[Mycotic aneurysm of the extracranial internal carotid artery].

Extracranial internal carotid artery aneurysm is rare. They usually appear as an enlarging pulsatile neck mass with associated tenderness and fever. The diagnosis is difficult and can lead to significant medical morbidity. Treatment of these lesions requires expert surgical management and necessitates an assiduous search for an underlying source. We report a case of a mycotic aneurysm of the extracranial carotid artery in an immunosuppressed patient successfully treated by wide excision and saphenous vein patch angioplasty.

Trigeminal neuropathy secondary to perineural invasion of head and neck carcinomas.

Perineural spread of head and neck cancer is an uncommon cause of cranial neuropathy. We studied five patients with cranial neuropathy resulting from perineural spread of head and neck carcinomas. Trigeminal neuropathy with facial pain or paresthesias was the most common clinical manifestation. MRI in the coronal plane under gadolinium enhancement established the diagnosis by visualization of the lower divisions of the trigeminal nerve. Perineural tumor spread can cause headaches in patients with head and neck cancer.

Hopfield network applied to blood vessel detection in angiograms.

A neural network classifier for detecting vascular structures in angiograms is developed. The classifier consists of a Hopfield network applied to a square window in which the centre pixel is classified from binary information within the window. Tests are performed using a binary test image corrupted by inverting a percentage of the image pixels. The resulting noisy images simulate the output of a detector using a simple threshold derived from local image statistics. The factors affecting the size of window and the choice of stored patterns are discussed. The results are compared with those obtained from a multi-layer perceptron using a similar approach. The Hopfield network is found to be effective at rejecting the high levels of noise that would result from low-contrast source imagery. Another important feature is that the processed image retains an accurate representation of blood vessel diameter.

Improved rate responsive algorithm in QT driven pacemakers--evaluation of initial response to exercise.

The QT pacemaker is a rate modulated pacemaker that uses the evoked QT interval as an indicator to determine its optimal pacing rate. Despite the generally favorable clinical results with this form of pacing, some flaws in the system have been reported, such as the frequently observed rather slow initial response of the pacing rate to physical exercise, and the phenomenon of oscillation of the heart rate. These problems can be attributed to the rate adaptive algorithm used in the current QT pacemaker. Recently, in a reexamination of the relationship between evoked QT interval and pacing rate, a curvilinear relationship between these parameters has been demonstrated. As a result, a new algorithm has been developed for the next generation of the QT pacemaker. Before this new algorithm was implemented in new implantable devices, it was evaluated in a multicenter clinical investigation, with emphasis on the initial response of the pacing rate to exercise. This study was carried out by means of special software in the programmer of the QT pacemaker. By employing real-time bidirectional telemetry, it was possible to submit the study population, consisting of 37 patients with implanted QT pacemakers of the current generation, to identical exercise tests. Comparing these exercise tests, it appears that a considerable gain in speed of response to exercise can be achieved by using the same sensor with a faster reacting, nonlinear rate adaptive algorithm.

[Permanent retention: a justified orthodontic compromise?].

The conditions on which an orthodontic case should be treated to become as stable and as self-retaining as possible are briefly discussed. The correct indications for permanent retention could be found in certain types of malocclusion in which these principles cannot be applied. They are mainly encountered in the treatment of adults and functionally deviating patterns. Moreover, some major problems and disadvantages of permanent lingual retention are mentioned. The terminal responsibility of this procedure towards the referring general practitioner is evaluated and warned for. A short overview of the technical evolution in the field of lingual retention leads to the presentation of a new, more durable modification of the flexible wire system. Some cases treated with this "meshflex retainer" are presented.

Transient tricuspid valve insufficiency following acute inferior myocardial infarction.

Tricuspid valve insufficiency following acute myocardial infarction is uncommon. It may be explained by right ventricular papillary muscle dysfunction. We report the case of a patient with acute tricuspid valve incompetence during the acute phase of a left ventricular inferior infarction with right ventricular myocardial involvement. The treatment during the acute phase consisted of volume loading and positive inotropic agents. Seven days later the clinical signs of tricuspid valve insufficiency and concomitant right heart failure had disappeared. To our knowledge transient tricuspid valve insufficiency following acute inferior myocardial infarction has not been reported previously.

Embolization of a coronary fistula with a controlled delivery platinum coil in a 2-year-old.

Congenital coronary artery disorders are rare. Elective closure is proposed in view of the morbidity reported later in life. Excellent results have been reported with surgery, and recently catheter occlusions have been successful. We report the case of a 2-year-old girl whose coronary artery fistula was occluded with a special controlled delivery coil system.

[Interdigital pilonidal sinus or barber's disease].

A case of barbers disease is presented. The pathogenesis, clinical course and therapy is reviewed. This rare occupational disease must be regarded separate from the postnatal sinus pilonidalis. Therefore the authors suggest a revalorization of the distinguishing name of fistula interdigitalis crinogenita given by G. J. N. Vleugels Schutter in 1963.
