[Growth and detection of aerobic bacterial pathogens in BacT/Alert FA, BacT/Alert SA, Bactec Plus Aerobic/F and Bactec Standard Aerobic/F].

The article compares the ability of detection and detection times (TTD) of aerobic bacteria in the BacT/Alert FA, BacT/Alert SA, Bactec Plus Aerobic/F and Bactec Standard Aerobic/F bottles. Compared bottles were inoculated at the same time with the identical suspension of bacterial strain. All bottles detected bacterial pathogens even in the case of very low number of inoculated bacteria. The differences of TTD were detected between the bottles of compared hemocultivation systems. Bactec Plus Aerobic/F system is faster in the detection of the members of family Enterobacteriaceae at 1-3 hours, BacT/Aert FA system is faster in the detection of coagulase-negative staphylococci. Most difference was detected at Pseudomanas stutzeri, where the detection in BacT/Alert FA bottles was at 15 hours faster in opposite of Bactec Plus Aerobic/F bottles. In the bottles without sorbent Staphylococcus aureus and Streptococcus sanqui were detected faster in the system Bactec Standard Aerobic/F. The bottles BacT/Alert SA detected faster skin corynebacteria. Detected differences have not a practical importance for the blood stream infection diagnostics.

[Quantitative microbiological diagnostic method of intravascular catheters infection using the BacT/Alert SA bottles].

OBJECTIVE: The study describes a method of quantitatively examining vascular catheters with the aid of hemocultural devices BacT/Alert. MATERIAL AND METHODS: The amount of bacteria present on the surface of the vascular catheters was determined based on the time to detection (TTD) in hemocultural vials inoculated with bacterial suspension stirred off the catheter surface. RESULTS: A total number of 348 vascular catheters was examined with this new method. Positive cultures were detected in 26 (7.5 %) catheters. Accordance to the results of microbiological catheter examination with clinical condition of the patient was found in 93.1 %. False positivity occurred in 1.1 % (4) and false negativity in 5.7 % (20) of cases. On the basis of clinical symptoms 42 cases of catheter sepsis were diagnosed out of which 22 (52.4 %) positive microbiological quantitative examinations of vascular catheters were established. The most frequent pathogen was coagulase negative staphylococci. CONCLUSIONS: The new method, when compared with present methods, is simpler, faster and able to detect the most frequent agent of catheter infection.

New safety assurance for biological skin covers.

The described system of safety assurance of cryopreserved allogeneic and xenogeneic dermoepidermal transplants comprises serological examination of deceased tissue donors, long-term storage of sera of all donors, microbiological control of prepared allogeneic and xenogeneic dermoepidermal grafts, labelling of released tissue grafts and monitoring of temperatures inside the mechanical freezer (-80 degrees C). From a total number of 76 donors from whom tissues were collected for transplantation during 1999-2001, tissues were discarded in two instances. One because of a positive HBsAg test, the others second one because of presence of anti-HTLV antibodies. In xenogeneic dermoepidermal grafts, out of a total number of 1,203 grafts prepared during the same period, 84 (6.9%) were discarded because of the presence of pathogeneic or potentially pathogeneic microbes. The system of labelling released grafts makes unequivocal identification of the pathway from recipient to donor possible, while at the same time respecting the anonymous character of the donor's data. In xenogeneic grafts it ensures the identification of the appropriate batch. Storage of cryopreserved biological skin covers at a temperature of -80 degrees C in low temperature cabinets with emergency back-up cooling with liquid nitrogen and a supplementary source of electric power, proved very useful. The system responds to the gradual implementation of the principles of Quality Management System ISO 9000 and Good Manufacturing Practice into the activities of tissue banks. Further tightening of the demands for the safety of allo- and xeno-transplantation is foreseen in conjunction with the occurrence of transmissible spongiform encephalopathy and porcine retroviruses.

[Urinary tract infections--still a real problem].

By analyzing the standard works in the scientific world literature, it was possible to construct a contemporary view on the etiology and antibiotic therapy of some urinary tract infections. It can be stated, that the most effective remedies are cotrimoxazol and fluoroquinolones. In contrary to beta-lactam antibiotics, cotrimoxazole and fluoroquinolones work with a rapid bactericidal effect, they have long elimination half-times and better sterilization capacity in the periurethral region, and it use brings minimal risk of recurrence of the infection. This facts are compared with results of bacteriological investigation of urine, which were done in the microbiological laboratories of University Hospital Hradec Králové, Czech Republic, in 1998. The most frequent origins of urinary tract infections are Escherichia coli and Enterococcus spp. Stains. The are keeping high susceptibility to contrimoxazole, furantoin and oxoline acid. The other Gram negative rods isolated from hospitalised patients are highly resistant to most of antimicrobial agents included fluorochinolons, which resistance culminates to 50%. The widest spectrum of pathogens and the highest resistance was found in the hospitalised patients of the university hospital. High percentages of resistant strains was also in patients from the district hospitals. The results are discussed.

Selective loss of BoLA class I determinants from the lymphocyte surface after acid treatment.

Acid treatment of bovine lymphocytes by a buffered solution of 0.263 m citric acid and 0.123 M Na2HPO4 at pH = 3.0, originally described for human and murine lymphocytes, selectively eliminated the antigenicity of MHC (BoLA) class I determinants also from bovine lymphocytes. The viability of acid-treated cell suspension was higher than 90%. The reactivity of acid-treated lymphocytes with BoLA class I typing alloantisera was lost in microcytotoxicity test, while their reactions with cross-reactive anti-HLA class II, anti-BoCD2 and BoCD5 monoclonal antibodies, and with antisera detecting two non-MHC lymphocyte alloantigenic specificities (BoLY w1 and R') remained unchanged in the microcytotoxicity and/or indirect immunofluorescence tests. The results thus show that this approach of modulating cell surface expression of MHC class I determinants may be used in cattle.

[Initial results of an in vitro fertilization and embryo transfer program in a private health institution].

Between October 1994 and the end of February 1995 115 patients were included in our programme. In these patients we monitored 141 cycles, performed 128 punctures and 100 ET. In 14 instances (9.2%) the cycle was abolished. By puncture of 1468 follicles 1370 oocytes were obtained which corresponded to a recovery rate of 93.3%. On average by aspiration 11.9 oocytes per patient were obtained. From the total number of acquired oocytes 54.6% were fertilized and 43.5% divided. Of 596 embryos 290 were transferred and the remainder was vitrified. A total of 17 clinical pregnancies were achieved, incl. 5 which terminated by abortion, the remainder proceeds. The authors are aware that they present a relatively small group during a short period of time, and thus the first results of the centre cannot be presented as a current standard. Despite this the results permit some optimism for the future and indicate a certain amount of standardization of the authors' clinical and laboratory work.

[Adjuvant effects of glucan on antibody formation in swine].

The effect of glucan, a yeast product isolated from Saccharomyces cerevisiae, on the antibody production to the bovine serum albumin and tetanus toxoid was investigated in miniature pigs. A significant stimulation of the immune response to both antigens differing in their thymus dependency was demonstrated during three weeks after the glucan application, by ELISA, in comparison to the untreated controls.

Diagnostic haemoperfusion.

Haemoperfusion and standard blood culture techniques were compared in the recovery of pathogenic Escherichia coli and Staphylococcus aureus strains circulating in the blood of rabbits with experimental bacteriaemia. On the average, diagnostic haemoperfusion gave positive isolations in 93.3%, blood cultures in 51.7%. This difference in favour of haemoperfusion tended to increase with the decreasing intensity of bacteriaemia. Clinically, haemoperfusion helped to establish correct diagnosis in a 46-year-old female patient with diabetic nephropathy and absceding staphylococcal pyelonephritis, whose blood and urine cultures were repeatedly negative. In this patient the authors succeeded in isolating the cause of septicaemia with the aid of the Czechoslovak haemoperfusion column Hemasorb 400 C. This study implies that diagnostic haemoperfusion used for the isolation of pathogens from blood is more reliable and less time-consuming than routine culture techniques.

[Ultrasonography of echinococcosis, toxocariasis and cysticercosis of the liver].

The diagnosis of tissue parasitoses has remained complicated until the present time for variability of clinical picture. Their incidence been increasing due to aimed and accidental findings of foci in parenchymal organs employing new methods of visualization such as computer tomography and especially ultrasonography whose advantage is a good detection rate, availability and a possibility to follow the dynamics of parasitosis development. The authors present a survey of ultra-sonographic findings of the liver in cases of tissue parasitoses occurring in Czechoslovakia, while using their own image documentation. If a focal affection in the liver is established the differential diagnosis considerations should also take into account a tissue parasitosis, whose ultrasound picture may be nonspecific.
