Treatment of drusen-related aging macular degeneration by photocoagulation.

This retrospective clinical study of the treatment of DR AMD by scatter pattern photocoagulation reveals a stabilization of vision and disappearance of drusen in 52% of 42 eyes followed for an average of 3 years. With the minimal risks involved, the benefit of temporary improvement or stabilization of vision in some eyes would make this technique worthy of further evaluation.

Optic disk findings in hypervitaminosis A.

Three cases of papilledema secondary to chronic excessive vitamin A intake are presented, and the optic disk changes are documented with intravenous fluorescein angiography. Two of the three patients reported in this study were symptomatic with blurred vision and systemic complaints. The symptoms of blurred vision and systemic complaints disappeared within one week, and papilledema resolved over several months after discontinuance of vitamin A. The fluorescein angiographic changes observed in the optic disk of patients with hypervitaminosis A are similar to those associated with other known causes of papilledema. Since vitamin A is a nonprescription drug, and its indiscriminate use is potentially great, any history of vitamin ingestion should be elicited during the evaluation of papilledema.

The treatment of acute branch vein occlusion by photocoagulation.

Twenty-eight eyes with retinal branch vein occlusion were treated by photocoagulation. The clinical posttreatment findings indicated rapid clearing of hemorrhages and exudates. Visual acuity in 12 (43%) eyes improved, in 11 (39%) remained static, and in five (18%) worsened.

Diagnosis and management of anterior uveal tumors.

Iris fluorescein angiography is another useful method in the evaluation of anterior uveal tumors. Three cases are presented which indicate that, in the diagnosis and management of anterior uveal tumors, certain fluorescein angiographic patterns may suggest malignancy. The presence of early hyperfluorescence, additional tumor foci, interruption of the normal iris radial vascular pattern by dilated tortuous vessels, and late leakage of fluorescein are suspicious findings, and indicate the necessity for comprehensive evaluation of such tumors.

The intracapsular versus the extracapsular cataract technique in relationship to retinal problems.

Five hundred and sixty-four consecutive eyes after cataract surgery with intraocular lens implantation were studied in relationship to the incidence of retinal detachment and cystoid macular edema in the intra vs the extracapsular extraction technique. In 124 eyes undergoing intracapsular cataract extraction, three (2.4%) developed retinal detachment. In 440 eyes undergoing extracapsular cataract extraction two (0.45%) developed retinal detachment. In 87 eyes undergoing intracapsular cataract extraction 7 (8%) developed cystoid macular edema. In 327 eyes undergoing extracapsular cataract extraction 4 (1.2%) developed cystoid macular edema. This study cannot be compared with other series in the literature because high risk cases and those with vitreous loss were excluded.
