Microsatellite analysis at 10q25-q26 in Sardinian patients with sporadic endometrial carcinoma: identification of specification patterns of genetic alteration.

BACKGROUND: Loss of heterozygosity (LOH) at chromosome 10q25-q26 has been reported previously in endometrial carcinoma (EC), suggesting the presence of tumor suppressor gene(s). Nevertheless, frequency of genome-wide microsatellite instability (MSI) has been demonstrated higher in EC than in other common malignancy, mostly due to defective DNA mismatch repair. The authors further evaluated the role of the chromosome 10q25-q26 in endometrial tumorigenesis as well as the clinical significance of any observed genetic alteration in sporadic EC. METHODS: Paired normal and tumor samples from 94 Sardinian patients with sporadic EC at various stages of disease were screened by polymerase chain reaction (PCR)-based microsatellite analysis. Genomic DNA was isolated from paraffin embedded tissues and amplified by PCR using microsatellite markers spanning approximately 14 cM at 10q25-q26. Microsatellite instability was studied at four loci mapping to different chromosomal locations. RESULTS: Thirty-two (34%) EC patients were found negative for genetic alterations within the 10q25-q26 region. Among the remaining 62 (66%) EC cases, the authors identified 1) a minimum consensus region of LOH of approximately 1 cM, between D10S610 and D10S542 markers; and 2) a subset of tumors with prevalence of instability at 10q25-q26 (10qMI+), as expression of the presence of a MSI+ phenotype. CONCLUSIONS: The authors' data establish the existence of significant correlations between disease stages and 10qMI+ (with or without MSI+). However, longer follow-up and additional studies are required to define the clinical significance of these findings as prognostic factors. Moreover, the minimum region of LOH at 10q25-q26 will be further analyzed for identifying the putative tumor suppressor gene involved in EC pathogenesis.

Cardiac dysfunction in acromegaly: evidence by pulsed wave tissue Doppler imaging.

OBJECTIVE: To verify whether the accuracy of data on myocardial function provided by pulsed-wave tissue Doppler imaging (PWTDI), a new echocardiographic application that allows quantitative measurements of myocardial wall velocities, could help towards a better understanding of the natural history of acromegalic cardiomyopathy. DESIGN: Eighteen patients with active acromegaly (ten men and eight women; mean age 48.0+/-15.0 years) with no other detectable cause of heart disease underwent PWTDI. Thirteen healthy individuals matched for age and body mass index acted as a control group. METHODS: Ejection fraction (EF), transmitral early/late diastolic velocity (E/A) ratio and isovolumic relaxation time (IVRT) were measured by conventional echocardiography; systolic peak (Sv) and early (Ev) and late (Av) diastolic peak velocities, Ev/Av ratio and regional IVRT (IVRTs) were obtained by PWTDI. RESULTS: All patients showed appreciably abnormal left ventricular global diastolic function represented by prolongation of the IVRT (P<0.001). Using PWTDI we found a prolongation of IVRTs and inversion of the Ev/Av ratio. In addition, the Ev/Av ratio proved to be significantly negatively correlated with IVRT; this correlation was not present in the case of the E/A ratio. Furthermore, a decrease in Sv was detected in the basal segment of the lateral wall (P<0.01), which had the greatest degree of diastolic dysfunction. CONCLUSIONS: PWTDI confirmed the acknowledged diastolic dysfunction that accompanies acromegalic cardiomyopathy and highlighted the greater sensitivity of regional PWTDI with respect to global Doppler diastolic indexes. Furthermore, by revealing an impairment of regional systolic function in presence of a normal EF, the findings with PWTDI contradicted the largely accepted theory that systolic function remains normal for several years in patients affected by acromegalic cardiomyopathy.

Glucose-6-phosphate dehydrogenase deficiency and lung cancer: a hospital based case-control study.

A hospital based case-control study was conducted to test the hypothesis of a lower lung cancer risk in G6PD-deficient subjects. Cases were 156 male patients with lung cancer, admitted to "Binaghi" Hospital, Local Health Unit (USL) 20, Cagliari (Italy), between January 1984 and November 1986. Controls were 235 male patients, admitted to the same hospital in the same time period, for diseases other than cancer (all types) and hemolytic anemia. No decrease of the lung cancer risk was found in G6PD-deficient subjects. This result, in line with recent reports in the literature, suggests that the genetic condition of G6PD deficiency does not provide significant protection against the development of lung cancer in humans.

Mechanisms of PCBs mixture toxicity on isolated rat hepatocytes.

Exposure of isolated hepatocytes to a polychlorinated biphenyl mixture induced a rapid loss of cell viability. The effect was not dose-dependent. The biochemical effects in the cellular toxicity did not involve glutathione content, protein sulfhydryl groups and lipid peroxidation. A transient increase in cytosolic Ca2+ was observed after exposing the hepatocytes to the polychlorinated biphenyl mixture. Our findings indicate that polychlorinated biphenyls are able to kill hepatocytes and suggest that elevation of cytosolic Ca2+ concentration could be responsable of the toxicity.

Blood lead levels in children of S.W. Sardinia.

The purpose of the present study was to make a preliminary evaluation of blood lead levels in school-aged children from three towns of southwestern Sardinia, Portoscuso, S. Antioco and Calasetta, the first of which is very close to a large industrial complex. Blood lead concentration was determined in venous blood samples by graphite furnace atomic absorption spectrophotometry. The survey revealed that children residing in the town of Portoscuso had higher mean blood lead levels (12.7 micrograms/dl) than children of the same age residing in S. Antioco and Calasetta (8.3 and 8.4 micrograms/dl respectively) and that levels were slightly higher in males than in females. This difference is suggested to be a result of the greater fallout of industrial pollutants in the area of Portoscuso.

[Isoflurane pollution in operating rooms].

In three operating rooms of a Sardinian hospital, the Authors have measured the air levels of isoflurane using static samplers. Anaesthetic mean daily concentrations ranged from 3.5 to 41.5 ppm in room A, from 4.1 to 24.4 ppm in room B, from 9.5 to 30.4 ppm in room C above all depending on the length and the number of surgical operations. The Authors emphasize the need of suitable prevention measures.

[Determination of the pneumoconiosis hazard in the cork industry].

The AA. have carried out an epidemiological investigation in order to control the prevalence of chronic respiratory diseases on a group of 385 people working in several cork industries in North Sardinia. At the same time an environmental investigation has been carried out. Standard questionnaire (CECA questionnaire) for chronic bronchitis and pulmonary emphysema, standard chest X-ray film, spyrographic investigation, ECG and sputum analysis for acid resistent bacteria have been used for epidemiological investigation. Environmental dust concentration and individual dust exposure, environmental concentration of sulphur oxides, nitrogen oxides, carbon mono- and dioxide, ammonia nad microclimatic conditions have been studied. Examination of medical data shows only a low degree of respiratory pathology in contrast with results of other authors, probably because of different methods used for epidemiological investigation and data interpretation and because of different environmental and working conditions.

[Environmental dust in a plant for the production of polyvinyl chloride].

The results of an investigation on environmental dust concentration and individual dust exposure in a drying and sacking section of an "emulsion" and "suspension" PVC plant are reported; data on environmental concentration and individual dust exposure have been obtained by gravimetric techniques using similar personal and static dust samplers. Morphology and particle size distribution have been studied by electron microscopy on airborne dust specimens. Morphological analysis showed remarkable differences concerning shape and size between particles sampled in the "suspension" polymer and "emulsion" polymer sacking sections. Remarkably high mean values concerning dustness have been observed: 1.30 mg/m3 (min. 0.15; max. 18.4 mg/m3) for environmental concentration and 5.39 mg/m3 (min. 0.28; max. 45.6 mg/m3) for individual exposure.

[Respiratory symptoms and pulmonary function in the Italian primary aluminum industry].

The paper reports the results of a longitudinal study of 1478 workers in the primary aluminium industry in Italy (aluminium reduction with the pre-baked anode process) covering the prevalence and incidence of chronic obstructive lung disease, chronic bronchitis and bronchial asthma, and analysis of lung function deterioration over time (annual decline in FEV1) in relation to occupational exposure and individual non-occupational features. The incidence of respiratory symptoms of chronic cough and expectoration was significantly higher among electrolytic shop workers, in whom the annual decline in FEV1 was also significantly greater. In this group the incidence and prevalence of asthmatic manifestations was particularly high compared to casting and workshop workers. Asthmatic symptoms showed a short latency period related to exposure in potrooms and seems to be characterized by a marked deterioration in lung function over time even after withdrawal from exposure.

[Mortality in workers of a primary aluminum foundry in Portovesme in Sardinia].

The standard mortality rates (SMRs) were calculated for 1148 workers of a primary aluminium plant in Portovesme, Sardinia, hired between 1971, when production started, and 1980. Status (living or decreased) was ascertained as at 31 December 1990 and the relationship between observed and expected deaths with respective 95% confidence limits were calculated on the basis of age-specific regional rates for each calendar year of the follow-up. The SMR for all causes was 81 with confidence limits between 61 and 108 based on a total of 48 deaths. Mortality due to malignant neoplasms did not differ from the expected rate. The observed deaths due to lung cancer were decidedly less than the expected number (3 observed versus 4.7 expected). A significant excess of cancer of the pancreas was observed with special reference to anode production, based, however, on only 3 observed cases against 0.8 expected. In the absence of a more precise definition of the causes of death, of the environmental exposure levels and of the non-occupational confounding factors, and considering the young age of the cohort under study, it is at present doubtful whether the excess of cancer of the pancreas can be associated with work in the primary aluminium industry. The results should therefore be taken as preliminary, indicating that further studies are required.

[Effects of the body in personal sampling of noise].

In acoustic free fields the human body changes the energy distribution surrounding it, mainly under narrow band or pure tone noise conditions. Hence, sound levels measured close to the body should be intrinsically incorrect if performed via personal sampling. An experiment was carried out to verify whether this statement is still valid in a diffuse field, such as occurs in industrial workplaces. Noise measurements were made in diffuse field laboratory conditions without the presence of a person (steady state) and were repeated close to the ear of a person (perturbed state). The measurements were carried out with integrating precision sound level meters and also with personal noise dose meters. The trials were repeated in an industrial environment. The states 1/3 octave band levels were matched, as also were the equivalent continuous levels. These findings show that in diffuse fields the human body does not significantly affect equivalent continuous level measurements performed near the body. The mean differences between equivalent continuous levels measured by sound level meter were less than 0.3 dBA, and ranged from -0.6 to 0 dBA between levels measured by sound level meter and by personal noise dose meter. The results of the trial performed in the plant showed closer differences between steady and perturbed states and between sound level meter and personal noise dose meter measurements.

[Polychlorobiphenyl content in adipose tissue of subjects not exposed professionally].

Polychlorinated biphenyl (PCB) levels were determined in 52 samples of adipose tissue removed during surgery from patients residing in a central region of Sardinia who had no previous occupational exposure. The average PCB value of the whole group was 0.72 microgram/g (range 0.11-2.81 micrograms/g); in males the average value (means = 0.97 microgram/g) was significantly higher than in females (means = 0.60 microgram/g). A positive correlation was observed between age and PCB levels in adipose tissue. This analysis could be usefully performed in occupationally exposed subjects so as to better assess the exposure level.
