The correlation of serum prolactin level and psychic stress in women undergoing a chronic hemodialysis programme.

Altogether eight women undergoing a chronic hemodialysis treatment were studied for five months. Four of them were in the premenopause and four in the postmenopause. Once a month levels of prolactin, FSH, LH, estradiol and progesterone were measured, Lüscher colour test was performed, from which the screamer index as an indicator of psychic stress was calculated and the levels of urea, creatinine and hematocrite were assessed to inform us of the adequacy of dialysis. We tested the hypothesis, whether there exists a correlation between psychic stress and the prolactin level in these women. For statistic evaluation we used linear regression under the use of dummy variables to intercept the interindividual differences in the Lüscher test. We found that the fluctuation of the prolactin level and of the value of the screamer index of the Lüscher test are parallel (p less than 0.05). These results bear witness of a participation of psychic stress in the onset and level of hyperprolactinemia in women being in a chronic hemodialysis treatment.

[Results of the Cattell 16 P.F. questionnaire in pregnancy].

Cattell's 16 P. F. Personality Questionnaire was used to examine 31 randomly selected pregnant women with a mean age of 24.9 years. The first and second examinations were performed at weeks 14-16 and 36-38 of pregnancy, respectively. While 13 women were examined using A form first, followed by B form, the sequence was reversed in 18 women. Results were evaluated by means of vector regression analysis. The difference in results of examination at the beginning and end of pregnancy was zero, however, there was a significant difference (p < 0.01) in the group with the AB sequence of questionnaire completion from that with the BA sequence. The difference applies especially to factors A, F, I and Q2.

When is it possible to meet the wish of a woman to terminate her labour by caesarean section?

The authors have evaluated 311 answers of Czech obstetricians to the questionnaire concerning their attitude towards pregnant women that want to terminate their labour by caesarean section. The questionnaire contained six model situations: (1) normal delivery; (2) previous caesarean section; (3) breech presentation; (4) previous delivery of an injured baby; (5) protracted labour and (6) caesarean section apparently indicated by another obstetrician. In each situation the obstetrician was encouraged to choose from five different answers. The results have revealed the lack of unanimity in some cases, 3, 4 and 5. Here we feel that the obstetrician should meet the wish of the woman. However, in those situations that are clear from the medical point of view and where there is no professional indication for a caesarean section, it is in accordance with the Czechoslovak rule of law not possible to meet the wish of the woman.

Cesarean Section

Hypnocontraception effectivity tested.

The effectivity of hypnocontraception was tested in 23 women selected as the most suitable ones from 86 registered interested women. The method was based on experience of the Milanese authors, especially Professor Marchesan. The hypnocontraception was performed in eight sessions, the temporary sterility was suggested for six months with a possibility of prolongation for another six months. In the course of the study 14 out of the 23 treated women became pregnant, seven of them in the course of the first two months and twelve of them in the course of the first six months. The remaining 9 women did not get pregnant. Out of them six terminated the hypnocontraception after six months, one after 12 months and four continue up to now (7-22 months). Consequently, the effectivity of the procedure applied in this study has been shown as not sufficient to recommend its introduction as a current means of contraception in the general population.

[FSH, LH, prolactin, estradiol and progesterone after burn injuries in premenopausal and postmenopausal women].

The authors examined 49 women who suffered severe burns and had to be hospitalized. Thirteen of these women were postmenopausal, 35 premenopausal and in one women, incipient pregnancy was revealed. In 14 women repeatedly FSH, LH oestradiol, progesterone and prolactin were assessed in serum by RIA. The group of premenopausal and postmenopausal women did not differ as to the levels of oestradiol, progesterone and prolactin. The gonadotropin levels in postmenopausal women were somewhat higher but the difference was significant only in FSH. The mean levels of the investigated hormones in premenopausal and postmenopausal women were: FSH 7.98 and 21.56 mIU/ml resp. oestradiol 151.88 and 183 micrograms/ml) resp. These differences cannot be explained by possible differences in the extent of burns or the time when the serum sample was taken. The authors interpret the results to the effect that the adrenals and steroid hormones produced by them resp. have a central position in the dysfunction of the pituitary-ovarian axis. These hormones influence by a backfeed mechanism the secretion of pituitary gonadotropins. This is consistent with data in the literature dealing with burns in men.

[Disorders of the menstrual cycle in women after entering military training. IV. FSH, LH, prolactin, estradiol, progesterone and cortisol in the first week after beginning training].

The authors investigated the hormonal levels of FSH, LH, oestradiol, cortisol and progesterone in women after entering a military training contre. The specimens were collected on the fourth day after admission to the Centre and the results correlated with later "separation" disorders of the menstrual cycle in these women. The authors did not find differences in the mean levels of any hormone in normally regularly menstruating women and in women with disorders of the cycle. They found, however, a significantly higher variance of LH and oestradiol levels in women with disorders of the cycle. When investigating the correlation of different hormone levels, they found a significant positive correlation of FSH:LH, LH:oestradiol, FSH:oestradiol, prolactin:cortisol, and an indirect correlation of LH:cortisol.

Estradiol

[An empiric study of postpartum states using Lüscher's color test].

In order to map the mental state in the early puerperium the authors gave to a group of 100 women for five days after delivery Lüscher's colour test. Based on the assembled data a tentative average norm for the post-partum state was elaborated: 54312067 (i.e. violet, yellow, red, blue, green, grey, brown, black). Deviations from the norm were found in the following groups of parturient women: Caesarean sections 31540276 (p less than 0.05, n = 9), single and divorced 34512067 (NS), duration of the first stage of labour more than 360 mins. 51423067 (p less than 0.01, n = 29) or less than 180 mins. 34512067 (p less than 0.001, n = 22), more than one delivery 34512067 (p less than 0.01, n = 52) and university graduates 43152607 (p less than 0.001, n = 31). The above norm could be used after verification on a more numerous population group as a simple diagnostic means of unsatisfactory experience during the early puerperium.

Conservative nonsurgical treatment of early detected tubal pregnancies with intra-amniotic methotrexate.

Four cases of early detected tubal pregnancies were managed conservatively by aspiration and administration of 20 mg methotrexate into the amniotic sac during laparoscopy. All three women for whom hysterosalpingograms are available had patent tubes on the side of the former ectopic pregnancy six months after treatment. Spontaneous menstruation occurred within six weeks after methotrexate application in all cases.
