Pharmacological control of intimal hyperplasia in small diameter polytetrafluoroethylene grafts. An experimental study.

OBJECTIVE: The efficacy of a postoperative treatment with Low molecular weight heparin (LMWH) vs Ticlopidine in controlling early thrombosis, intimal hyperplasia and growth of true endothelial cells in small prosthetic expanded polytetrafluoroethylene (ePTFE) grafts (4 mm) interposed in the carotid artery of sheep has been evaluated. EXPERIMENTAL DESIGN: Thirty animals were randomized into three different groups: control group (CTRL); ticlopidine hydrochloride (TICL) 250 mg/b.d. by month for 4 weeks from day 1; LMWH 3,085 IU AXa s.c. preoperatively and once a day for the same period. RESULTS: Complete thrombosis of the graft occurred in 7 sheep in the CTRL group and 5 in the TICL group while 2 partial thrombosis were observed in the LMWH group (n.s.). In all the evaluable cases, hyperplasia was observed in both anastomotic areas and did not involve the middle portion of the graft. The mean +/- SD intimal thickness was 603 +/- 20 micron in the CTRL group, 356 +/- 10 in the TICL group and 152 +/- 17 in the LMWH group (p < 0.001) compared to the 60 +/- 12 of the normal intima. True endothlial cells were found mainly in the LMWH group close to the arterial anastomosis. CONCLUSIONS: The postoperative use of LMWH seems to inhibit intimal hyperplasia, with interesting results also on patency and cellular coverage. Further studies are necessary to support this promising trend.

[Prevention of vascular intimal hyperplasia in small caliber prostheses. Preliminary results].

Intimal hyperplasia is one of the main risk factors for the patency of small diameter bypass grafts. The standard unfractioned heparin (UH) is able to control this phenomenon but the clinical use is not fit for long term treatment; on the other hand the antiplatelet drugs have an anti-thrombotic effect but they seem to be unable to control intimal hyperplasia. Low molecular weight heparins (LMWH) have an anti-thrombotic effect superimposable to that of UH with minimal side-effects and might inhibit intimal hyperplasia too. Based on these criteria, we carried out an experimental study on sheep with the aim of evaluating the efficacy of postoperative treatment with LMWH versus an anti-platelet drug in controlling intimal hyperplasia and growth of true endothelial cells in small prosthetic ePTFE grafts (4 mm) interposed in the carotid artery. At the operation, 30 sheep were randomly located in 3 groups: A = control group, no treatment; B = Ticlopidine hydrochloride 250 mg/bid by mouth for 4 weeks; C = LMWH 3.075 IU AXa (0.3 ml) sc preoperatively and then once a day for the same period. Complete thrombosis of the graft occurred in 7 sheep of group A, 5 in B and 2 partial in group C. The intimal hyperplasia was moderate-severe in group A, mild-moderate in group B and no-mild in group C. True endothelial cells were found mainly in the LMWH group; in the other groups and in other portions of the grafts the cellular coverage was accomplished almost completely by fibroblasts. The study is still in progress with 6 further sheep treated with LMWH.

Trophic response and morphological changes in pancreas of caerulein treated rats: dose and time dependent effects.

The present study was performed to determine the effect of the duration of chronic caerulein administration given at different doses, on the rat pancreas. Four groups of rats, one treated with 0.9% NaCl (control) and the others with caerulein 2, 5 and 10 micrograms/Kg twice a day i.p. were used. After a treatment period of 15, 30 and 60 days, 6 rats from each group were anesthetized, the pancreas was removed, and growth and composition of pancreatic tissue were determined. Small samples were taken for histological examination. Caerulein induced pancreatic hyperplasia and hypertrophy. The dose of caerulein used and the length of the treatment did not significantly modify the trophic effect. Focal perivascular and periductular lymphomonocytic infiltrates associated with cellular abnormalities were seen at 30 and 60 days. The results suggest that 1) the trophic effect of caerulein is not dose-and-time dependent and 2) morphological abnormalities can appear during long term treatment with CCK analogous.

Comparative study of percutaneous and peroperative fine-needle aspirations in the diagnosis of pancreatic cancer.

A consecutive series of 51 patients with suspected pancreatic cancer were studied in order to evaluate the contribution of cytologic data in establishing a definitive diagnosis of the tumor. A percutaneous ultrasonically-guided, and a peroperative fine-needle, aspiration were performed in 26 and 29 patients, respectively; 4 cases underwent both methods. Final histologic diagnosis in surgical or autopsy specimens showed cancer of the pancreas in 41 cases and chronic pancreatitis in 10. Cytology was successful in 33/41 cancer patients (80.5%): percutaneous aspiration was positive in 15/20 (75%) cases, and peroperative aspiration in 18/25 (72%). The difference between the two methods was not significant. Neither technique gave false positives in chronic pancreatitis. In none of the 51 patients was relevant morbidity reported. These cytological studies are at present accurate and mostly safer than histologic examinations. In particular ultrasound-guided percutaneous aspiration should be given precedence, since it provides preoperative confirmation of the tumor, thus obviating the need for exploratory laparotomy.

[Anatomicopathological findings relating to fungal infections: considerations on their increase and microbiological findings].

2,366 consecutive autopsies carried out at the Institute of Anatomy and Histopathology of the University of Genova over the last 10 years are re-examined. 33 cases of visceral mycosis were found (a frequency of 1.4%). The following factors were considered: number and location of fungal infection, morphological characteristics of mycetes responsible, host tissue reaction, relationship with any other disease manifestation or predisposing factors. It is emphasized that these infections are increasing in frequency and severity. Clinics should therefore always consider possible fungal causes in the differential diagnosis of infections.

[Intraoperative cyto-histologic analysis in exocrine carcinoma of the pancreas; relation to choice of therapy].

The Authors analyze the various cyto-histology techniques of investigation during surgical operation in the exocrine cancer of the pancreas, both on the results reported in literature and on their experiences based on a sample of 44 patients. They Authors, at the conclusion, single out the preferential technique in the citology by fine needle biopsy, for its reliability, practicality and lack of any complication.

Fundus angiography with fluorescein-labelled peptide fraction from bovine factor VIII: correlation with histologic findings.

A peptide fraction (VUEFFE) obtained from bovine factor VIII, with a high affinity for vessel endothelium, was bound to fluorescein and used for angiography in rabbits. Several eyes had undergone laser photocoagulation on the retina. The location of the dye was studied histologically with fluorescence microscopy on the enucleated, freeze-dried eyes. The angiographic and histologic findings were compared with findings obtained using sodium fluorescein. Fluorescein-labelled VUEFFE gave a longer lasting fluorescence of the retinal vessels with histologic evidence of dye deposition on the vessel wall. It also showed marked affinity for the photocoagulated tissues, giving intense and prolonged staining of the laser burns. Possible clinical applications are discussed.

[Non ossifying fibroma with vertebral localization. Clinical case].

A case of non ossifying fibroma of a vertebral body causing a severe spinal cord compression is reported. A review of the literature shows that in the group of benign neoplasms of bone, non ossifying fibroma is uncommon and that its vertebral location is quite rare. The Authors discuss about the clinical, radiological and pathological features of these lesions which may be diagnostically confusing.

Fine-needle aspiration cytodiagnosis: a simple and safe procedure for cancer of the pancreas.

One hundred fine-needle aspirations (45 percutaneous ultrasonically-guided and 55 intraoperative) were performed in 82 patients suspected of having pancreatic cancer, in order to evaluate the contribution of cytologic data to definitive diagnosis of this tumor. The overall success rate of both cytologic methods was 90.6 p. 100 in our 64 patients who were histologically proved to have pancreatic cancer. Sensitivity was 77.4 p. 100 with percutaneous and 75.5 p. 100 with intraoperative aspirations. In all 18 cases with the final diagnosis of chronic pancreatitis results of both techniques were negative. Neither relevant morbidity nor mortality were recorded in any of the 82 patients. These cytologic studies are at present reliable, rapid, highly cost-effective (especially percutaneous aspiration) and most important, safer than histologic examinations. We believe that fine-needle aspiration cytology has gained a primary role in the diagnostic process of pancreatic carcinoma. Therefore the old view of considering examination of cytologic material only as a screening procedure should be abandoned.
