Contact lens disinfection.

Contact lens disinfection plays an important role in both patient education and office management. Several methods of disinfection are available for protecting the contact lens wearer from contamination by a variety of organisms, but no one system is best suited to all lenses, patients, or situations. Furthermore, these disinfection agents are only one component in an effective lens care system. Essential also is thorough lens cleaning and rinsing as well as preventing lens case contamination. The practitioner needs to educate each patient for optimal lens care compliance at home, and to instruct staff on maintaining safe trial lenses in-office.

The effect of rigid gas permeable lenses on corneal sensitivity.

Long term wear of hard (PMMA) contact lenses decreases corneal sensitivity. Clinicians have noted that in some patients who are refitted with rigid gas permeable (RGP) lenses, there is often an increase in lens awareness. To document this symptom, we monitored changes in corneal sensitivity of long term PMMA wearers who were refitted with RGP lenses. Corneal sensitivity measurements were conducted over a 6-month period. During this time, corneal touch threshold decreased to normal levels within a few weeks after refitting with RGP lenses. These results suggest an oxygen dependency factor in the control of corneal sensitivity accompanying contact lens wear.

Correction of residual astigmatism on an against-the-rule cornea of moderate toricity.

The use of prism ballasting and toric base curves are both commonly cited means of stabilizing lens orientation, although it is rare that both are necessary in the same lens. Base curve cylinder has an optical effect as well as a stabilizing effect, and this can be utilized to advantage under specific circumstances. This report presents a case which emphasizes the benefit of combining these two techniques of stabilization in a rigid lens design and also illustrates the utilization of the toric base curve in compensating residual astigmatism.

Bioptic low vision aid: a simple approach.

This report describes the design of a variable focus telescopic system for a low vision patient who wanted to be able to see at numerous fixed working distances. A simple and effective means of mounting the telescope onto an existing spectacle lens is described. The discussion presents an analysis of the optical principles used and a formula for determining the equivalent power of a nearpoint telescope.

Krukenberg's spindle and contact lens-induced edema.

Krukenberg's spindle, a pigment deposit on the corneal endothelium, is frequently an inconsequential finding, although it can be associated with pigmentary glaucoma. A case report is presented describing corneal changes occurring with a gas-permeable contact lens (Polycon) in the presence of a Krukenberg spindle.
