ERCP-guided percutaneous fine-needle pancreatic biopsy.

In patients with a radiologic diagnosis of unresectable pancreatic carcinoma, exploratory laparotomy for tissue diagnosis is no longer required. Histologic confirmation of the diagnosis may be obtained safely and accurately with percutaneous fine-needle aspiration biopsy. Endoscopic retrograde cholangiopancreatography (ERCP) precisely localized the biopsy site for cytologic diagnosis of adenocarcinoma in 13 of 14 patients (93 percent) with pancreatic carcinoma.

Inverted diverticula of the gastrointestinal tract.

Inverted diverticula of the gastrointestinal tract are rare. This paper briefly presents the roentgen findings of an inverted Meckel's diverticulum and is the first reported case of the roentgen appearance of an inverted colonic diverticulum.

Forums in gastrointestinal roentgenology: transhepatic portal venography and selective obliteration of gastroesophageal varices using isobutyl 2-cyanoacrylate (bucrylate).

Five patients with Child's class C alcoholic cirrhosis and actively bleeding gastroesophageal varices underwent transhepatic portal venography and selective obliteration of varices with isobutyl 2-cyanoacrylate (Bucrylate). Temporary control of bleeding was obtained in four patients and complete control in one. This new technique is best utilized to stop active variceal hemorrhage in patients who are not candidates for portasystemic shunt surgery or to control bleeding while the patient's general medical condition is improved to decrease the risk of subsequent shunt surgery.

Cavernous hemangioma of the liver: ultrasonography, arteriography, and computed tomography.

Gray-scale ultrasonography and angiography were performed on 5 patients with cavernous hemangioma of the liver. Four patients also had radionuclide scans and 2 had computed tomography. Angiography was the most specific diagnostic examination; the findings of the other 3 techniques were nonspecific, yet valuable for initial identification of the lesions and their characterization as solid tumors.

Long-term radiographic-pathologic follow-up of patients treated with visceral transcatheter occlusion using isobutyl 2-cyanoacrylate (Bucrylate).

Visceral transcatheter occlusive therapy was performed with isobutyl 2-cyanoacrylate (Bucrylate) in 14 patients. Of 10 patients who subsequently died, postmortem examination in 6, performed 2--196 days post-embolization, showed only a mild histiocytic foreign body giant cell reaction to Bucrylate. The reaction was confined to the vessel lumina and did not involve the vessel walls or contiguous parenchymal tissues. Clinical and radiographic follow-up in the other 4 patients (range = 30--180 days) showed no evidence of untoward reaction to Bucrylate. No ischemic or inflammatory complications were observed in any of the peripheral organs.

Double-contrast gastrography of the fundus and cardia: normal landmarks and their pathologic changes.

The normal radiographic landmarks of the gastric fundus are the macromucosal (rugal) pattern, the micromucosal (areae gastricae) pattern, and the esophageal rosette. In a comparison of 100 normal double-contrast and conventional barium examinations, the landmarks were most consistently and reliably demonstrated by double-contrast technique. In a group of 36 patients with various types of fundal pathology, double-contrast gastrography demonstrated distinctive changes in these landmarks and led to reliable diagnoses in the majority of patients. Double-contrast gastrography is considered the preferred method for examining patients with suspected abnormalities of the fundus or cardia.

Impact of new diagnostic imaging methods on pancreatic angiography.

The results of 100 consecutive pancreatic arteriograms performed in concert with a combination of other diagnostic procedures (gray scale ultrasonography, computed tomography, endoscopic retrograde cholangiopancreatography, percutaneous transhepatic cholangiography) were evaluated to determine the value of angiography in diagnosis and management of patients with known or suspected pancreatic disease. Angiography was found to be valuable for diagnosis in 68% (68/100) of cases and was considered helpful for management in 81% (57/70) of patients with pancreatic neoplasm, pancreatitis, or a nonpancreatic abnormality.

Transcatheter occlusive therapy of genitourinary abnormalities using isobutyl 2-cyanoacrylate (Bucrylate).

Transcatheter occlusive therapy was performed with isobutyl 2-cyanoacrylate (Bucrylate) in 14 patients with a variety of genitourinary abnormalities. Bucrylate was found to be a valuable agent when used to: (1) occlude vessels with very slow flow; (2) occlude vessels in patients with coagulopathies; (3) occlude high flow arteriovenous fistulas; (4) perform superselective vessel occlusion; and (5) produce permanent vessel obliteration. Bucrylate was used effectively in this group of patients and was used without complications or evidence of histotoxicity.

Villous tumors of the stomach and small bowel.

Villous tumors of the stomach and small bowel are premalignant lesions with focal adenocarcinomatous change as high as 72%. They are frequently multiple and associated with other gastrointestinal neoplasms. Thirty percent of those in the stomach are associated with an independent gastric carcinoma. Villous tumors have morphological features that facilitate specific roentgenographic diagnosis. Endoscopic biopsy may confirm the diagnosis but may give false-negative results for malignant neoplasms and therefore should not delay treatment. After diagnosis, surgical resection is mandatory and subsequent radiological-endoscopic follow-up is essential.

Ultrasound findings of renal angiomyolipoma.

Angiomyolipoma (AML) IS A BENIGN RENAL NEOPLASM. Preoperative diagnosis is important because of the different surgical approaches to benign and malignant tumors. Angiographic findings of AML and renal carcinoma may be similar and differentiation difficult. B-mode ultraslund examination of AML demonstrates a different image pattern which may be helpfou in differentiation.

Selective testicular venography for localization of nonpalpable testis.

Selective testicular venography is a safe, accurate outpatient procedure for preoperative localization of a nonpalpable testis. It may obviate surgery if testicular agenesis is identified or expedite surgical exploration by precise preoperative localization of the undescended testis.

Rapid diagnosis of pancreatic carcinoma.

An algorithmic approach for radiological evaluation of suspected pancreatic carcinoma was applied in more than 200 patients. Outpatient ultrasonography and barium studies of the upper gastrointestinal tract were followed by inpatient endoscopic retrograde cholangiopancreatography, angiography, and percutaneous biopsy. Evaluation took no more than two hospital days and exploratory laparotomy was not needed. Twenty-five patients had carcinoma. The diagnostic accuracy of the algorithm was 96%. Although this method did not increase the diagnosis of resectable tumors, it did result in rapid, accurate diagnosis of pancreatic carcinoma at relatively low cost and with minimum patient discomfort. Hopefully, these results will eventually lead to earlier diagnosis and improved survival.

Cholangiographic abnormalities in patients with inflammatory bowel disease.

Twenty patients with inflammatory bowel disease, abnormal liver function tests and abnormal endoscopic retrograde cholangiograms were found to have a spectrum of abnormalities affecting the intra- and extrahepatic biliary trees. The intrahepatic systems were abnormal in all patients and demonstrated ductal stenosis, ectasia, decreased arborization and major duct obstruction. The extrahepatic systems were abnormal in 75 percent of cases with stenosis, diverticula formation and mural irregularity being the most frequent abnormalities. The value of endoscopic retrograde cholangiography in this patient group is to exclude extrahepatic obstruction, establish a nonoperative diagnosis, and assist in determining the method of treatment.
