Soft contact lens wear in Hong Kong-Chinese: predicting success.

The tear stability and tear volume of 55 asymptomatic Hong Kong-Chinese were assessed before soft contact lens (Hydron Zero 6) were and repeated on three visits over 28 weeks of lens wear. Soft lens wear caused a significant initial transient decrease in the non-invasive tear break-up time (NITBUT), but no change in tear volume as assessed by the self-prepared cotton thread test (SP-CTT) and phenol red thread (PRT) test. After 28 weeks, the subjects were classified according to the length of time they could wear their lenses without dry eye related signs and symptoms. The NITBUT and PRT tests were unable to predict the success of soft contact lens wear. However, the SP-CTT gave a sensitivity and specificity of 88.5 and 52.4%, respectively, in the prediction of subjects most likely to be successful wearers. The positive and negative predictive values at the chosen cut-off point were 69.7 and 78.6%, respectively. Wetting values, that is, combinations of NITBUT and SP-CTT values or NITBUT and PRT values, did not predict success in soft contact lens wear.

Factors affecting the central corneal thickness of Hong Kong-Chinese.

PURPOSE: The aims of this study were to investigate the effect of age, intraocular pressure, refractive error (spherical equivalent) and corneal curvatures on the central corneal thickness of Hong Kong-Chinese. We also compared the central corneal thickness of Hong Kong-Chinese with those previously reported for other national/ethnic groups. METHODS: The central corneal thicknesses of 151 subjects of age 10-60 yrs were measured using an ultrasound pachometer. Intraocular pressure, refractive error and the corneal curvatures of these subjects were also recorded. RESULTS: The mean +/- SD central corneal thickness of the right eye and left eye were 575 +/- 32 microm and 574 +/- 31 microm respectively. No significant difference in central corneal thickness was found between the right and left eyes or between male and female subjects. Central corneal thickness decreased with increasing age but the effect appeared to be due to differences in female subjects only. The maximum decrease in central corneal thickness occurred in subjects in the age group of 10-25 yrs, and in this age group, central corneal thickness and age was significantly correlated in both male and female subjects. Intraocular pressure and central corneal thickness was significantly correlated. There was no correlation between central corneal thickness and refractive error or between central corneal thickness and corneal curvatures. CONCLUSIONS: Central corneal thickness decreased with increasing age but this appeared to be due to female subjects only. Central corneal thickness was significantly correlated with intraocular pressure, but not with refractive error or corneal curvatures. Our subjects also had significantly thicker corneas than those reported for Caucasian subjects.

Fast neutrons in prostatic adenocarcinomas: worldwide clinical experience.

Primary tumor control remains a major problem in the treatment of locally advanced prostate carcinoma. Clinical local failure rates approach 30-40% and may be significantly higher when results of prostatic biopsy or prostate-specific antigen (PSA) levels are considered. The low growth rate and cycling fraction of prostate adenocarcinoma suggest potential therapeutic advantage for the high linear energy transfer (LET) of neutrons. The Radiation Therapy Oncology Group (RTOG) performed a multi-institutional randomized trial (RTOG 77-04) comparing mixed beam (neutron plus photon) irradiation to conventional photon irradiation for the treatment of locally advanced prostate cancer. A subsequent trial by the Neutron Therapy Collaborative Working Group (NTCWG 85-23) compared pure neutron irradiation to standard photon irradiation. Both randomized trials demonstrate significant improvement in locoregional control with neutron irradiation compared to conventional photon irradiation in the treatment of locally advanced prostate carcinoma. To date, only the mixed beam trial has shown a significant survival benefit. Future analysis of the larger NTCWG trial at the 10-year point should confirm whether or not improved locoregional control translates into a survival advantage. These findings have significant implications for all local treatment strategies including dose-escalated conformal photon irradiation, prostate implantation, and neutron radiation. Given the large numbers of patients afflicted with this disease, a positive survival advantage for neutrons or mixed beam therapy would provide a strong incentive for the development of economically feasible clinical neutron facilities.

Evaluation of a self-prepared cotton thread test (SP-CTT) and the Phenol Red Thread (PRT) test.

Two cotton thread tests (a self-prepared cotton thread test (SP-CTT) and the Phenol Red Thread (PRT) test) were performed on 46 dry eye and 37 normal Hong Kong (HK)-Chinese, all subjects being diagnosed by ophthalmologists in a private clinic. The mean SP-CTT value for the dry eye subjects (mean +/- SD: 6.2 +/- 4.2 mm/15s) was significantly lower than that of the normal subjects (8.3 +/- 4.3 mm/15s). However, no significant difference in the mean PRT value was found between the two groups (dry eye subjects: 10.7 +/- 4.5 mm/15s; normal subjects; 11.4 +/- 4.5 mm/15s). The specificity and sensitivity of the SP-CTT were 78.4% and 45.7 when a cut-off value of < or = 4 mm was used, and 56.8% and 65.2% when the cut-off value was changed to < or = 6.5 mm. Our results showed that the specificities and sensitivities of both tests were poor when applied to these dry eye patients.

Disruption of the tear film by the application of small drops of saline and surfactant.

The effects of disrupting the tear film by placing 5 or 10 microl drops of saline and mild surfactant solution (SS) directly onto the tear films of 10 subjects were observed and are described here. Disruption of the tear film was only transient, with restoration of the film occurring within about two to three blinks for saline drops, and four to six blinks for SS drops. When a drop of SS was placed on a cornea, two types of black spots/areas may be observed: black areas apparently similar to those observed in the TBUT test, and black areas forming outside the border of the pattern of the spreading SS drop. In general, staining of the black areas formed in the fluorescein-stained tears did not occur even when blinking was inhibited for up to 13 s. Stained areas on the corneal surface did not have a tendency to form black spots first; the tear film over these areas did not appear to be less stable than on other areas of the corneal surface.

Soft contact lens care systems and corneal staining in Hong Kong-Chinese.

One hundred and ninety two patient files (for the year September 1995-September 1996) from the Contact Lens Clinic of The Department of Optometry & Radiography, The Hong Kong Polytechnic University were surveyed. A total of 117 patients mostly using three lens care systems (CIBA VISION AOSept, Allergan Oxysept 1-step and Bausch & Lomb Multipurpose solution (MPS)) were included in this retrospective study of the incidence of corneal staining in patients using different lens care systems. The incidence and severity of staining observed was significantly higher and worse respectively in patients using MPS than in patients using the peroxide systems. Corneal staining was observed in 32.3% of the patients using AOSept, in 17.8% of the patients using Oxysept 1-step, and in 70.7% of the patients using MPS system. However, these percentages did not take into account other factors (which we assumed to be affecting the three groups equally) which may have caused some of the corneal staining observed. Of the patients who had corneal staining: 31% (9/29) of the patients using MPS system changed to peroxide system, and 38% (11/29) of the patients were re-educated on proper lens care; 77% (14/18) of the patients using the peroxide systems did not use proper care procedures. The tear stability and volume of HK-Chinese have been reported to be low compared to those reported for Caucasians, which may render Hong Kong-Chinese soft contact lens wearers more prone to adverse reactions to preservatives and to rapid deposit formation. The majority of the patients with corneal staining did not complain of any ocular discomfort even when severe staining was observed.

Tear break-up time: clinical procedures and their effects.

Three successive (representative) values of fluorescein-instillation TBUT (tear break-up time) were obtained from a group of young Hong Kong (HK)-Chinese. The median TBUT1, TBUT2 and TBUT3 were 3.2, 3.8 and 4.5 sec, respectively. Wilcoxon signed-rank tests showed that TBUT1 was significantly different from TBUT2 and TBUT3, but there was no difference between TBUT2 and TBUT3. It is therefore important, when measuring the TBUT, to specify which TBUT value was taken as the representative TBUT. Fluorescein was reapplied about six min after the first application, and the TBUT value re-determined (TBUTR). The median TBUTR was not significantly different from TBUT1. The subjects were requested to return another day and TBUT values (TBUTW) were determined, with the subjects opening their eyes as wide as possible, such that the palpebral aperture size was increased. The mean +/- SD normal palpebral aperture size was 10.0 +/- 1.3 mm, and the mean +/- SD palpebral aperture size with the eyes wide open was 11.4 +/- 1.5 mm. The median TBUTW (3.6 sec) was significantly longer than the median TBUT1 (3.0 sec), but this difference appeared to be due to the extremely long TBUTW of one subject; no significant difference was found between the median TBUTW (3.4 sec) and TBUT1 (2.8 sec) when data from this subject were excluded. There was no significant correlation between changes in palpebral size and TBUT value.

Comparison of the performance of the Nidek NT-2000 noncontact tonometer with the Keeler Pulsair 2000 and the Goldmann applanation tonometer.

We compared the performance of a new noncontact tonometer (NCT), Nidek NT-2000, with the Keeler Pulsair 2000 and the Goldmann applanation tonometer (AT). Twenty-two subjects with intraocular pressure (IOP) ranging from 10 to 21 mmHg were recruited and their IOP measured using each technique on 2 different days. The NT-2000 produces repeatable IOP readings and is comparable to the Goldmann AT. There is a significant between-visits difference in IOP when using the Pulsair 2000. The reproducibility of IOP measurements made with the Pulsair 2000 is relatively poor when compared to the Goldmann AT and the NT-2000. IOP measurements made with the Goldmann AT show good repeatability and reproducibility when the mean of three IOP readings are used. When a single Goldmann reading is used variability is increased. There is no significant difference in the IOP readings made on the first visit between techniques.

Neutron radiotherapy for adenoid cystic carcinoma of minor salivary glands.

PURPOSE: To examine the efficacy of fast neutron radiotherapy for the treatment of patients with locally advanced, adenoid cystic carcinoma of minor salivary glands and to identify prognostic variables associated with local control, overall survival, and cause specific survival. METHODS AND MATERIALS: Eighty-four patients having adenoid cystic carcinoma of minor salivary glands were treated with fast neutron radiotherapy during the years 1985-1994. All patients had either unresectable disease or gross disease remaining after attempted surgical extirpation. Seventeen patients had previously received conventional radiotherapy and their subsequent treatment fields and doses for neutron radiotherapy were modified for critical sites (brainstem, spinal cord, brain). Although the median doses (tumor maximum and tumor minimum) only varied by < or = 10%, treatment portals were substantially smaller in these patients because of normal tissue complication considerations. Twelve patients (13%) had distant metastases at the time of treatment and were only treated palliatively with smaller treatment portals and lower median tumor doses (< or = 80% of the doses delivered to curatively treated patients). Seventy-two patients were treated with curative intent, with nine of these having recurrent tumors after prior full-dose radiotherapy. The median duration of follow-up at the time of analysis was 31.5 months (range 3-115). Sites of disease and number of patients treated per disease site were as follows: paranasal sinus-31; oral cavity-20; oropharynx-12; nasopharynx-11; trachea-6; and other sites in the head and neck-4. RESULTS: The 5-year actuarial local-regional tumor control rate for all patients treated with curative intent was 47%. Patients without involvement of the cavernous sinus, base of skull, or nasopharynx (51 patients) had a 5-year actuarial local-regional control rate of 59%, whereas local-regional control was significantly lower (15%) for patients with tumors involving these sites (p < 0.005). In the latter cases, normal tissue injury considerations precluded delivery of the full dose to the entire tumor. Patients with no history of prior radiotherapy (63 patients) had an actuarial local control rate of 57% at 5 years compared to 18% for those (9 patients) who had been previously irradiated with conventional photons (p = 0.018). Eliminating the dose-limiting factors of prior radiation therapy and/or high risk sites of involvement, the 5-year actuarial local-regional control rate for these 46 patients was 63%, with an actuarial cause specific survival rate of 79%. Lymph node status was a predictor of distant metastasis: 57% of node positive patients developed distant metastases by 5 years compared to 15% of patients with negative nodes (p < 0.0005), and patients who had nodal involvement developed distant metastases sooner than node negative patients (p < 0.0001). The 5-year actuarial overall survival and cause specific survival for the 72 patients treated with curative intent were 59% and 64%, respectively. CONCLUSIONS: Fast neutron radiotherapy offers high local-regional control and survival rates for patients with locally advanced, unresectable adenoid cystic carcinomas of minor salivary glands. It should be considered as initial primary treatment for these patients, as well as for other patients in whom surgical extirpation would cause considerable morbidity.

The incidence of depression in new users of beta-blockers and selected antihypertensives.

We studied the occurrence of depression in new users of propranolol (n = 704), other beta-blockers (n = 587), angiotensin-converting enzyme inhibitors (n = 976), calcium channel blockers (n = 742), and diuretics (n = 773) in the Harvard Community Health Plan population. The period of the study was from April 1988 to June 1991. All study subjects were followed for new or newly recurrent depression for up to 6 months after receiving their first study prescription. Case status was confirmed by blinded medical record review. We found 10 cases of depression that met DSM-III-R criteria ("major depression") and an additional 18 cases that had one or more symptoms consistent with depression ("minor depression"). Rates of major depression in users of beta-blockers and users of non-beta-blocker study drugs were 5.8 per 1000 person-years of exposure and 9.6 per 1000 person-years, respectively. None of the cases of major depression was propranolol associated. Rates of major or minor depression (combined) in users of beta-blockers and users of non-beta-blockers were 20.2 per 1000 person-years and 25.2 per 1000 person-years, respectively. The age- and sex-adjusted relative risk of major or minor depression associated with the use of beta-blockers compared to non-beta-blockers was 0.8 (95% CI, 0.3-1.9). The relative risk associated with propranolol compared to non-beta-blockers was also 0.8 (95% CI, 0.1-2.7). Therefore, depression occurred no more frequently in beta-blocker users than in other members of the study base.

Clinical trials of neutron radiotherapy in the United States.

The development of clinical neutron facilities in the 1980s, capable of delivering high energy neutrons spurred full scale phase III testing of neutron beam radiotherapy in a number of tumors including salivary gland, head and neck, prostate, and non small-cell lung cancer. The Radiation Therapy Oncology Group (RTOG) and the Medical Research Council (MRC) jointly sponsored a randomized trial for the treatment of advanced stage salivary gland tumors comparing neutron to conventional photon and/or electron radiotherapy. Although no improvement in survival was seen, the study demonstrated a striking and statistically significant difference in the local-regional control of unresectable salivary gland tumors (56 vs 17%), favoring neutron beam irradiation. Subsequent clinical trials of neutron beam irradiation were initiated by the Neutron Therapy Collaborative Working Group (NTCWG) sponsored by the National Cancer Institute (NCI). A phase III trial comparing neutron to photon radiotherapy for inoperable regional non-small cell lung cancer showed no overall improvement in survival. However, a statistically significant improvement in survival was observed in the subset of patients with squamous cell histology. The NTCWG trial comparing fast-neutron therapy versus conventional photon irradiation in the treatment of advanced squamous cell carcinomas of the head and neck showed a statistically significant improvement in initial complete response (70 vs 52%) favoring neutrons. However, subsequent failures erased any difference in ultimate local-regional control rates and survival curves were essentially the same in both arms. The randomized study of the NTCWG for locally advanced prostate cancer demonstrated a significant decrease in local-regional failure (11 vs 32%) at 5 years, favoring the neutron arm. Furthermore, biochemical measures of disease control also favored the neutron arm with prostate specific antigen (PSA) levels elevated in 17% of the neutron-treated patients compared to 45% of the photon-treated patients at 5 years. At the 5-year analysis, no significant difference in survival was observed between the two arms; however, longer follow-up is necessary to assess the ultimate impact of improved local-regional control on survival. An analysis of complications in this series revealed the importance of beam shaping and treatment planning capabilities in maintaining long-term sequelae following neutron irradiation at an acceptably low level.

Effect of fluorescein on the tear stability of Hong Kong-Chinese.

We used the noninvasive tear break-up time (NITBUT) technique to measure the tear stability of 24 asymptomatic Hong Kong (HK)-Chinese before and after instillation of fluorescein. One eye was used as the test eye while the fellow eye served as control. We compared the NITBUT of the test eyes pre- and postinstillation of fluorescein, and compared the NITBUT of the test eyes measured postinstillation of fluorescein with the NITBUT of the control eyes. After instillation of fluorescein, both longer and shorter NITBUT were recorded in the test eyes; similar results were observed for the control eyes (no fluorescein instilled) between the first and second measurement. The median NITBUT of the test eye pre- and postfluorescein instillation were 13.3 and 13.0 s, respectively. Fluorescein did not cause any significant change in the NITBUT of HK-Chinese and the instillation of fluorescein in one eye does not affect the NITBUT of the fellow eye. Fluorescein did not appear to affect the locations of tear break up (tear defects) observed using the NITBUT test. Tear defects are most frequent near the inferior periphery of the cornea.

Mechanisms of amiodarone-induced inhibition of Ca2+ current in isolated neonatal rabbit ventricular myocytes.

BACKGROUND: Amiodarone is an effective antiarrhythmic drug used to treat a wide variety of ventricular and supraventricular tachyarrhythmias. Recent voltage clamp studies indicate that amiodarone may possess a variety of antiarrhythmic effects. METHODS: The tight-seal, whole-cell voltage clamp technique was used to investigate the acute effects of amiodarone on L-type Ca2+ channel kinetics in isolated neonatal ventricular myocytes. RESULTS: We found that acute perfusion with 1 mumol/L amiodarone inhibited peak inward Ca2+ current by 39.1% (4.85 +/- 0.42 to 2.95 +/- 0.6 pA/pF, n = 10, p < 0.001) without changing the shape of the current-voltage relation. In addition, amiodarone shifted Ca2+ channel steady-state inactivation to more negative membrane potentials. In the absence of amiodarone, half inactivation of the Ca2+ current occurred at a membrane potential of -23.8 +/- 0.2 mV compared to -34.2 +/- 0.6 mV after addition of amiodarone (n = 11, p < 0.01). Furthermore, amiodarone significantly delayed Ca2+ current recovery from previous inactivation. CONCLUSIONS: These results provide evidence that amiodarone blocks voltage-dependent Ca2+ current in isolated neonatal rabbit ventricular myocytes by a variety of different mechanisms. The inhibitory effect of amiodarone on L-type Ca2+ current may represent an important facet of amiodarone's acute antiarrhythmic activity in the immature heart.
