Oral submucous fibrosis as a precancerous condition.

An evaluation of the malignant potential of submucous fibrosis, based on observations of 89 patients with the disease in Ernakulam District, Kerala, India was done. Out of the 89 patients, 12 were referred patients and 77 were diagnosed in population based studies. At the time of first examination oral cancer was found to be coexistent in nine (10%) cases. For 66 patients follow-up observations ranging from 4 to 15 yr (median 8 yr) were available. During this period malignant transformation was observed in three patients (4.5%). Considering malignant transformation and coexistence together, oral cancer was observed in 13% of the present material. Leukoplakia was found in 26% of the patients with submucous fibrosis. Out of 42 submucous fibrosis patients biopsied, 12% showed squamous cell carcinoma, 26% epithelial dysplasia, and 76% atrophic epithelium. These findings reinforced the hypothesis that submucous fibrosis is a precancerous condition.

Glycogen content in Gastrothylax crumenifer and Cotylophoron orientale after the isolation of parasites, their starvation and refeeding with various carbohydrates.

The average reserve glycogen content in unstarved (control) parasites, 4.7 +/- 0.14 g/100 g F.T. (fresh weight of tissue) in Gastrothylax crumenifer and 3.15 +/- 0.14 g/100 g F.T. in Cotylophoron orientale has been estimated biochemically. The consumption of endogenous glycogen was found to be 18.47%, 44.37%, 66.66% and 78.13% of F. T. in G. crumenifer and 14.92%, 40.95%, 66.34% and 71.42% of F.T. in C. orientale during 12, 24, 36 and 48 hours of starvation. Both the parasites were able to resynthesize 71.71% and 75.42% of lost glycogen (maximum up to 16 hours) on refeeding in nutrient medium containing glucose. The maximum resynthesis of lost glycogen in both the parasites was observed in nutrient medium containing glucose rather than the other carbohydrates such as fructose, lactose, sucrose and trehalose.

Post-hypoxic leukoencephalopathy with late recovery.

A man developed severe spastic quadriplegia following a mixed drug overdose-induced coma. Diagnostic studies were consistent with hypoxic leukoencephalopathy. The patient made a dramatic functional recovery 3 1/2 months after admission, with Computed tomography scan and EEG findings returning to normal. The time course of recovery suggests that remyelination may have taken place. This case illustrates the importance of early and continued vigorous rehabilitation therapy for patients in whom late recovery from critical illness is possible.

Passive measurement of radon and thoron using TLD or SSNTD on electrets.

An electret is an electrical analogue of a permanent magnet and it carries a permanent electric charge. Our previous work has shown that such electrets are suitable for collecting decay products of radon and thoron in passive chambers. In the present work, the decay products are directly collected on the surface of a TLD or SSNTD providing in situ registration of the radiation from the decay products of radon and thoron. A 101. chamber, the sides of which were covered with a layer of Whatman No. 1 (W-1) filter paper, showed the following responses: (i) SSNTD (CR-39) recorded 92 +/- 13 tracks per cm2 per pCi/l, hr for radon and 9 +/- 1.5 tracks per cm2 per pCi/l. hr for thoron; (ii) for similar levels TLD (CaF2(Dy)) chips recorded an equivalent of 1.35 +/- 0.16 mR for radon and 0.30 +/- jk0.09 mR for thoron. Taking advantage of the differential response of the two chambers (one covered with a layer of W-1 filter paper and the other with a 75 mm polyurethene foam), simultaneous measurement of radon and thoron could be achieved.

Comparison of carcinogenicity of betel quid with and without tobacco: an epidemiological review.

A comparison of carcinogenicity of the habit of chewing betel quid with and without tobacco has been attempted through a reassessment of the available epidemiological evidence on aetiology of oral cancer and pre-cancer. Primary evidence comes from the reports which emphasize a low incidence of oral cancer among population groups who practise the habit of chewing betel quid without tobacco compared with those which practise the habit of chewing betel quid with tobacco. A more specific comparison is sought here through the quantitative estimation of the relative risks of the two types of betel habits. Out of three studies selected, two studies showed that the relative risk for the habit of betel quid without tobacco was not significant whereas the relative risk for the habit of betel quid with tobacco was highly significant. In the third study, although both relative risks were significant, the relative risk for the habit of betel quid with tobacco was significantly higher than the risk for the habit of betel quid without tobacco. The evidence from studies of oral pre-cancer was also in the same direction. It is suggested that in intervention and primary prevention programmes for oral cancer, the emphasis in the first instance might be upon the exclusion of tobacco from the betel quid rather than the complete discontinuation of the habit of chewing betel quid.

Chewing and smoking habits in relation to precancer and oral cancer.

In a prospective epidemiologic house-to-house survey of a random sample in the district of Ernakulam in Kerala State, the annual incidence rate of leukoplakia per 1,000 adults was found to be 2.1 for males and 1.5 for females. The rate was highest in the mixed tobacco habits group and lowest (0) in the no habits group. During the same period, oral cancer developed only among the individuals, who had a history of a previously diagnosed oral lesion. Malignant transformation was significantly higher among the speckled leukoplakia cases. The rate of malignant transformation was also highest among leukoplakias associated with tobacco chewing habits. These results suggest that leukoplakias associated with different tobacco habits may have a different natural history.

Incidence and early forms of oral submucous fibrosis.

The yearly incidence of submucous fibrosis per 100,000 person-years was found to be 9 for males and 20 for females over a 10-year period in the Ernakulam district of Kerala, South India. In the same area possible early case of submucous fibrosis, under the clinical diagnosis of leukoplakia and lichen planus, showed the typical histologic features of the disease.
