[Biological fixation and clinical results of the cement-free Lübecker total endoprosthesis made from spongiosa metal].

We reviewed 138 primary arthroplasties performed until March 31, 1986 clinically and radiographically. They were followed up for an average of 76 months (60 to 102). 125 (90.6%) hips could be classified as very good/good, 8 (5.8%) as fair and 5 (3.6%) as poor. The average preoperative Harris hip score was 43.6 (12.61) points and the average 6-year score was 91.2 (63-100) points. Two years after implantation, thigh pain associated with the prosthesis was of minor importance (4.4%). Histological and scanning--electron microscopical examinations of the bone--metal interface of retrieved human femoral stems were done. The three-dimensional ingrowth of lamellar and woven bone accompanied by dense fibrous tissue into the "spongiosa" metal, cancellous surface of the implant was demonstrated. The clinical consequence of this mechanical "interlocking" up to a depth of 3 mm, supported by the encouraging clinical results, is evident. Cementless spongiosa metal prosthesises should be implanted in patients older than 70 years if the bone stock will be good enough.

[Nonspecific bacterial spondylitis--an analysis of 32 cases].

With the decrease of tuberculous spondylitis non-specific bacterial spondylitis has got more importance. Regarding the difficult differential diagnosis between non-specific spondylitis, tuberculous spondylitis and tumor the question arises about the role of primary surgical procedure. 32 cases of the orthopedic departments of Brno and Lübeck are demonstrated, in whom surgical intervention has been chosen without exception (exposure with or without spondylodesis). In 19 cases the bacteriological and in all cases the histological findings ensured the diagnosis "non-specific bacterial spondylitis". Diagnostic proceedings corresponded to the descriptions in literature, x-rays (incl. tomograms) and scintigraphy had superior importance, CT had to show the expansion of the inflammatory process into the vertebral canal. Concerning diagnosis, course of disease and duration of therapy blood sedimentation rate and leucocyte count were the dominant parameters. Time of bed-rest and corset-wearing depends on clinical and laboratory findings, antibiotic treatment should be continued until mobilisation and normalisation of clinical and laboratory findings. In one case we had a grave complication during the operation (lesion of v. iliaca), apart from this healing up occurred in all cases within one year, in the course of which radiological stabilisation followed normalisation of clinical and laboratory parameters.

[Fèvre-Languepin syndrome. A case description].

Fèvre-Languepin syndrome is a rare congenital malformation syndrome characterized in particular by the presence of a labio-maxillo-palatal cleft, anomalies of the reproductive organs and a bilateral popliteal pterygium. Findings in a boy who is now four years old are reported. As a result of 14 operations, as well as other therapy, a cosmetically and functionally satisfactory result has been achieved. The principal orthopedic surgical procedures and the postoperative management are described.

Alterations of phosphatidylcholine concentrations in the serum of patients with mammary cancer.

As determined by reversed-phase liquid-solid extraction, concentration of total phosphatidylcholines in the serum of 53 patients with mammary cancer was significantly (P<0.003) increased to 2.24 +/- 0.59 mg/ml when compared with the normal value of 1.86 +/- 0.36 mg/ml. In 31 fractions of their individual molecular species separated by high-performance liquid chromatography there was a shift to compounds composed of longer fatty acids that were significantly more abundant than in normal serum. The concentration of fractions containing stearic, oleic, linoleic, eicosatrienoic, arachidonic and docosahexaenoic acid as well as traces of C17 acids was 2-5 fold enhanced. On the other hand, molecular species containing palmitic acid combined with palmitoleic, oleic linoleic, arachidonic and further poly-unsaturated fatty acids are considerably decreased in cancer serum. All these atypical phosphatidylcholines are apparently specific products of tumor cells and are released into the blood stream. The possibility of using these alterations of phospholipid metabolism for the follow-up of cancer patients is further investigated.
