Intravenous lignocaine pretreatment to prevent intraocular pressure rise following suxamethonium and tracheal intubation.

Intravenous lignocaine (1.5 mg kg-1) was evaluated in patients undergoing intraocular surgery as a means of preventing the rise in intraocular pressure which accompanies tracheal intubation. In patients given either suxamethonium or pancuronium to facilitate tracheal intubation, lignocaine pretreatment conferred no benefit over placebo in preventing the intraocular hypertensive response.

Atracurium and intraocular pressure.

The effect of atracurium on intraocular pressure was studied by comparing it with pancuronium in a randomised controlled trial. The intraocular pressure was measured in patients undergoing cataract surgery before administration of the muscle relaxant, at 1, 3, and 5 minutes after its administration, and at 1 minute after tracheal intubation. Atracurium was found to decrease intraocular pressure to a significantly greater degree than pancuronium. The intraocular pressure after tracheal intubation was found to be significantly higher than that measured immediately after induction of anaesthesia. The authors conclude that atracurium provides an acceptable alternative to pancuronium for ophthalmic surgery but does not overcome the ocular hypertensive effect of tracheal intubation.

Laser trabeculoplasty in the pseudo-exfoliation syndrome.

Because of the high incidence of pseudo-exfoliation in Ireland and because of the difficult problem this group poses regarding control it was decided to treat a group of these patients with the Argon Laser. This paper is a preliminary report of our experience.

The medical profile of cataract patients.

The purpose of this paper is to highlight the high incidence of significant medical problems in an unselected group of patients undergoing cataract surgery and if possible to decide what medical investigations should be carried out prior to operation. 62.5 per cent of our patients had a significant medical problem and 54.5 per cent were receiving medical treatment. Over 50 per cent had abnormal ECGs and chest X-rays. Confusion was the main complicating factor post-operatively and a course of management for these patients is described. Haemoglobin, urea and potassium, a urinalysis, a fasting blood glucose, chest X-ray and an ECG are suggested to delineate the medical status of these patients and to reduce the risks of general anaesthesia in this group.

Slit lamp ophthalmodynamometry.

A short clinical trial of the 'Dynoptor' Slit Lamp Ophthalmodynamometer has shown it to be a practical instrument. A mean O.A.P. of 52.3 mm/hg has been recorded.
