[The use of platysmal myocutaneous flaps in laryngeal reconstruction].

Vertical frontal subtotal laryngectomy, a procedure that can remove as much as 90% of the larynx, was used to produce a functionally acceptable neolarynx with platysmal myocutaneous flaps. Thirty-two patients with laryngeal carcinoma underwent this operation. All flaps survived and there was no necrosis after operation. With the exception of one patient who require nasal feeding because of an irritating cough when swallowing food, all the other patients regain breathing, swallowing and phonating of laryngeal function. The operative methods, advantages and experiences are discussed in this paper.

Interaction of manganese-mesoporphyrin with oleic acid vesicles.

We investigated the interaction between manganese(III)mesoporphyrin (MnMeso), a metalloporphyrin, and liposome membranes containing oleic acid (OA; cis-9-octadecenoic acid). MnMeso associates preferentially with OA but minimally with egg phosphatidylcholine (EPC). Using small unilamellar vesicles, we characterized the MnMeso-OA binding at neutral pH. Our data suggest that MnMeso binds to the OA bilayer with Kd = 6.8 x 10(-4) M; the binding stoichiometry of MnMeso-OA was 1:3.4. This OA-MnMeso interaction was analyzed further for changes in the T1 relaxation property of MnMeso. OA increased the T1 of MnMeso significantly more than did EPC, suggesting that the OA-MnMeso interaction was stronger than that of PC-MnMeso. The side-chain specificity of the OA interaction with this porphyrin derivative was further supported in an experiment with manganese mesotetra(4-sulfonatophenyl)porphine, which lacks hydrophobic side chains for OA interaction. The association of MnMeso with the OA membrane was proposed according to the structure of MnMeso and OA and further verified using electron microscopy. A strong association of MnMeso with OA, an absorption enhancer of the gastrointestinal tract, may be useful for delivery of MnMeso as an oral contrast agent for magnetic resonance imaging.

Extracorporeal circulation for great vessels surgery: a review of 131 cases.

A retrospective analysis of 131 cases of major vascular surgery, mainly aneurysms of the ascending and descending aorta, was undertaken to determine whether use of different bypass methods chosen according to location of the individual lesions resulted in improved results. For the 93 cases of ascending aortic aneurysm, the method for cardiac protection was improved by the use of continuous retrograde coronary sinus perfusion with cardioplegic blood. In sixteen cases with dissection involving the aortic arch, deep hypothermic circulatory arrest and continuous retrograde cerebral perfusion through the superior vena cava was employed for brain protection. The safe brain circulation arrest time was thus prolonged to the longest record of 81 minutes. Among the group of 93 cases, there were four operative and four hospital deaths, giving a mortality rate of 8.6% (8/93). For the 34 cases of descending aortic aneurysms, left heart bypass was employed to avoid ischemia of the heart, lungs, brain, and the abdominal organs; the operative mortality rate was 8.8% (3/34). In three cases of interrupted aortic arch, separate upper and lower body perfusion under deep hypothermia with low flow rate perfusion for the upper body provided the necessary conditions for radical surgery. In one patient with Budd-Chiari syndrome, a total corrective surgery was achieved under right heart bypass.

[Chlamydia pneumoniae infection in patients with pneumonia, bronchitis and acute upper respiratory tract infection in Beijing].

A prospective study for C. pneumoniae infection was conducted in 134 patients with acute respiratory disease and 134 matched patients with other medical reasons from January to May 1994 in Beijing. The positive rate of micro-IF (MIF) IgG antibodies was 93% (125/134) and 89% (119/134) respectively (0.1 < P < 0.25) and the geometric mean titers (GMT) were not found to be significant (55.9 +/- 4.2 vs 41.7 +/- 2.8, 0.1 < P < 0.20). Ten patients, four (4/30) with pneumonia and six (6/81) with bronchitis were serologically diagnosed as having recent C. pneumoniae infection. Chronic bronchitis was statistically more frequent in patients with C. pneumoniae infection than those without (0.025 < P < 0.05). Patients with chronic bronchitis (GMT, 87.1 +/- 3.9; n = 36) tended to have higher IgG titers than those without (GMT, 47.6 +/- 4.1; n = 98, 0.02 < P < 0.05). Culture of retro-pharyngeal swab specimens for C. pneumoniae was negative in the all 134 studied patients.

Experimental study of a new sutureless intraluminal graft with a shape-memory alloy ring.

A new sutureless intraluminal graft has been developed with a ring made of a titanium-nickel shape-memory alloy. The diameter of the ring can be reversibly reduced by compression in 4 degrees C normal saline solution and recovers its original shape in 38 degrees C physiologic saline solution during insertion in the aorta. The new graft was implanted in nine dogs and was evaluated histologically and angiographically at various intervals from 15 to 90 days. No complications, such as dislodgment, aortic rupture, stenosis, or aneurysmal dilation, were observed. The component of the shape-memory ring allows the graft to be introduced and secured in place easily. The function of this ring is better than that of the rigid ring.

[Laryngeal carcinoma with laryngeal abscess].

We had encountered 5 cases of laryngeal carcinoma associated with abscess. Four of them were advanced laryngeal cancer with abscess induced by mechanical injury, and one with post-radiation laryngeal perichondritis & abscess formation. Management included total laryngectomy in 3 patients, partial laryngectomy in 1 patient, and resection of post-radiation abscess in one. Laryngeal abscesses are seen rarely today. Endeavour should be made to decrease such complication. The authors had presented their experience in the management of laryngeal carcinoma associated with laryngeal abscess, together with a review of the history and some of the current ideas about this problem.
