Melatonin in psychiatric disorders: a review on the melatonin involvement in psychiatry.

In normal subjects, the secretion of melatonin, the pineal hormone that regulates the rhythm of many functions, exhibits a circadian pattern synchronized with the day-night cycle. An alteration of this secretory pattern has been found in various psychiatric disorders (seasonal affective disorder, bipolar disorder, unipolar depression, bulimia, anorexia, schizophrenia, panic disorder, obsessive compulsive disorder). At present, it is not known if such alterations have an etiological role or are secondary to the dysfunctions underlying the different disorders. In addition, we do not know if the involvement of melatonin in several disorders has the same significance in the pathophysiology of each disorder. An understanding of the role of the pineal hormone and of its alterations in psychiatric diseases could help to identify the biological mechanisms underlying such disorders.

Season of birth in panic disorder.

Several studies have investigated the seasonal distribution of the birth dates of patients with psychiatric diseases. Our purpose was to verify if there is a specific distribution (by month) of birth dates in subjects with panic disorder (PD). The birth dates of 843 outpatients with a diagnosis of PD were compared with those of 1,181 subjects with other mental diseases. The birth dates of psychiatric patients were compared to those of the general Tuscane and Italian populations. The monthly distribution of birth in patients with PD (with and without comorbidity) peaked in September to December, while no relevant deviation in birth rate was observed in other mental diseases. Our results suggest a pathogenic role of birth seasonality in the development of PD.

Lectin binding sites in parotid acinar secretory granules of normal and isoproterenol treated rat.

Lectin staining patterns in secretory granules of rat parotid gland acinar cell of untreated and isoproterenol-injected animals were examined by electron microscopy. We used four lectin-gold complexes: Ulex europaeus agglutinin I (UEA-I), Helix pomatia agglutinin (HPA), wheat germ agglutinin (WGA), Glycine max agglutinin (SBA). Specimens were low temperature embedded in the hydrophilic Lowicryl K4M resin. The normal acinar cells produced glycoconjugates which were positive for all of the lectins used and with a characteristic topographic distribution in relation to the morphological type of granule. The cells of isoproterenol-treated rat showed marked ultrastructural changes in the size and structure of granules; significant changes in lectin binding sites in the granules were also observed.

Suicidality and aggressive behaviour.

Psychiatrists have always maintained that there is a relationship between aggressive behaviour and suicide in depressed patients. However, this relationship is based on inconsistent and undocumented hypotheses, not on reliable clinical experimental data. The present study was designed to investigate the relationship between aggressive behaviour assessed by means of the Buss and Durkee Hostility Inventory (BDHI), and suicide in a sample of 134 depressed out-patients. The group with a higher level of suicidal behaviour was of younger age. The association between depressive subtypes (major depression, recurrent; major depression, single episode; bipolar disorder, depressive episode; dysthymia) and suicidality was found to be statistically significant. In contrast, there was no correlation between depressive subtypes and aggressive behaviour. The relationship between suicide and guilt as measured by the BDHI suggests that, in depression, suicidal behaviour becomes part of a symptom pattern in which aggression does not appear to be the main component. The suicide dimension arises when the cognitive sphere is involved. In fact, in depression, suicide is included among the cognitive disturbances, together with guilt, paranoid and obsessive-compulsive symptoms, depersonalization/derealization and agitation.

Season of birth in psychiatry. A review.

Numerous studies suggest that seasonal birth may play a pathogenic role in the development of mental disorders. A birth excess of 10% during winter and spring has been shown in schizophrenia. The few studies carried out on affective disorders revealed a significant increase of births in the first quarter of the year in bipolar disorders and major depressive disorder. Subjects with seasonal affective disorder show a peak of births in May. Data on personality, eating and 'neurotic' disorders are less consistent. At the moment there are no data in the literature about anxiety disorders.

Comparison of aggressive behavior between men and women in sport.

A group of 33 sportsmen and 43 sportswomen and a group of 488 subjects (less than 30 years old) selected from a sample of 816 subjects representative of the general Italian population were compared using the factor scores and total scores on the Italian version of the Buss and Durkee questionnaire. Analysis suggests a positive correlation between aggressiveness and sport and are compatible with the hypothesis that sporting activities serve as a means of controlling aggressiveness of women and help the individual psychosocial development of men. The decision to practice sports would seem to be connected with the needs to conform to cultural stereotypes, which portray the man as aggressive and the women as gentle and submissive.

Imipramine binding as a predictor of fluoxetine response in depressed patients.

We evaluated platelet 3H-imipramine (3H-IMI) binding parameters (Bmax and Kd) at baseline (t0) and 2 months after (t1) treatment with fluoxetine in a group of outpatients affected by a major depressive episode, according to DSM-IIIR criteria. The possible relationships between biological parameters and the Hamilton Rating Scale for Depression (HRSD) total score were examined. The results confirmed previous reports that depressed patients possessed a lower density (Bmax) of platelet 3H-IMI binding sites than healthy controls, but the Bmax tended to decrease with fluoxetine. In addition, we observed a negative correlation between Bmax values at t0 and the HRSD total scores at t1 indicating that the patients who responded better to fluoxetine were those with a lower Bmax at t0. Future studies will substantiate this preliminary observation on the possible usefulness of platelet 3H-IMI binding as a predictor of pharmacological response.

Methadone dosages and therapeutic compliance during a methadone maintenance program.

In 97 heroin addicts in a methadone maintenance treatment program (MMTP) with social services, urine screening for opioid metabolites identified groups with different treatment compliance. The positive correlation between compliance and methadone dosage confirms that MMTPs are effective in rehabilitating patients when 'adequate' dosage, i.e., capable of preventing relapse, is used.

Platelet serotonergic markers and aggressive behaviour in healthy subjects.

The availability of peripheral markers of the serotonergic system, which appears to play a major role in aggressive behaviour, prompted us to investigate in healthy subjects the possible correlations between platelet 3H-imipramine (3H-IMI) binding or mono-amine-oxidase (MAO) activity and aggression as measured by the Italian version (QTA) of the Buss and Durkee Inventory. The results showed the presence of significant and positive correlations between MAO activity and QTA total scores and the scales of negativism and verbal aggression, and no correlation between 3H-IMI binding parameters and QTA. These findings seem to suggest that perhaps the MAO activity is more linked to the dimension of aggression than is IMI binding.

Psychological stress and body temperature changes in humans.

We investigated the possible changes in body temperature, heart frequency, and blood pressure in 22 residents sitting for the yearly exam at the Specialty School of Psychiatry at Pisa University. All subjects were then evaluated 2 or 3 weeks later, in calm situations. In a subgroup, a specific plasmatic diazepam binding inhibitor (BBIA), previously described, was also measured. The results showed that all subjects underwent significant stress-related changes in the parameters studied, which suggest the involvement of different mechanisms in preexam stress.

Emotivity, personality, and task-dependent EEG asymmetry.

In order to verify if task-dependent EEG asymmetry is related to emotivity or personality traits, the relationship between EEGraphic (EEG mapping) asymmetry, personality aspects, and emotional-affective state in 12 healthy volunteers was evaluated by means of standardized methods (SAD and CPI). Our subjects show an EEG asymmetry which is currently attributed in the literature to anxiety. In our subject the absence of anxiety and the presence of hyperthymic characteristics suggests that the different individual trends observed in various studies which have used methods similar to ours, could, at least partly, depend on interference on these variables.

Personality features and platelet 3H-imipramine binding.

In an attempt to provide further information on the functions of the serotonergic system in humans, we investigated the possible correlation between a peripheral serotonergic marker, such as platelet 3H-imipramine binding, and the Minnesota Multiphasic Personality Inventory (MMPI) items and scales in a group of 20 healthy subjects. The results showed the presence of significant positive correlation between the maximum binding capacity, a measure of the number of 3H-imipramine sites, and 17 MMPI items suggestive of personality features of self-certainty, feeling of well-being and sense of reality.

Stress-related changes of benzodiazepine binding inhibitory activity (B.B.I.A.) in humans.

The presence of benzodiazepine binding inhibitory activity (B.B.I.A.) in sera from 44 psychiatric patients and from 14 healthy volunteers, prompted us to investigate whether or not this activity underwent changes in stressful situations. We measured the inhibitory units (IU) of deproteinized sera of 12 subjects, immediately before and 2 weeks after sitting for a difficult university exam. Our results showed significantly higher IU values (i.e., higher B.B.I.A. concentrations) in the samples taken just before the exam. This preliminary finding clearly suggests the involvement of B.B.I.A. in anxiety mechanisms.

Interaction between the serotonin and dopamine systems in humans: preliminary findings.

The availability of peripheral tools, such as platelet [3H]-imipramine binding and electroretinogram b-wave amplitude, for investigating serotonin and dopamine systems respectively, prompted us to explore the possible relationships between these two systems in a group of 8 healthy drug-free volunteers. The results showed a significant correlation between the peripheral parameters of serotonin and dopamine systems and underline the importance of studying different neurotransmitters at the same time in humans.

Age-related differences in human platelet 5-HT uptake.

Platelet 5-hydroxytryptamine (5-HT) uptake was measured in a group of young healthy volunteers and a group of aged healthy volunteers. Our results show a significant increase in the maximal velocity of uptake (Vmax) in the platelets of aged subjects. This could result in a decreased concentration of extracellular 5-HT in old age. Such reduced levels of 5-HT, if they occurred at 5-HT synapses in the brain, could result in susceptibility to the development of neurological or psychiatric illness.
