Surgical treatment of scaphoid pseudarthrosis: evaluation of the results after soft tissue arthroplasty and inlay bone grafting.

A retrospective study of the results after surgical treatment of scaphoid nonunion in two orthopedic centers is described. One center used Bentzon 's soft tissue-arthroplasty method in 15 cases, and the other center used Matti - Russe bone grafting procedure in 32 cases. Of these 47 cases, 46 had established pseudarthroses. The clinical results were similar in both methods (average follow-up time: 61.4 and 44.0 months, respectively). Soft tissue arthroplasty permitted an earlier return to work, but the risk of long-term degenerative changes developing cannot yet be excluded.

[Preoperative intra-arterial digital subtraction angiography in aortoiliac vascular occlusion].

Sixteen patients with advanced aorto-iliac occlusive disease were examined with digital subtraction angiography (DSA) using intraarterial injection of contrast medium. The preoperative evaluation of the vessels distal to the occlusion and the visualization of the femoral and popliteal arteries were successfully performed with intraarterial DSA. As compared to conventional angiography the marked contrast enhancement of DSA is demonstrated. Less spatial resolution of DSA lacks importance as the herein discussed topic is concerned. The limited size (23 cm) of the image intensifier of the X-ray equipment used was a major drawback of the method. Using intravenous DSA aorto-iliac occlusion can be shown. In patients with long range vascular occlusion and sparse development of collaterals only intraarterial DSA can demonstrate the run-off vessels including the trifurcation of the popliteal artery.

[Spun glass hair (author's transl)].

A 5-year-old girl with the characteristic clinical picture of "spun glass hair" is described; additionally she suffers from atopic dermatitis. This case is of special interest since the child not only suffers also from endogenic eczema, but that another member of the family (child's aunt) also suffers from the same hair anomaly. The diagnosis is based upon the clinical symptom that the hair cannot be managed, and is proved by observation of triangular and kidney-shaped cross-sections taken from the child's hair. Electron scan microscopic examination of the hair revealed grooves on the surface of the hair which exhibit similarity to the hair-condition pili canaliculi. The following physical properties of these irregularly shaped hairs were measured: Fibre thickness, tensile strength, extent of fibre expansion shortly before breakage, and form elasticity. None of these properties were found to differ from those of normal hair.

Life quality assessment of breast cancer patients receiving adjuvant therapy using incomplete data.

Previous research on the effects of adjuvant treatment for women with operable breast cancer focused exclusively on disease-free and overall survival. In this study we evaluate life quality of premenopausal node-positive breast cancer patients receiving adjuvant chemotherapy for at least three months. For the first time, a modified latent variable model is used to assess treatment outcome in a prospective clinical trial. This poses a number of econometric problems which did not occur in the preceding studies. One of them is how to deal with patients whose records are incomplete. The data are provided by the International Breast Cancer Study Group (study VI). The results indicate that the lowest dose treatment improves life quality faster than the remaining three alternatives. At the end of the 24 months observation period no significant differences between the four treatment options remain. Although the lowest dose treatment is also the least costly no definite conclusion regarding cost-effectiveness can be drawn at this stage since survival data is not yet available.
