Nonparametric regression applied to quantitative structure-activity relationships

Several nonparametric regressors have been applied to modeling quantitative structure-activity relationship (QSAR) data. The simplest regressor, the Nadaraya-Watson, was assessed in a genuine multivariate setting. Other regressors, the local linear and the shifted Nadaraya-Watson, were implemented within additive models--a computationally more expedient approach, better suited for low-density designs. Performances were benchmarked against the nonlinear method of smoothing splines. A linear reference point was provided by multilinear regression (MLR). Variable selection was explored using systematic combinations of different variables and combinations of principal components. For the data set examined, 47 inhibitors of dopamine beta-hydroxylase, the additive nonparametric regressors have greater predictive accuracy (as measured by the mean absolute error of the predictions or the Pearson correlation in cross-validation trails) than MLR. The use of principal components did not improve the performance of the nonparametric regressors over use of the original descriptors, since the original descriptors are not strongly correlated. It remains to be seen if the nonparametric regressors can be successfully coupled with better variable selection and dimensionality reduction in the context of high-dimensional QSARs.

[Lung disease induced by isotretinoin].

A young man without any past history of note had taken isotretinoin for disfiguring acne before the summer season. He presented with a severe bilateral pneumonia, associated with dyspnoea two months after the start of treatment. On the pulmonary radiography there was a bilateral ground glass appearance which was worse on the right. The elevated level of eosinophils (54% in 564,000 cells/ml) in the alveolar lavage lead to a diagnosis of allergic pneumonia. The rapidly favourable outcome following the cessation of the medication and with the addition of corticosteroids seemed to us a supplementary argument in favour of a diagnosis of eosinophilic pneumonia, due to isotretinoin which seemed the primary initiating factor.

[Is annual X-ray examination of the chest still justified in industrial medicine?].

Between 1975 and 1979, 137.526 annual X-ray examinations of the chest were performed among the employees of a large Paris administration. Fifty-four cases of tuberculosis, 28 cases of sarcoidosis, 10 cases of bronchial carcinoma and 26 miscellaneous chest diseases were detected. This study confirms the inadequacy of radiological examination for detecting chronic obstructive bronchopulmonary conditions. The small proportion of tuberculosis (0.39 p.1000) and bronchial carcinoma (0.07 p.1000) revealed by this method suggests that it should be reserved to high risk patients.

[Encephalopathy with pure nicotinic acid deficiency in alcoholic patients. Two cases, with anatomoclinical study in one (author's transl)].

Two chronic alcoholic patients developed severe encephalopathy while receiving high doses of parenteral thiamine and pyridoxine. Both presented with unusually marked hypertonia of the gegenhalten type, myoclonias and fluctuating mental impairment with memory disturbances and hallucinations. Later on, one patient went into stupor and coma and died ; the other recovered after parenteral niacin treatment. No skin lesions or diarrhoea were observed. Post-mortem examination of the first patient confirmed the presence of a pellagra-like encephalopathy characterized by widespread neuronal chromatolysis. There were no changes suggesting an associated alcoholic encephalopathy.

[Laparoscopy in the detection of hepatic metastasis of early bronchial cancer (100 laparoscopies)].

Staging 125 cases of primary lung cancer, peritoneoscopy for detection of liver metastasis was done each time it was possible. 100 peritoneoscopies were done, 92 were available. Tolerance was excellent. First investigation showed 16,3% liver metastasis. Higher frequency (35,3%) is found in oat cells carcinomas and lower in epidermoids (13,1%) and adenocarcinomas and/or undifferentiated (7,1%). Above all, in 7,6% of cases, peritoneoscopy alone was able to give diagnosis of liver metastasis not detected by other means. Peritoneoscopy shows no false positive, is safe and more effective than scans of biologic tests, can be repeated, and must become part of staging of lung cancers.

[Tomodensitometry in stereotaxic conditions (possibilities and limitations in the diagnosis and treatment of brain lesions)].

The stereotaxic approach of cerebral lesions for diagnostic (biopsies) and/or therapeutic purposes (interstitial radiotherapy) has been well documented. These techniques depend on an exact spatial definition of the tumoral volume, which is often difficult to obtain through conventional neuroradiology, stereoencephalography, rheography... The use of tomodensitometry in this context is obviously of upmost interest if, and only if, the scan itself is obtained in stereotaxic conditions. The authors present a stereotaxic system with which all the diagnostic data, including CT scan itself, are recorded and matched within the same stereotaxic space. This method achieves an exact mapping of lesions within the actual stereotaxic brain space. Hence any further stereotaxic gesture can be calculated and controlled with a suitable accuracy. The interest of these procedures in the management of brain lesions is demonstrated through an experience of 52 supra tentorial tumors. 13 cases of lesions under 20 mm in diameter are especially discussed.
