[A vascular pedicled duplicated fibula transplant for treatment of lumbar instability following tuberculous spondylodiscitis].

Operative treatment of tuberculous spondylodiscitis is still an important part of the treatment for lumbar spine instability. We report on a patient who suffered an extensive relapse with microbiological confirmation of tuberculous spondylodiscitis following operative spinal treatment for unspecific spondylodiscitis. X-Ray examination showed development of pronounced lumbar instability, which was first treated with the aid of an external fixateur and later by means of a doubled fibular bone graft with a vascularised stem with no dorsal instrumentation, which led to bony consolidation.

Influence on mitochondria and cytotoxicity of different antibiotics administered in high concentrations on primary human osteoblasts and cell lines.

Osteomyelitis, osteitis, spondylodiscitis, septic arthritis, and prosthetic joint infections still represent the worst complications of orthopedic surgery and traumatology. Successful treatment requires, besides surgical débridement, long-term systemic and high-concentration local antibiotic therapy, with possible local antibiotic concentrations of 100 microg/ml and more. In this study, we investigated the effect of 20 different antibiotics on primary human osteoblasts (PHO), the osteosarcoma cell line MG63, and the epithelial cell line HeLa. High concentrations of fluoroquinolones, macrolides, clindamycin, chloramphenicol, rifampin, tetracycline, and linezolid during 48 h of incubation inhibited proliferation and metabolic activity, whereas aminoglycosides and inhibitors of bacterial cell wall synthesis did not. Twenty percent inhibitory concentrations for proliferation of PHO were determined as 20 to 40 microg/ml for macrolides, clindamycin, and rifampin, 60 to 80 microg/ml for chloramphenicol, tetracylin, and fluoroquinolones, and 240 microg/ml for linezolid. The proliferation of the cell lines was always less inhibited. We established the measurement of extracellular lactate concentration as an indicator of glycolysis using inhibitors of the respiratory chain (antimycin A, rotenone, and sodium azide) and glycolysis (iodoacetic acid) as reference compounds, whereas inhibition of the respiratory chain increased and inhibition of glycolysis decreased lactate production. The measurement of extracellular lactate concentration revealed that fluoroquinolones, macrolides, clindamycin, rifampin, tetracycline, and especially chloramphenicol and linezolid impaired mitochondrial energetics in high concentrations. This explains partly the observed inhibition of metabolic activity and proliferation in our experiments. Because of differences in the energy metabolism, PHO provided a more sensitive model for orthopedic antibiotic usage than stable cell lines.

The implications of 18F-FDG PET for the diagnosis of endoprosthetic loosening and infection in hip and knee arthroplasty: results from a prospective, blinded study.

BACKGROUND: The most frequent complications of joint arthroplasty are septic or aseptic loosening of endoprostheses. Preoperative differentiation is essential, since very different treatment methods result from the diagnoses. The aim of the current study was to evaluate the clinical value of 18F-Fluoro-deoxyglucose positron emission tomography (18F-FDG PET) as a diagnostic modality for inflammation and loosening in hip and knee joint prostheses. METHODS: 18F-FDG-PET examinations and multiphase bone scan were performed on hip and knee endoprostheses in 27 patients prior to revision surgical procedures planned for prosthetic loosening. Intact prostheses were found at the opposite site in some patients so that additional 9 joints could be examined with the field of view of 18F-FDG PET. Verification and valuation of the PET and scintigraphic image findings were conducted by comparing them with information combined from intraoperative findings, histopathology, and microbiological investigations. RESULTS: Evidence of loosening was correctly determined in 76.4% of cases using 18F-FDG-PET, and in 75% of cases using bone scan. The detection of periprosthetic inflammation using 18F-FDG-PET had a sensitivity of 100% for septic cases and of 45.5% in cases of increased abrasion and aseptic foreign-body reactions. However, reliable differentiation between abrasion-induced and bacterial-caused inflammation was not possible using 18F-FDG-PET. CONCLUSION: 18F-Fluoro-deoxyglucose positron emission tomography (18F-FDG-PET) allows reliable prediction of peri-prosthetic septical inflammatory tissue reactions. Because of the high sensitivity of this method, a negative PET result in the setting of a diagnostically unclear situation eliminates the need for revision surgery. In contrast, a positive PET result gives no clear differentiation regarding the cause of inflammation.

[Dynamic ultrasound examination of the shoulder of golf players].

Although golf is becoming popular it can result in injury, usually from overuse and from poor technique. The shoulder is a commonly affected site, with the lead shoulder (or the left shoulder in the right-handed golfer) vulnerable to injury. With this study we tried to figure out any hyperlaxity of the lead shoulder using ultrasonography. 33 golf-players were investigated by questionnaire, clinical examination and dynamic ultrasonography. Neither clinically nor by using ultrasonography hyperlaxity of the shoulder was found. Nevertheless hyperlaxity followed by secondary impingement should be considered.

[Magnetic resonance tomogography after cryotherapy of long bones in an animal model].

PURPOSE: To describe the MR findings following cryoablation of long bones. MATERIALS AND METHOD: Cryoablation was performed in femoral and tibial bones of 24 sheep under general anesthesia. MRI of the treated and untreated contralateral bones was performed immediately thereafter and at 2, 4 and 6 months after the cryosurgical procedure. RESULTS: On the MRI performed immediately after cryotherapy, the lesions showed low signal intensities relative to the normal bone marrow on unenhanced T1- and T2-weighted images. At 2, 4 and 6 months after cryoablation, the lesions showed high signal intensities on STIR images, low signal intensities on T1-weighted and heterogeneous enhancement on contrast-enhanced T1-weighted MR images. The femoral lesions decreased in size from 31 +/- 3 mm immediately after the cryotherapy to 13 +/- 4 mm 6 month later and the tibial lesions from 29 +/- 7 mm to 19 +/- 4 mm. CONCLUSION: MRI shows bone marrow lesions immediately after cryotherapy and can easily monitor healing lesions. MR imaging is suitable for following cryotherapy.

[Isokinetic testing of the shoulder of handball players].

Aim of the study was to evaluate an isokinetic profile testing handball players to describe muscular imbalance. 30 athletes (15 male and 15 female) were measured at the Cybex 6000 test-unit for concentric internal and external rotation. Both shoulders, throwing arm (dominant shoulder) and the opposite shoulder were tested by speeds at 60 degrees/s and 180 degrees/s. There were sex specific differences which are also found in other sports. But these contrary data are not useful to define effective training concepts for handball-players with shoulder problems. From our point of view isokinetic testing of the shoulder joint in handball players can not reach any sufficient result. From our point of view there is no reason for using isokinetic testing to evaluate shoulder problems.

Iatrogenic paraplegia in spinal surgery.

INTRODUCTION: Paraplegia as a result of a surgical spinal procedure is a rare complication. The risk cannot be precisely quantified due to the lack of current data. The aim of this study was to record a sufficiently large number of major spinal operations, especially extended methods in scoliosis surgery. Hereby, a reliable statement regarding the risk of severe neurological complications with these surgical techniques should be possible. First, a retrospective analysis of patients from a German spine centre (spinal fusion) and a survey of 17 German centres of spinal surgery were conducted for the retrospective acquisition of severe iatrogenic neurological complications. MATERIALS AND METHODS: The study included 1194 patients who underwent a spinal fusion during the period 1992-2002. The incidents of postoperative paraplegia are described in detail, and case studies done. Possible causes, methods of intraoperative monitoring and options of therapy are discussed according to research in relevant publications. Additionally, severe neurological complications of 3115 spinal operations were recorded in a standardised survey conducted throughout major German spinal centres. RESULTS: Of the 1194 patients surveyed, 7 (0.59%) experienced a postsurgical complete or incomplete paraplegia. In 3 of the recorded cases, the cause could be identified. The survey of 3115 scoliosis surgeries showed that iatrogenic paraplegia occurred with a frequency of 0.55%. The risks associated with short spinal fusions (0.14%), cervical discectomies (0.07%) and lumbar discectomies (0.03%) are considerably less. CONCLUSION: Operative treatment of scoliosis with a high degree of correction carries a risk of neurological complications of about 0.5%. Mechanical as well as ischaemic damage to the spinal cord can be detected early by means of consistent intraoperative neuromonitoring.

Rate of perioperative complications in thermal ablation of bone: an animal trial.

INTRODUCTION: While only few data are available yet for radioablation of bone tissue, the occurrence of bone marrow embolisms during cryoablation has been documented. It was the aim of this study to assess perioperative complication rates of thermoablation in animals using state-of-the-art ablation probes. MATERIALS AND METHODS: Eight adult sheep were placed under general anaesthesia, and using a radioprobe, thermoablation was performed on the medial side of the head of the right tibia. In addition, freezing was performed with miniature cryoprobes on both femurs and the head of the left tibia. Haemodynamic monitoring and determination of blood gases and electrolytes were done intraoperatively. The animals were killed 1 week after surgery, and the ablation sites and lung tissue examined macroscopically and microscopically. RESULTS: None of the animals showed pathological changes in any of these parameters. Blood gases remained unremarkable throughout the operation. All eight animals showed an average drop in haemoglobin of 0.97 g/100 ml. Two animals showed fresh embolisms in the pulmonary vessels. CONCLUSION: Experimental thermoablation of bone tissue in large animals failed to show any significant perioperative complications following surgery. The lung embolisms which did occur were not clinically relevant. The use of mini-cryoprobes or radioprobes as alternative or complementary measures for treating pathologically altered bone tissue seems viable and does not involve any undue risks.

[Changes in stability after cryosurgical treatment of long tubular bones. An animal experiment study].

The incidence of spontaneous fractures after cryosurgical treatment is described in the literature. The purpose of this study in the sheep model was to analyze the possibility of minimizing the potential risk of bone failure using a new miniature cryoprobe with minimal tissue traumatism and exact control of the ablation. In each of 24 sheep ablations at the right femur and left tibia were performed by drilling. The ablation at the femur was restricted to an area of 2 cm(2) of only one cortical bone, whereas at the proximal tibia the whole tibial plateau was included. The opposite side, which was treated with analog drillings without cryoablation, served as control. The ultimate bending strength of the femur and the ultimate compression strength of the tibia were examined 2, 4, and 6 months after the operation. After 2 months there was a significant difference ( p<0.05) in the ultimate compression strength between the treated and untreated tibiae, whereas the ultimate bending strength of the treated femora tended to be lower. After 4 and 6 months the side treated with cryosurgery was only marginally weaker than the untreated side. Spontaneous fractures were not observed during the whole experimental period. The good controllability of the freezing procedure and the low iatrogenic weakening of the bone using a modern miniature cryoprobe minimizes the risk of pathological postoperative fractures. After ablation of larger bone sections, the treated extremity should be partially unloaded or managed by osteosynthesis for at least 3 months.

[Minimally invasive approaches to hip and knee joints for total joint replacement].

The manuscript features the different minimally invasive approaches to the hip for joint replacement. These include medial, anterior, anterolateral, and posterior approaches. The concept of minimally invasive hip arthroplasty makes sense if it is an integral part of a larger concept to lower postoperative morbidity. Besides minimal soft tissue trauma, this concept involves preoperative patient education, preemptive analgesia, and postoperative physiotherapy. It is our belief that minimal incision techniques for the hip are not suited for all patients and all surgeons. The different minimally invasive approaches to the knee joint for implantation of a knee arthroplasty are described and discussed. There have been no studies published yet that fulfill EBM criteria. The data so far show that minimally invasive approaches and implantation techniques for total knee replacements lead to quicker rehabilitation of patients.

[The minimally invasive unicompartmental knee system "Preservation"First clinical results and analysis of complications].

The Preservation uni knee system is a minimally invasive procedure for implantation of a unicompartmental knee prosthesis. The early results show that the rehabilitation process is quick; the postoperative pain and blood loss is less than after conventional knee arthroplasty. Still there are some pitfalls. The implantation of the tibial mobile-bearing component requires a sound operative technique; otherwise complications will occur. The combination of the available navigation system with the Preservation uni will improve operative accuracy.

[Injuries and muscle tightness in soccer].

Soccer is one of the most popular sports worldwide. Due to its characteristic many injuries are expected. Muscular tightness is postulated as an intrinsic risk factor for the developement of a muscle injury. Muscle flexibility testing can identify soccer players at risk for musculoskeletal lesions. One-hundred-and-seventeen male amateur soccer players aged 4 to 29 were examined for former injuries and muscular flexibility by using Janda's "functional muscular diagnostic". Muscle injuries and ankle lesions predominated among the injuries. The "functional muscular diagnostic" is suitable for screening muscular flexibility in soccer players. You found comprehensive muscular tightness among the soccer players. The value of stretching for preventing exercise-related muscle injury is still unclear.

Retro-pharyngeal obstruction in association with osteophytes of the cervical spine.

We describe five patients with cervical spondylosis and large anterior osteophytes causing pharyngeal compression. All had dysphagia, two had obstructive sleep apnoea and another two had dyspnoea and stridor on inspiration. One, with perforation of the pharynx, required emergency tracheostomy. Only three had pain in the neck or arm. Compression of the retroglottic space was confirmed in all patients by pharyngoscopy and in all the symptoms were relieved by excision of the osteophytes. Three also underwent intervertebral fusion. One had some persistent sleep apnoea.

[Treatment course in endoprosthetic knee replacement--a systematic insurance cost calculation exemplified by a concrete example].

In view of the planned introduction of the DRG system in Germany, this paper gives the reader a guideline on how to analyse costs and productivity of medical procedures. Total knee replacement was chosen because it is one of the standard operations in orthopedic surgery with existing special calculations for this type of operation. By using the example of a total knee replacement, productivity phases are used and their costs are estimated. With these phases the clinician is able to analyse the efficiency of his department and a benchmarking is possible. Nevertheless, we should not forget that although cost reduction in hospitals is essential the patients' health is the main aim of treatment.

[Attachment points of the posterior longitudinal ligament and their importance for thoracic and lumbar spine fractures].

Spine fractures with damage of the posterior wall of the vertebra often can be anatomically reconstructed by indirect reduction. Whether the posterior longitudinal ligament (PLL) is responsible for the reduction is still subject to debate. The aim of our investigation was to ascertain the role of the PLL in closed reduction of spine fractures by identifying the bony attachment points of this ligament. We performed a gross anatomical dissection, a light- and polarized microscopic investigation on 22 human cadaverous thoracic and lumbar spines to determine the points of attachment of the PLL. We found two layers of the PLL. The superficial layer runs from the first thoracic down to the third lumbar vertebra with a width of 0.4-1.0 cm and from there descends as a thin rudiment to the sacrum. The deep layer shows a segmental rhomboid structure. Lateral fibers are attached to the annulus fibrosus and at the rim of the adjacent vertebrae. Medial fibers are attached additionally to the posterior wall of the vertebral bodies by bridging the foramina basivertebralia. Since these foramina become enlarged in the caudal parts of the vertebral column, the number of attachment points at the posterior wall of the vertebral bodies decreases caudally. Good results for reconstruction of the posterior wall in vertebral fractures of the thoracic and upper lumbar spine can be explained by the anatomical situation of the PLL and stress the important role of the PLL in indirect reduction of spine fractures.

Cryosurgery in long bones with new miniature cryoprobe: an experimental in vivo study of the cryosurgical temperature field in sheep.

AIM: The aim of this in vivo study was to determine whether new miniature cryoprobes provide adequate tissue cooling in long bones by measuring the field of temperature under various conditions. METHODS: Freezings were performed in femoral and tibial bones of 10 sheep under general anaesthesia. Applying one cryoprobe, temperatures of -75 degrees C resp. -51 degrees C could be reached within 0.75 cm resp. 1.00 cm of the probe. RESULTS: Histological examinations revealed compact bone and marrow necroses along the isotherm in all 10 sheep. Using two cryoprobes simultaneously, a mean temperature decrease to -71 degrees C between the two cryoprobes was achieved thanks to the synergistic freezing effect. Looking at consecutive freezes, it was apparent that with similar cryoprobe end temperatures, the temperature dropped faster if the number of freezing cycles was increased. CONCLUSION: In conclusion, it was seen that despite its small diameter, the new miniature cryoprobe delivers adequate in vivo tissue cooling in long tubular bones. Employing the synergistic freezing effect by using two or more cryoprobes simultaneously, efficient in vivo freezing of larger bone segments is also possible. Thus, cryosurgery with the new miniature probes can provide a valuable complement to conventional resection of long tubular bones, and offers a viable alternative to surgical treatment of neoplastic diseases of the skeletal system.

[Results of shoulder isokinetic testing in volleyball players].

Isokinetic testing of the shoulder joint shows valuable indication to possible functional disorders and muscle imbalance. Concentric testing of several parameters using the Cybex 6000 practice unit was done by professional volleyball players performing external and internal rotation of the dominant and non-dominant shoulder at speeds of 60 degrees/s an 180 degrees/s. Sex-specific significant differences were shown which are comparable to other kind of sports.

Metastasizing chordoma of the lumbar spine.

Chordoma is a dysontogenetic bone tumour that appears in the region of the axial skeleton. Its malignant transformation has been underestimated in the past, since the incidence of metastasis is disputed. This paper describes a chordoma of the fifth lumbar vertebra, which at first was monolocular, but in the course of the disease led to a diffuse metastasis of the spine. Specific histological findings and the differential diagnosis of the chordoma are discussed, as well as previous descriptions in the literature relating to course, diagnosis, and therapy.
