Hazard assessment of substances produced from the accidental heating of chemical compounds.

Accidental events concerning process industries can affect not only the staff working in, but also the environment and people living next to the factory. For this reason a regulation is imposed by the European Community to prevent accidents that could represent a risk for the population and the environment. In particular, Directive 96/82/CE, the so-called 'Seveso II directive', requests a risk analysis involving also the hazardous materials generated in accidental events. Therefore, it is necessary to develop simple and economic procedure to foresee the hazardous materials that can be produced in the case of major accidents, among which the accidental heating of a chemical due to a fire or a runaway reaction is one of the most frequent. The procedure proposed in this work is based on evolved gas analysis methodology that consists in coupling two instruments: a thermogravimetric analyzer or a flash pyrolyzer, that are employed to simulate accident conditions, and a FTIR spectrometer that can be used to detect the evolved gas composition. More than 40 materials have been examined in various accident scenarios and the obtained data have been statistically analyzed in order to identify meaningful correlations between the presence of a chemical group in the molecule of a chemical and the presence of a given hazardous species in the fume produced.

Synthesis and characterization of poly(amido-amine)s belonging to two different homologous series.

Systematic work on the synthesis and characterization of two series (referred to as type A and B) of poly(amido-amine)s with tertiary amino-groups in the main chain is reported. The polymers were synthesized by polyaddition of alpha, omega-bis-(methyl-amino) alkanes with 1,4-bis-acryloylpiperazine (type A) and 1,12-bis-acryloyl-n-diaminododecane (type B). These materials, which will be employed in the preparation of block and graft copolymers for biomedical use, have been characterized by means of various techniques and the most interesting features are reported.

Radioimmunoguided surgery.

Although clinical staging of neoplastic diseases has long remained the only approach to prognosis and treatment, parameters for stage determination, such as tumor size (T) and lymph-node involvement (N), do not always provide effective indication of optimal treatment. Molecular medicine has also provided useful indications as to an alternative and/or integration to clinical staging. Despite the numerous possibilities afforded by pre-operative staging techniques, failures in defining the real spread of neoplasias into surrounding structures have remained a very important diagnostic problem. The labeling of monoclonal antibodies binding with neoplastic target cells by way of radioactive isotopes introduced the techniques known as immunoscintigraphy and SPECT, which then evolved into radioimmunoguided surgery. Fourty patients suffering from colorectal cancer whose age ranged between 42-82 years were singled out for this study. Before undergoing surgery, they were administered pancoloscopy and macrobiopsies, AP-LL chest x-rays, hepatobiliary ECT, echoendoscopy, abdomen and pelvis CT with nephrostographic phase, and total body bone scintigraphy. They were treated with 125I-B72.3 and 125I-FO23C5 (5% and 95% of patients, respectively). Thyrosuppression was achieved by Lugol solution (15 drops x 3/die) from the 6th day before infusion and until the day of surgical operation. Radioimmunoguided surgery (RIGS) has also been tested on staging and second-look of ovarian tumors. Five years after surgical operation the survival rate of Dukes A patients (15%) was confirmed to amount to 100%, whereas for Dukes B patients (50%) having undergone RIGS-guided exeresis on single unrecognized metastases (2 patients) and on unrecognized n+ (5 patients) the survival rate was found to be 85% after 5 years; 2 patients deceased due to relapse; 1 patient deceased due to e.p.a. Finally, for Dukes C patients; (35%) having undergone RIGS-guided exeresis on unrecognized liver micrometastases (1 patient), on single isolated metastases (2 patients) and in the occurrence of multicentric lymph-node positivity (9 patients), the survival rate after 5 years was found to amount to 64%; 5 patients deceased due to relapse. Out of 19 patients without pre-operative evidence of ovarian tumor as opposed to just 1 patient suspected of pelvic recurrence, after intra-operative surgical radicalization (45%), 1 patient was diagnosed fibrosis (suspicious lesion on CT) and 1 other patient peritoneal MTS (negative CT) by means of RIGS. RIGS has made it possible: to localize primary and/or metastatic lesions; to determine tumor-free margins, loco-regional disease spread; to differentiate suspicious foci on inspection and palpation (biotopic sampling); to detect invisible and impalpable tumor foci (occult sites); to verify radical exeresis; to evaluate lymphatic drainage stations; to enable guided exeresis of liver metastases.

[Proctologic day-surgery. Results of 2000 surgical interventions].

The aim of the study was the evaluate of results of 2000 surgical operations for ano-rectal disease performed in the day-surgery setting (7-24 hours hospital stay) with improvement of both cost effectiveness and patient comfort. From January 1980 to December 1998, 2000 patients underwent surgical operations: 1011 for haemorrhoids; 708 for anal fissure; 172 for fistula in ano; 80 for pylonidal disease; and 45 for anal stenosis. 97.6% of patients were operated on with loco-regional anaesthesia; the others with narcosis and peripheral anaesthesia. The hospital-stay was 24 hours in 697 patients (34.5%), while 1319 (65.5%) operated on under loco-regional anaesthesia were hospitalised for 7-10 hours. Three patients (0.2%) developed acute hemorrhage after hemorroidectomy during the immediate postoperative period. They underwent reintervention under general anaesthesia with a hospital stay of 7 days. Four patients (0.6%) with perianal abscess after internal sphincterotomy underwent incision 10 days after the operation. Two patients with perianal hematoma after sphincterotomy prolonged the hospital stay for three days. In 1048 patients (51.9%) clinical recovery was observed at first follow-up (7 days); 48% had recovered at the 2nd follow-up (14 days). In 1608 patients (98%) anatomical recovery was observed at the follow-up three months after surgery. Patient satisfaction 6 month after operation was high in 79%; good in 27%; low in 1%. These results seems confirm the feasibility of proctological day surgery in almost all patients, with both a considerably cost reduction and enhanced patient comfort and compliance.

[Breast carcinoma in the elderly].

Breast cancer represents 40% of all carcinomas and is responsible for the 20% of deads due to a cancer in female population. Patients in more of 50% age over 65 years. 150 patients with breast cancer older then 65 years underwent surgery. T1 and T2 < 2 cm patients were treated with quadrantectomy and radiotherapy. T2 > 2 cm and T3 patients underwent radical mastectomy. T4 patients were treated with neoadjuvant chemotherapy before surgery. Mean follow up was 48 (range 12-78) months. 43 patients (28.6%) had a progression of the neoplasia. Five years mortality rate was 18.6%. Surgery nowadays represents the treatment of choice for breast cancer even in elderly woman. Associated diseases in these patients shouldn't always considered an absolute contraindication for surgery.

[Carcinoma of the male breast: reconstructive technique].

Male breast cancer is a rare neoplasm. Adhesions in its deeper and superficial levels and axillary adenopathies (50-60%) are often found at the diagnosis time. Surgery is considered the main step in the treatment of the male breast cancer. Radical mastectomy often leads to a wide asportation of the skin consequently causing some problems in the management of the chest-wall defect. Otherwise inadequate resections can cause local recurrences. The Authors, after a brief analysis of the main aspects concerning the prognosis and the management of the male breast cancer, report the use of transverse thoracoepigastric skin flap in the reconstruction of surgical wound after mastectomy.
