriatic patients. Diagnostic criteria should be improved in order to allow a better classification of these bullous diseases and to separate the photo-toxic bullae which are the commonest, from other bullae such as epidermolysis bullosa acquisita, localized bullous pemphigoid and drug-induced bullous pemphigoid.

[Infectious endocarditis of a healthy heart in an infant].

The case of a 22 months old child with no previous medical history hospitalised for an acute infection with pyrexia, arthritis, meningitis and leukocytosis with polynucleosis is reported. All bacteriological investigations were sterile; the search for soluble antigen and serological tests were negative. Antibiotic therapy (Ampicillin and Thiamphenicol) cured the meningitis and arthritis. On the 10th day of treatment the temperature rose, a systolic murmur was detected and echocardiography showed the presence of a large vegetation on the anterior mitral leaflet. Three weeks later (on Ampicillin and Amikacine), asymptomatic abolition of the femoral pulses and disappearance of the vegetation on echocardiography were observed. Angiography confirmed obstruction at the bifurcation of the aorta. Surgical removal of the embolism resulted in revascularisation of the femoral artery and was followed by apyrexia. This infant probably developed endocarditis on a healthy heart. It was complicated by systemic embolism and mitral regurgitation which at present is well tolerated.

[Conditions of methemoglobine dosage in veterinary toxicology (author's transl)].

Evolution of the percentage of methemoglobine in blood was studied to find a method of conserving samples. It is recommended that blood be frozen rapidly after sampling due to instability of the compound, and that this low temperature be maintained until analysis. Other processes have a limited effectiveness, namely dilutions in aqueous solution of NaCl (7%) or in phosphate buffer (pH 6.5) with hemolysis (saponine) and keeping at +4 degrees C.

[Echocardiography in the evaluation of constrictive pericarditis. 25 cases].

Twenty five patients with constrictive pericarditis underwent cross-sectional and M-mode echocardiography before pericardectomy. The numerical data were compared with those in thirty healthy subjects. The various parameters noted regarding the left cavities were as follows: --The presence of a pericardial detachment which may sometimes be quite large (more than 10 mm in 6 cases); the degree of parallelism of the pericardial membranes in systole, which is all the less maintained as the detachment is severe. --The increase in size of the right ventricle and decrease of the left, with p less than 0.001 in both cases. --The absence of posterior motion of the left side of the septum in systole in 22 cases, followed by a sudden posterior displacement during the rapid filling stage, after which there is virtually no motion until the end of diastole. --The presence at the level of the posterior wall of a so-called slow filling slope, which is smaller (1.9 +/- 1.5 mm/s) than in the normal subject (p less than 0.001). --A change in left ventricular endocavitary diameter variations, which results from the two previous findings. The tracings thus show the compliance disorder with a very steep rapid filling slope and a plateau during the slow filling stage. --Finally the changes in mitral kinetics involving an earlier (p less than 0.001) and lower (p less than 0.001) F point than in the control series and on the other hand an larger E wave, sometimes forming a summation with A wave in sinus rhythm and a false A wave in atrial fibrillation. We think this type of motion is due to the disturbances in the two mechanisms (recirculation, breaking of a jet) which in physiological conditions seem to combine to chose the mitral valve at point F and account for the valve position in mid-diastole.

[Ultrasonic topographical study of pancreatic vascular relationships. II. Left renal artery and vein (author's transl)].

An ultrasonic study of 100 normal subjects, associated to the analysis of 17 anatomical preparations, showed that, in 60% of subjects, the left part of the pancreas is located in front of the left renal vessels; this means that the pancreas possesses a much more caudal situation than classically admitted by most of sonologists, and by certain anatomists. An ultrasonic study of the pancreas must therefore begin with transverse scans; the scanning orientation will be modified if the pancreas is oblique; such a modification is immediate if the examination is carried out in real time and in the standing position, a technique which must be routinely employed.
